Rehabilitation Practice and Science

Objective: To describe a patient with the critical illness polyneuropathy superimposed on diabetic neuropathy and to highlight the importance of early rehabilitation after critical illness. Methods: A 42-year-old man with a previous history of diabetic polyneuropathy developed flaccid tetraplegia with respiratory failure after septic shock due to a liver abscess. This clinical manifestation, combined with the result of an electrodiagnostic study, suggested the acute exacerbation of polyneuropathy. Because a pre-existing polyneuropathy history complicated a differential diagnosis, we performed a comprehensive work-up to rule out other systemic diseases or metabolic disorders that may cause polyneuropathy. Swallowing Therapy For Systemic Amyloidosis-Induced Dysphagia: A Casereport, I-Hsien Wu, Ching-Chi Chen, Mei-Yi Wu, Li-Chen Tung, Jung-Tai Liu

Rehabilitation Practice and Science

Amyloidosis is defined as the extracellular deposition of fibrillar proteins in different organs and tissues. The clinical features of amyloidosis include cardiomyopathy, hepatomegaly, proteinuria, macroglossia, and dysphagia. We report the case of a 52-year-old man with myeloma-associated amyloidosis who complained of progressive dysphagia, dysarthria, and macroglossia. Videofluoroscopic swallowing study revealed severe reduction in tongue movements, aspiration after swallowing, and retention in the valleculae and pyriform sinuses. The patient underwent swallowing therapy for 6 months; the therapy included compensatory neck flexion, vocal cord adduction exercises, supraglottic swallow, and the Mendelsohn maneuver. An experiment was conducted for 210 days to demonstrate the role of vegetable-fish culture integration in the growth, yields and economic benefits of fish and vegetables. Two 200 m2 earthen fishponds were stocked with Nile tilapia Oreochromis niloticus at 20,000 fish fingerlings per hectare. Pond A was fertilized with chicken manure and stocked fish fed on 35% crude protein supplementary diet referred to here as treated fish pond (TFP). A new species of Phoradendron, Ph. mathiasenii, is described, and a range extension and host are given for Ph. tikalense. Both species are from Honduras and grow as epiparasites, i.e., mistletoes parasitizing other mistletoes. The parasitic host for both epiparasitic species of Phoradendron is Psittacanthus angustifolius (Loranthaceae), which in turn is parasitic on the terrestrial host, Pinus oocarpa, a common forest tree in the collection area. High levels of prenatal cortisol have been previously reported to retard fetal growth. Although cortisol plays a pivotal role in prenatal maturation, heightened exposure to cortisol can result in lower body weights at birth, which have been shown to be associated with adult diseases like hypertension and cardiovascular disease. This study examines the relationship between natural variation in gestational cortisol and fetal and postnatal growth in marmoset monkeys. Urinary samples obtained during the mother’s gestation were analyzed for cortisol. Marmoset body mass index (BMI) was measured from birth through 540 days in 30- or 60-day intervals. Demensia Alzheimer menempati urutan kesembilan penyebab kematian di Amerika Serikat. Demensia adalah kondisi yang sering dialami yang berhubungan dengan berbagai faktor dan gaya hidup terutama diet. Penelitian ini bertujuan untuk mengetahui hubungan defisiensi asupan riboflavin (vitamin B12) dengan demensia pada usia lanjut (usila). Penelitian ini menggunakan desain studi cross sectional dan dilaksanakan pada bulan September 2007 sampai dengan Januari 2008. Sampel penelitian adalah 141 lansia berumur lebih dari sama dengan 60 tahun yang diambil secara purposive sampling. Background

High dietary fat intake is a major risk factor for development of cardiovascular and metabolic dysfunction including obesity, cardiomyopathy and hypertension.

Methods

The present study was designed to examine effect of high fat (HF) diet on cardio-vascular structure and function in spontaneously hypertensive rats (SHR), fed HF diet for 15 weeks, a phenotype designed to mimic metabolic syndrome.

