Midwifery Group Practice (MGP) is a model of care designed to provide 'low risk' pregnant women with continuous care by one midwife throughout pregnancy, labour and birth and the early postnatal period. A one-year trial of this model concluded in June 2005 at Wollongong Hospital.

Evaluation of the MGP program focused on clinical outcomes and consumer acceptance. Data sources included peer reviews of selected cases and satisfaction surveys of the women who took part. During the antenatal, intrapartum and postpartum periods, midwives noted any deviations from normal and followed specified criteria for consultation and referral to other health professionals. As outlined in ABF Information Paper Number 1, a critical element of Activity Based Funding (ABF) is the need to define, classify, count, cost and pay for each health care activity in a consistent manner. For acute patients who stay in hospital for more than a day, this is not an issue. These patients are classified as acute ‘overnight admitted patients’ and are counted by Diagnosis Related Group (DRG).

This paper investigates why (potential) primary care patients attend an ED rather than a GP. An understanding of why patients make decisions such as this is critical if the health system is to be better positioned to meet the increasing demand for improved services and outcomes. We found that the rate of potential primary care presentations varies greatly by age and by sex and that the pattern of primary care presentations is different to that of other ED presentations. Activity-Based Funding (ABF) is a central feature of the Rudd hospital reform plan (‘A National Health and Hospitals Network for Australia’s Future’). ABF is not a new idea, either to the health sector or to other industries. Indeed, the Council of Australian Governments (COAG) agreed in November 2008 to introduce a nationally consistent ABF approach as part of its National Partnership Agreement on Hospital and Health Workforce Reform. The Rudd hospital reform plan proposes to accelerate the ABF work program that has already been agreed with the States and Territories.

This paper examines trends in potential ‘primary care’ presentations at EDs, comparing these with other ED presentations and to primary care attendances in the community.MethodsThe study draws on EDIS data (Emergency Department Information System), which at December 2005 covered 76 per cent of attendances in New South Wales, and MBS data from Medicare Australia. Annual counts of potential ‘primary care’ presentations to EDs are compared with those of other ED presentations and to primary care presentations in the community.
