Many professional organizations publish clinical practice guidelines for the use of probiotics in gastrointestinal disorders. Generally, no two guidelines align perfectly, and some differ markedly. These discrepancies occur because clinical practice guidelines are not purely mechanical outputs of data synthesis. They are shaped by structured interpretation, value judgments, and predefined thresholds for uncertainty. These elements become particularly visible when effect sizes are small, heterogeneity is high, and outcomes vary in clinical relevance-as is the case with probiotic research. This article examines why discordance among probiotic guidelines is predictable and provides practical guidance for clinicians who must decide among conflicting recommendations …

Background: Dietary management is key to preventing and controlling type 2 diabetes mellitus (T2DM). However, few studies have examined differences in Healthy Eating Index (HEI-2020) scores and components among individuals with and without diagnosed T2DM. This study aimed to assess gender differences in HEI-2020 scores and its 13 components by T2DM status. Methods: Using National Health and Nutrition Examination Survey (NHANES) data from 2011 to 2018, this cross-sectional study analyzed 19,464 adults. Black birthing people in the United States continue to experience unequal maternal health outcomes, shaped in part by racism and barriers to nutritious food, culturally relevant health education, and supportive community spaces. While prenatal nutrition education and food assistance programs can provide important resources, information alone cannot address the challenges many people face when trying to access healthy and affordable food during pregnancy. Childhood Iron Deficiency Anemia (IDA) remains a major public health challenge in Egypt, where its persistently high and rising prevalence carries substantial consequences for child growth, cognitive development, immunity, and long-term health outcomes. US Army body composition policies (AR 600-9 and recent directives) are essential for military readiness, injury prevention, and force health protection, yet implementation gaps persist, particularly with the 2026 Department of Defense (DoD)-mandated waist-to-height ratio (WHtR < 0.55) transition. This DNP project evaluated policy implementation effectiveness at Brooke Army Medical Center using the W.K. Kellogg Foundation Logic Model and CFIR 2.0 framework. Analysis of 2022–2025 de-identified administrative data and 16 stakeholder questionnaires identified ≥10 implementation gaps, including low awareness of new standards and workload impacts. The project developed 8 evidence-based recommendations and an implementation roadmap to improve compliance, optimize resources, reduce administrative burden, and strengthen military readiness.

Background: Nil per os (NPO) guidelines are utilized for patient safety during procedures to reduce the risk of pulmonary aspiration. The American Society of Anesthesiologists (ASA) recommends clear liquids up to 2 hours before non-emergent procedures, although many facilities still implement the outdated practice of ordering NPO after midnight. This practice can lead to prolonged fasting, which may cause increased hunger, thirst, hypotension, nausea, and dizziness, ultimately resulting in patient dissatisfaction and discomfort.

Hydroponics is a popular method of sustainable plant cultivation where plant roots are suspended over a liquid nutrient solution. Fruit production in Utah can be challenging due to local climate and soil conditions. Tart cherries and apples are major fruit crops in Utah, and improvements to these cropping systems would be beneficial for farmers and local consumers. Two experiments were conducted to evaluate desired improvements for these crops. The first study looked for salt tolerance in ten new apple rootstocks. Salty soils can be found throughout the state limiting the land suitable for apple production. When grown in salty soils apple trees often have reduced growth and production potential. Purpose of Review: This narrative review provides discussion on current evidence regarding the relationship between sodium intake and cognitive function in animal and human studies, as well as potential mechanisms underlying this relationship. Recent Findings: Recent evidence suggests high sodium intake is associated with development of cognitive impairment and neural dysfunction. Additionally, studies have proposed that high sodium intake is associated with increased aggregation of Aβ-amyloid and that hypertension (for which high sodium intake is a risk factor) modulates the relationship between Aβ-amyloid and development of cognitive impairment and Alzheimer’s disease. Summary: This cross-sectional study of 300 older adults (aged ≥ 50 years) found that less education, lower income, and health care card ownership are associated with reduced bone, muscle, and physical function measures. This underscores the need for targeted preventive strategies for osteoporosis and sarcopenia that address socioeconomic-related disparities. Purpose: The prevalence of chronic diseases follows a social gradient, although this is unclear in musculoskeletal conditions. This study evaluated the feasibility of using woodchips from the invasive Pyrus calleryana (Callery pear) as substrate for mushroom cultivation. Pleurotus ostreatus, Hericium erinaceus, and Ganoderma lucidum were grown on a pear wood substrate and on a conventional oak substrate to compare, yield, sensory attributes, and metabolite production. P. ostreatus showed no significant differences in yield or biological efficiency (p > 0.05), only sweetness (p = 0.022) differed significantly. H. erinaceus yield and efficiency were similar (p > 0.05), with greater second-flush yields in oak controls (p = 0.034) but no sensory differences (p > 0.05). Background: Systemic lupus erythematosus (SLE) disproportionately affects African American women and contributes to racial health disparities in the United States. Although genetic susceptibility influences disease risk, growing evidence suggests that social determinants of health (SDOH), including neighborhood conditions, structural racism, and socioeconomic disadvantage, contribute to disease development and progression. Epigenetic mechanisms may provide a biological pathway linking adverse social conditions to chronic disease disparities. This dissertation examined the influence of SDOH on SLE disparities through evidence synthesis and empirical investigation to inform public health practice and policy.

