We study the design of a healthcare appointment system with a single physician and a group of patients whose service durations are stochastic. The challenge is to find the optimal arrival sequence for a group of mixed patients such that the expected total cost of patient waiting time and physician overtime is minimized. While numerous simulation studies report that sequencing patients by increasing order of variance of service duration (Smallest-Variance-First or SVF rule) performs extremely well in many environments, analytical results on optimal sequencing are known only for two patients. The current work examines a contemporary workplace stressor that has only recently been introduced into the literature: illegitimate tasks. Illegitimate tasks are work tasks that violate identity role norms about what can reasonably be expected from an employee in a given position. Although illegitimate tasks have been linked to employee well-being in past work, we know little about the potential explanatory mechanisms linking illegitimate tasks to work-relevant negative psychological states. Using a sample of 213 US-based employees of mixed occupations and a cross-sectional design, the present study examines job satisfaction and intrinsic motivation as outcomes of illegitimate tasks. The state of Maryland, in collaboration with the Centers for Medicare and Medicaid, developed the first all-payer system model in the U.S. in 1971, and some 35 years later in response to financial pressures, modernized this program. The focus of the modernized program was to improve overall per capita expenditure, quality of care, and the outcome of Marylanders’ health.

Conclusion

Most rural Nebraskans own their home. And, when asked about housing programs or priorities they would support in their community, most rural Nebraskans support programs that help seniors age in their homes, programs that would help upgrade the condition of existing homes and providing affordable rental housing. They are less likely to support organizing volunteer efforts to maintain existing housing stock, but still one-half support this option as well.

Conclusion

By many different measures, rural Nebraskans are positive about their community. Many rural Nebraskans rate their community favorably on its social dimensions: as friendly, trusting and supportive. Most rural Nebraskans say it would be difficult to leave their community. And, most rural Nebraskans disagree that their community is powerless to control its future.

Medicare fraud and abuse costs are estimated at 3%-10% of overall Medicare spending, which is expected to expand considerably until 2024 and as such the amount of fraud dollars would be expected to increase proportionally. Study Design—The effects of participant characteristics along with descriptions of pain, and psychological involvement, such as fear avoidance, were assessed using structural equation modeling to identify relationships between these factors and disability as a result of low back pain.

Created by the Affordable Care Act (ACA), Consumer Operated and Oriented Plans (CO-OPs) are private, non-profit health insurers that were designed to increase insurance plan choice and lower premiums in the Health Insurance Marketplaces. Early analyses of the ACA suggested that CO-OPs may be particularly beneficial for rural communities, where fewer individual and small group health insurance options have traditionally been available.

OBJECTIVE: To investigate how food commercials influence children's food choices.

STUDY DESIGN: Twenty-three children ages 8-14 years provided taste and health ratings for 60 food items. Subsequently, these children were scanned with the use of functional magnetic resonance imaging while making food choices (ie, "eat" or "not eat") after watching food and nonfood television commercials.

RESULTS: Our results show that watching food commercials changes the way children consider the importance of taste when making food choices. Children did not use health values for their food choices, indicating children's decisions were largely driven by hedonic, immediate rewards (ie, "tastiness"); however, children …

Most rural Nebraskans have elementary, middle and high schools in their community or nearest community. Most of the schools available are public but many rural Nebraskans report having both public and private schools of each grade level. Long waiting times in emergency departments (EDs) not only reduce patients’ perceived quality of care, but also increase crowding which can adversely affect patients’ outcomes. Waiting time has been found to affect patients’ outcomes and is closely associated with delays in the provision of ancillary services to ED patients by the diagnostic/treatment laboratories. The paper examines the relationship between farm animal welfare, industrial farm animal production, and human health consequences. The data suggest that when the animal welfare of land-based farm animals is compromised, there are resulting significant negative human health consequences due to environmental degradation, the use of non-therapeutic levels of antibiotics for growth promotion, and the consequences of intensification.
