Using Literary Ethnography As A Form Of Qualitative Document Synthesis To Explore The Maltreatment Of Vulnerable Populations: An Examination Of Verbal Neglect And Abuse In Nursing Homes, Jason S. Ulsperger

Studying vulnerable populations can be highly problematic. This is especially true when they are located in institutional settings. When gatekeepers block access and a researcher wants to examine a delicate topic, one ethical, feasible way to paint an interpretive picture of everyday life involves the use of a literary ethnography. With data on the verbal neglect and abuse of elders in United States nursing homes, this paper details the six-stages of a literary ethnography. Older People With Complex Health Needs Desire For Change: A Qualitative Study, Diane Jorgensen, David R. Thoman, Matthew Parsons

The Qualitative Report

As a sub study of another study we examined older people with complex health and disability support needs' desire for change. The aim was to compare this across different ages, residence, and gender. Semi-structured interviews were held with 129 participants and the data were analyzed using a General Inductive Approach. Six themes emerged, Health, No Change, Personal Changes, Family, Housing, and Services. The two most popular themes were a desire for health changes and to have no change. The Role Of Ombudsmen In Assuring Quality For Residents Of Long-Term Care Facilities: Straining To Make Ends Meet, Carol O'Shaughnessy

Assuring quality of care for residents in long-term care facilities has been a serious and continuing concern of policymakers for decades. The Older Americans Act’s long-term care ombudsman program is a consumer advocacy model intended to improve quality of care by helping the 2.5 million residents of almost 67,000 nursing and other residential care facilities resolve complaints about their care and protect their rights. Despite broad recognition of its value in assisting residents and its efforts to complement federal and state oversight of long-term care facilities, some observers are concerned about the program’s ability to meet its legislative mandates. Caregivers of the elderly face conflicting legal demands; they must make certain the elder’s needs are being met while not forcing undesired care on an adult capable of informed decisions. This dichotomy may be a reason a large volume of reported elder abuse derives from unintentional neglect on behalf of informal familial caregivers. The current research examines this possibility with exploratory interviews and an experiment. The interviews between elders and their family (30 dyads) revealed that many did not intend for the living arrangements to become permanent, and the nonelders were largely unprepared for the magnitude of changes and responsibilities …

Effects Of Caregiver Burden And Satisfaction On Affect Of Older End-Stage Renal Disease Patients And Their Spouses, Maureen Wilson-Genderson, Rachel A Pruchno, Francine P Cartwright

Rowan-Virtua School of Osteopathic Medicine Departmental Research

We examined the extent to which a 2-factor model of affect explains how the burdens and satisfactions experienced by caregivers influence their own well-being and that of the spouses for whom they provide care. Using data from 315 older patients with end-stage renal disease and their spouses, we extended tests of Lawton et al.'s (1991) 2-factor model both longitudinally and dyadically. Multilevel modeling analyses partially support the 2-factor model. Consistent with the model, mean caregiver burden has a stronger effect on both caregiver and patient negative affect than does mean caregiver satisfaction. The scheme was a pilot project, from 2004 to 2005, which led to the development of a Universitywide protocol for Service-Learning at Lingnan University.

The current study investigated the nature of anticipatory grief in a sample of dementia caregivers, and examined the relationship between anticipatory grief and caregiver burden. Anticipatory grief can be described as one's reaction to the impending death of a loved one, in addition to responses to other previously endured, ongoing, and anticipated losses. Interest in anticipatory grief has been inconsistent, and has typically focused on terminal diseases such as cancer. The purpose of this qualitative research study was to develop a model focused on spiritual influences within older adults undergoing relocation to long term care (LTC) facilities. Today, there are 1.5 million Americans aged 65 or older who experienced relocation and reside in LTC facilities. Nothing has been done to qualitatively study the spiritual aspects of this experience. While the profession of nursing has a diagnosis for Relocation Stress Syndrome, as found in the North American Nursing Diagnosis Association (NANDA) reference, it does not incorporate the specific spiritual stressors older adults undergo during relocation; rather addressing only physiological and psychosocial …
