Business sustainability is essential for any nonprofit organization. Revenue diversification can play a key role in business sustainability. In this case study, the revenue diversification practices and challenges of an international, behavioral health, nonprofit organization based out of the western United States were examined. The organization provides training, support, and resources in an experiential modality of behavioral health treatment for clients worldwide. Data collection processes consisted of a review of current revenue diversification literature, semistructured interviews with three senior-level leaders in the organization, and a review of select organizational documents. Although nonprofit organization (NPO) leaders play crucial roles in society, financial distress and vulnerability are common for many NPO leaders, with some NPOs closing as a result of these conditions. The purpose of this single-case study was to explore the diversification strategies used by 10 leaders and senior staff of an NPO in the mid-Atlantic region of the United States through the conceptual lens of Markowitz's modern portfolio theory. Facing government funding cuts, U.S. nonprofit leaders have had to seek other revenue streams to remain operable and ensure that the clients they serve continue to receive support. Nonprofit hospitals are under increased pressure to maintain financial stability and compliance with Internal Revenue Service (IRS) net community benefit requirements. Boards of directors are not always confident that the compensation packages awarded to executives stimulate them to act in the organization's best interest. The principal-agent theory formed the basis of this correlational study. Archival data from National Center for Charitable Statistics, Guidestar, and the Center for Medicare & Medicaid Services were collected from 117 nonprofit urban hospitals for the fiscal year 2013.