Many mycorrhizal fungi are hypogeous (truffle-forming), producing underground fruiting bodies. Studying truffles is inherently challenging due to their hidden nature. However, mycophagous mammals provide a unique opportunity to study hypogeous fungi because they rely on truffles as a food source. Although several truffle species have recently been described in the southern Appalachian region, their fruiting patterns remain poorly understood. To address this, we quantified truffle richness and spore loads from 670 Peromyscus scat samples collected across 42 sites spanning an environmental gradient during 2024 in the Blue Ridge Mountains of East Tennessee. The external school food environment, comprising food outlets and marketing near schools, shapes what young people eat. Yet little is known about how Australian adolescents perceive and navigate these environments. Focus groups were conducted with 31 school students (Years 7–12) from Perth, Western Australia, to explore their experiences of the external school food environment and its influence on their food-related behaviours. Transcripts were thematically analysed and six themes were identified. Accessibility emphasised proximity, transport links, opening hours and speed of service, with older students using outlets as social spaces. Background: Prebiotic dietary supplementation has been shown to improve glucose homeostasis in type 2 diabetes patients. The aim of this analysis was to determine whether pre-pregnancy body mass index (BMI) modifies the effect of prebiotic supplementation from mid-pregnancy on reducing the risk of gestational diabetes mellitus (GDM). Methods: In a double-blinded, randomised controlled trial, pregnant women < 21 gestational weeks were randomly assigned (1:1) to consume daily prebiotics (14.2 g galacto-oligosaccharides and fructo-oligosaccharides) or placebo (8.7 g maltodextrin) powder. An effect modification analysis was performed to assess the heterogeneity of the effect of prebiotic supplementation in relation to pre-pregnancy BMI on GDM diagnosis. Results: Between June 2016 and November 2021, 329 women were assigned to the prebiotic group (50.4%), and 323 (49.5%) were assigned to the placebo group. Overall, 288 of 652 women (44.2%) were classified as overweight/obese prior to pregnancy (BMI ≥ 25 kg/m2). The distribution was balanced, with 146/329 (44.4%) randomised to the prebiotics group and 142/323 (44.0%) in the placebo group. Pre-pregnancy BMI modified the effect of prebiotic supplementation, with the intervention reducing GDM rates in women with a BMI ≥ 25 kg/m2 …

This systematic review and meta-analysis collated results from three previous reviews to evaluate the predictive discriminative performance of dementia risk models when validated in population-based settings. Background: Transitioning from independent living to a residential care facility often results in a significant loss of independence, autonomy, and connectedness, leaving new residents with profound feelings of loss and grief. In light of these challenges, this study explored the potential benefits of a tailored in-house exercise program for new residents. To examine, among people with type 2 diabetes (T2D), whether cardiovascular disease (CVD) phenotypes and cardiovascular medications are associated with any fracture as the primary outcome and major osteoporotic fracture (MOF) as a secondary outcome. Nationwide registry-based matched case–control study in Denmark (2013–2021) among people with T2D. Cases were individuals with T2D who sustained an incident fracture (n = 12,390), and controls were fracture-free individuals with T2D matched 1:3 by age, sex, T2D onset date, and weighted Charlson Comorbidity Index (n = 37,170). Objective: Police force recruits have a high musculoskeletal injury burden, which results in a substantial economic burden and can lead to attrition. The objective of this study was to identify and prioritise the strategies perceived as important and feasible to reduce the prevalence, incidence, and burden of injury in police force recruits. Design: Mixed-methods concept mapping study. Background: Legumes can substitute for meat and are thus a particularly important food group for environmental sustainability. As the use of legumes among Finnish children is marginal, there is potential for a substantial increase in their consumption. However, food education delivered through early childhood education and care (ECEC) settings has rarely focused specifically on environmentally sustainable foods, such as legumes. Background: The proliferation of fast-food outlets (FFOs) in residential areas has raised growing concerns about their impact on community health, safety, and wellbeing. This qualitative study examined both pre-development community consultation submissions and post-development resident experiences surrounding the development of a McDonald’s outlet in a newly established suburb on the urban fringe of Perth, Western Australia (WA). Methods: Using a qualitative descriptive design, the study analyzed 183 community submissions lodged with the local council prior to development and conducted semi-structured interviews with 11 residents living within 300 m of the outlet after construction. Background: Provision of vascular imaging results has been investigated to prompt changes towards healthy lifestyle behaviours, but effects on body composition and muscle health are unknown. Indigenous Alaskans rely on subsistence hunting for food, with Barren-ground caribou making up approximately 27% of their diet. It is believed by subsistence hunters that non-local hunting is forcing a change in caribou migration behavior. This study examined non-local harvesting records from 2013 – 2025 in the northwest and interior Game Management Units (GMU) and Alaksa’s Western Arctic Caribou Herd (WACH) migration range data to answer the question, is there a spatial relationship between non-local harvests and WACH migration behavior. The development of plant-based meat analogues (PBMAs) that accurately replicate meat texture and flavor remains limited by fundamental structural differences between plant and animal proteins. Most notably, the globular conformation of plant proteins contrasts with the fibrous anisotropy of animal muscle, thereby limiting viscoelasticity. Additional challenges include beany off-notes arising from lipid oxidation products such as hexanal and anti-nutritional factors (ANFs) that reduce bioavailability. Problem/Research Question: Taste alteration affects up to 70% of oncology patients receiving chemotherapy, leading to decreased dietary intake, malnutrition, and poor quality of life. It is often under reported due to lack of taste assessments during toxicity screening and has no known treatment. Researchers have shown that breast cancer patients were able to assess and control alterations with a researcher-delivered intervention. Research is needed to translate this promising strategy for clinical use with populations beyond breast cancer.
