The ethics of perinatal care, and the experiences of families who receive such care, remains a nascent area of inquiry. It can be hard to see how existing "good death" constructs apply to the experiences of fetal patients and their families. In this paper, we explore two themes raised by a case at our fetal health center: anticipation and accompaniment. In this case, a mother presented to our fetal health center; her unborn son, our fetal patient, was diagnosed with life-threatening hypoplastic left heart syndrome and endocardial fibroelastosis. Impact Of Ischemic Time On Functional Exercise Capacity After Orthotopic Heart Transplantation, Katelyn D. Brown

Purpose: Ischemic time (IT) is an independent risk factor for poor functional exercise capacity (FEC) following orthotopic heart transplantation (OHT). The OHT recipient’s post-transplant FEC (peak VO₂₎ is directly associated with improved quality of life. However, there is debate in the literature about the deleterious impact of extended IT on FEC following OHT.

Methods: Fifteen OHT recipients (14 men and 1 woman; < 3 months from OHT) performed a symptom-limited graded CPET where peak VO₂ was measured in an outpatient cardiac rehabilitation. IT was obtained from the anesthesia post-operative note. VO₂ and IT values were dichotomized based on previous literature; high, ≥14 mL/kg/min (or ≥12 mL/kg/min if taking a beta …

The College of Arts and Sciences (CoAS), one of Drexel’s largest colleges, has intentionally aimed to grow and deepen its commitment to civic engagement across the last decade. As Internet usage has increased, web-based technologies such as Skype and Face Time have become more common alternatives for qualitative interviewing, especially for research participants who are geographically distant from the researchers. Challenges to the use of these tools have been identified, but as technology is currently changing at a rapid pace, more recent research is needed to provide up-to-date information on the feasibility of web and video conferencing technologies for qualitative interviewing. Three years and three relapses later, I have my MA, I defiant will to fight this disease and the hope that by chronicling my experience I might help others. Nurses often witness suffering and loss. These experiences are intrinsic to the practice of nursing and can exert a strong, cumulative effect on nurses’ professional quality of life (QOL). Stamm (2010b) conceptualizes professional QOL as a composite of compassion fatigue and compassion satisfaction. Inferior professional QOL has been linked to numerous negative healthcare outcomes. Oncology nurses are particularly vulnerable to compassion fatigue. The Professional Quality of Life Scale (ProQOL), created by Stamm (2009), is the current survey used to measure professional QOL, but it may not be specific enough for oncology nurses.

The reduction of nurse burnout has shown to have an increase in patient satisfaction and quality of care. It also has shown a decrease in patient-related or sentinel events. Accordingly, with the reduction of nurse burnout, there has also been a reduction of compassion fatigue. This project has illuminated the benefits of acuity-based caseload at the micro-level, including the cost-effective nature of overtime pay, including HCHAPS benefit score. The method of obtaining, planning, and implementing was based on the Plan, Do, Study, Act (PDSA), which required collaboration amongst multiple disciplines, groups, departments, and executives. In this paper, the author begins with the question of how we can recognize whether or not we are in an opportune time to re-evaluate our current economic system. Through a literature review the canvas is set for the surveys of 63 people from across the United States about the way they feel about the economic system within which we currently find ourselves. Mr. Eric Stephanson begins the interview by describing his path into ministry and divinity and how that intersected over a lifetime, into a career in pediatric palliative and pediatric hospice care. The experiences of pediatric oncology nurses with prognosis-related communication (PRC) remain largely unknown. The purpose of this article is to report results of focus groups wherein 15 pediatric oncology nurses from three Midwestern pediatric cancer programs provided descriptions of PRC and how they experience PRC within their daily practice. Data from focus groups were analyzed via an interpretive descriptive approach, which resulted in three themes: (1) nurses’ operational definition of PRC, (2) nurses’ roles in PRC, and (3) nurses’ preparation for engagement in PRC. From discussions within the focus groups, nurses recognized that PRC occurs across a continuum. Nurses distinguished …

Problem: Hospital readmission rates have steadily climbed in the United States and the cost of unplanned readmissions can be detrimental. It has been identified that greater discharge preparation and quality care coordination greatly impacts the patient’s plan of care and reduces the risk of unplanned 30-day readmissions. Transition programs help reduce the psychosocial barriers that prevent patients from being able to self-manage their conditions outside the acute setting and help patients effectively navigate the through the continuum of care.

To the Editor: Recent therapeutic advances for mantle cell lymphoma (MCL) include inhibitors of Bruton’s tyrosine kinase (BTK), a critical component in the B-cell receptor signaling pathway [1, 2]. Remarkably, approximately two thirds of patients with relapsed/refractory (R/R) MCL treated with ibrutinib, the first-in-class BTK inhibitor, achieve a durable response [3–5]. However, ibrutinib treatment also commonly produces off-target adverse events (AEs) such as bleeding, atrial fibrillation, diarrhea, and infection. Second-generation BTK inhibitors with greater selectivity include tirabrutinib (GS/ONO-4059), acalabrutinib, and BGB-3111 [6]. In 2017, acalabrutinib received FDA approval for the treatment of MCL based on a complete response (CR) rate …

Ms. Stacy Remke begins the interview by describing her initial career path in social work around aging and how she came to develop her interest in care planning for pediatric end of life needs. Ms. Remke then describes her observation that both providers and parents would not have meaningful discussions about the realities and practicalities of children who were seriously ill or dying. She also noticed that providers were resistant to utilizing hospice services for seriously ill pediatric patients and that they expressed sentiments like "you'll take care of my patients over my dead body." Understanding Assistance In Dying: Arguments In Favor Of The End Of Life Option Act, Malgorzata Hausdorff

Abstract

There are many people in our society who are profoundly uncomfortable with the legalization of physician assistance in dying, previously known as physician - assisted suicide. The End of Life Option Act is designed to help people who are dying, who have no realistic chance for meaningful improvement of their health, and who are senselessly suffering from their condition.

There are misconceptions regarding assisting in dying is the terminology itself. Article discusses the law, as well as, clarifies the vocabulary related to the topic.

In conclusion the article raises the question: Is neglecting suffering consistent with the healers ethics?

Nurses have difficulty describing the steps of advocacy to guide practice. In order to improve the status of end-of-life care, nurse educators need to be cognizant of the advocate role in nursing practice. The purpose of this comparative descriptive study was to describe nurses' perceptions of advocacy behaviors in end-of-life nursing practice. The novice to expert process and the seven domains of a caring practice as they relate to advocacy behaviors (Benner, 1984) provided a framework in which the nurse can move towards becoming an effective patient advocate.