Results

Development of metabolic syndrome like phenotype was confirmed using parameters, including body weight, total cholesterol and blood pressure levels. Background: Along with obesity, physical inactivity, and family history of metabolic disorders, African American ethnicity is a risk factor for type 2 diabetes (T2D) in the United States. However, little is known about the differences in gene expression and transcriptomic profiles of blood in T2D between African Americans (AA) and Caucasians (CAU), and microarray analysis of peripheral white blood cells (WBCs) from these two ethnic groups will facilitate our understanding of the underlying molecular mechanism in T2D and identify genetic biomarkers responsible for the disparities.

Mutations in the insulin gene can impair proinsulin folding and cause diabetes mellitus. Although crystal structures of insulin dimers and hexamers are well established, proinsulin is refractory to crystallization. Although an NMR structure of an engineered proinsulin monomer has been reported, structures of the wild-type monomer and hexamer remain undetermined. We have utilized hydroxyl radical footprinting and molecular modeling to characterize these structures. Differences between the footprints of insulin and proinsulin, defining a "shadow" of the connecting (C) domain, were employed to refine the model. 3-hydroxy-3-methylglutaryl coenzyme A reductase (HMGCR) inhibitors, commonly known as statins, may possess cancer preventive and therapeutic properties. Statins are effective suppressors of cholesterol synthesis with a well-established risk-benefit ratio in cardiovascular disease prevention. Mechanistically, targeting HMGCR activity primarily influences cholesterol biosynthesis and prenylation of signaling proteins. Pravastatin is a hydrophilic statin that is selectively taken up by a sodium-independent organic anion transporter protein-1B1 (OATP1B1) exclusively expressed in liver. Simvastatin is a hydrophobic statin that enters cells by other mechanisms. Mercury is found throughout the environment with a distribution on a global scale in nearly all terrestrial to marine environments. Biotransformation of divalent mercury and subsequent bioaccumulation of organo-mercury species is catalyzed by sulfate reducing bacteria. Trophic transport of the resultant methyl mercury, a highly toxic species, leads to upward accumulation in the trophic structure of aquatic environments and ultimately to human exposure through consumption of methyl mercury laden fish. Drinking shallow groundwater with naturally elevated concentrations of arsenic is causing widespread disease in many parts of South and Southeast Asia. In the Bengal Basin, growing reliance on deep (>150 m) groundwater has lowered exposure. In the most affected districts of Bangladesh, shallow groundwater concentrations average 100 to 370 μg L−1, while deep groundwater is typically < 10 μg L−1. Groundwater flow simulations have suggested that, even when deep pumping is restricted to domestic use, deep groundwater in some areas of the Bengal Basin is at risk of contamination. However, these simulations have neglected the impedance of As migration by adsorption to aquifer sediments. Here we quantify for the first time As sorption on deeper sediments in situ by replicating the intrusion of shallow groundwater through injection of 1,000 L of deep groundwater modified with 200 μg L−1 of As into a deeper aquifer. Arsenic concentrations in the injected water were reduced by 70% due to adsorption within a single day. Basin-scale modelling indicates that while As adsorption extends the sustainable use of deep groundwater, some areas remain vulnerable; these areas can be prioritized for management and monitoring.

The feasibility and outcomes of a 12-week extracurricular family-based intervention led by physical therapists that was designed to increase physical activity (PA) in three Hispanic male middle-school children was examined. This intervention has limited feasibility and may increase physical activity levels for overweight Hispanic middle school children. In a second study, differences in motor proficiency, strength, endurance, and PA among healthy weight, overweight and obese children were examined, and correlations between BMI and physical impairments were explored. From a bibliometric perspective, scholarly inbreeding has been identified in journals through the excessive use of both author and journal self-citations. However, editorial bias toward researchers from the same institution as the editorial management team has seldom been considered. According to the Spanish Foundation for Science and Technology, this occurs when a journal publishes more than 20% of documents authored by researchers affiliated with the same institution as the editorial management team.
