Computer-based information systems feature in almost every aspect of our lives, and yet most of us receive handwritten prescriptions when we visit our doctors and rely on paper-based medical records in our healthcare. Although electronic health record (EHR) systems have long been promoted as a cost-effective and efficient alternative to this situation, clear-cut evidence of their success has not been forthcoming. Background: Ventilator associated pneumonia is one of the leading hospital acquired infections associated with increased mortality and lengths of stay in mechanically ventilated patients.

Hypothesis: It is hypothesized that the introduction of chlorhexidine gluconate will show reduction in the incidences of ventilator associated pneumonia and a greater reduction with tooth brushing vs oral swabbing. It is also hypothesized that the electronic health record will provide more evidence of quality of care than an external EHR sources.

The Institute of Medicine report suggests medical errors account for up to 98,000 patient deaths each year. Therefore, the US healthcare system is looking to information technology applications as one means of making patient care safer. The adoption of health information technology and the meaningful use of electronic health records is a byproduct of the 2009 American Recovery and Reinvestment Act (ARRA). One measure of ARRA is the Health Information and Technology for Economic and Clinical Health (HITECH) Act which authorizes the Centers for Medicare and Medicaid Services (CMS) to provide monetary incentives to hospitals and providers who demonstrate meaningful use of certified electronic health records (EHRs).
