慢性阻塞性肺疾病晚期病人之安寧緩和醫療需求, 亭儒 陳, 文郁 胡, 之勛 鄭, 慶餘 陳

本文藉由文獻查證與臨床經驗,探討慢性阻塞性肺疾病(chronic obstructive pulmonary disease, COPD)晚期病人之安寧緩和照顧需求。結果顯示COPD晚期病人在生理層面,忍受複雜而困難症狀且未被適當地處理;心理靈性層面,病人出現負向情緒反應的比例相當高,靈性需求常被忽略;社會層面,出現社會隔離及家屬代為做決策情形;溝通方面,呈現病人對於生命末期臨床資訊與末期照護議題討論皆不足。文中也提出安寧療護於COPD晚期病人之應用,包括介入時機、從全人觀點進行症狀處置、全面評估心理靈性需求、運用社區資源與溝通生命末期照顧議題等。未來可考慮於醫療專業人員之養成教育或繼續教育課程增加此部份的內容,以提升對COPD晚期病人末期照顧的知識與照顧品質。

This study assesses the cumulative effect of a training curriculum focused on teamwork, which was delivered to technical employees in a multinational organization. Employees were given a 10-item survey prior to the training and four months after the survey. Several aspects of the literature were incorporated into the design of the curriculum and analysis of the results: content was tailored toward the employees’ environment, management was asked to promote the training to contribute to a positive climate, and assessment was deferred by four months to maximize the chance that employees might implement the lessons from the training into practice. This qualitative phenomenological study attempts to gain the physician's essences, meanings and understanding surrounding death and dying. Starting with themes revealed in literature, in-depth interviews were used to ask selected physicians providing care: How does death affect them? Are physicians receiving adequate training in end-of-life care? Does the medical culture still see death as failure? How do they find meaning in end-of-life care? Using three different groups of physicians: early in career, mid-career, and retired, this research attempted to examine the phenomena of death and dying over apparent time and developmental experiences of physicians. Nurses perform a vital role in the care of dying patients and their families. Hence, experiences of nurses are a meaningful source from which to advance holistic end-of-life care. In this study, a hermeneutic phenomenological perspective was used to explore the phenomenon of end-of-life nursing care. Details derived from a scientific exploration into the experiences of 13 registered nurses who provided care for patients and families at end-of-life in the inpatient hospital setting offer understandings regarding this important phenomenon. The increasing number of patients with end stage renal disease (ESRD) has caused a substantial rise in the number of individuals receiving hemodialysis. One of the persistent questions with this group has been the influence of the procedure on quality of life. The present study explored the important factors that influence quality of life in hemodialysis patients. Participants were 160 (71 men and 89 women, mean age 62.84 years) community-dwelling, hemodialysis patients. Aims: We sought to identify differences in pain management between two groups; nursing home residents with malignant cancer and dementia with and without hospice services.
Methods: Decedent records from 2003-2009 were assessed for diagnosis of dementia and cause of death as cancer. Ten malignant cancer diagnoses were determined a priori from the CDC 2004 data on the top 10 malignant cancers for all races and genders. Fifty-five decedents from 10 nursing homes were included in the final sample. Four instruments were used: Minimum Data Set (MDS) a standardized assessment tool required of most U.S. nursing homes. There is limited research on female spouses and their end-of-life experience with their dying husbands. The purpose of this study was to explore wives' perceptions of hospital and hospice care at their husbands' end-of-life. The grounded theory method was used for this study. Data were collected over thirteen months and then transcribed and coded for meaning. Participants were 25 elderly widows (62-103 years of age) with 19 husbands' deaths in a hospice setting and 6 in the hospital. They were widowed from 6 months to 10 years after the death of their spouses. They were good informants with excellent recollections …

Many patients clinging to hope in the form of potentially curative treatment could benefit from hospice services, but, for the most part, it is not until the patient accepts the finality of his or her condition that the physical, psychological and social benefits of hospice care become accessible to the patient and his or her family. Under current Medicare regulations and other health care payers’ policies, patients must abandon the hope of curative treatment before opting for hospice services. As a result, many terminally ill patients access the services late, sometimes a few hours before death. Scholars have proposed that …

The aim of this study was to develop and translate a psychometrically valid and reliable, population-based, needs assessment instrument, the PCNA-EAV, to measure the health care and support needs of patients with advanced cancer. The cross-sectional study design combined qualitative and quantitative methods, to test instrument reliability and validity, and to examine the association between sample characteristics and health care and support needs. The 116-item, PCNA-EAV, comprised 10 domains of need: physical/functional; social; psychological/emotional; information; communication; helpful resources; financial; religious/spiritual; priority of need; and preference for care The target population was all cancer patients in the department of oncology at …
