Fears that terrorists may have the capabilities and intent to disseminate a variety of biologic agents has once again brought smallpox into the American consciousness. On December 13, 2002, recognizing that the global discontinuation of routine smallpox vaccination over two decades ago had left most Americans unprotected and vulnerable to the ravaging effects of the virus, the President announced a precautionary measure to begin vaccinating teams of emergency responders. The program commenced January 24, 2003. In the ensuing months, public health departments scrambled to meet the goal of vaccinating approximately 500,000 first responders, a protected phalanx that could quickly and …

Global climate change has been at the forefront of environmental concerns for the last few decades. Based on the success of the United States' sulfur dioxide cap-and-trade program, a market for greenhouse gas emissions has slowly emerged. Russia's recent ratification of the Kyoto Protocol will bring an international market into force in February 2005. This thesis examines the economic theory behind cap-and-trade markets as solutions for environmental problems and discusses the characteristics of the markets for greenhouse gas emissions. The opportunities for Alaska's participation are examined, particularly offsetting diesel emissions from rural generators with wood or alternative energies. The calculations …

Quality Improvement In Maryland: Partnerships And Progress, Judith D. Moore, Lisa Sprague
National Health Policy Forum
This one-and-a-half-day site visit to Easton, Maryland, explored the activities of the Delmarva Foundation, the Quality Improvement Organization (QIO) for Maryland and the District of Columbia. The program reviewed Delmarva’s responsibilities under their Medicare contract and the initiatives they have undertaken with hospitals and other providers to improve health care outcomes and to define, collect, and report quality data. Local hospital officials added their perspective on quality programs and working with Delmarva. Topics of particular interest were health information technology, patient safety, and quality reporting both to government agencies and to consumers.

This policy brief examines the structural-level opportunities and challenges associated with the delivery of HIV prevention services in or closely linked to the clinical care setting. It focuses on two of the major public programs for HIV care in the U.S: Medicaid, the nation's major public health program for low-income Americans, and the largest source of public financing for HIV/AIDS care in the U.S.; and the Ryan White CARE Act, the nation's only HIV-specific care and support services grant program which operates as the payer of last resort at the state and local level. Maximal oxygen uptake (VO2 max) is the best criterion measure for aerobic fitness and the prescription of exercise intensity for programs designed to enhance cardiorespiratory fitness. There are two ways of obtaining VO2 max: maximal tests, which require subjects to exercise to the point of volitional exhaustion and provide the most accurate measure; and submaximal tests, which are less physically strenuous but have lower accuracy. A popular submaximal protocol is the YMCA bike test. Steady state heart rate (HR) is measured at multiple submaximal workloads and extrapolated to the subject's estimated maximal HR (220-age). BACKGROUND: A case of West Nile virus (WNV) encephalitis associated with transfusion of blood that did not react when tested for WNV by minipool (MP) nucleic acid testing (NAT) is described. A Nebraska man developed clinical encephalitis 13 days after surgery and transfusion of 26 blood components. Antibody testing confirmed WNV infection. An investigation was initiated to determine the source of this infection.
STUDY DESIGN AND METHODS: The patient’s family members were interviewed to identify risk factors for WNV infection. Residual samples were retested for WNV RNA using transcription-mediated amplification (TMA) assay and two polymerase chain reaction (PCR) assays. Blood …

The initial symptoms of a Tanapoxvirus (TPV) infection are indistinguishable from smallpox, which is caused by variola virus (VV). In the post-9/11 area, reagents that can quickly differentiate a TPV infection from smallpox are very desirable, but currently unavailable. Monoclonal antibodies (mAbs) directed specifically against TPV can be used for this purpose. Six mAbs were developed and characterized that specifically bind to TPV infected owl monkey kidney (OMK) cells, but not uninfected cells. The apparent molecular mass of the TPV protein(s) recognized by 5 mAbs were calculated using the Western blot technique. One mAb demonstrated no binding using the Western …

OBJECTIVE: Bacille Calmette-Guerin (BCG) is given at 2-months of age; diphtheria toxoid, tetanus toxoid, whole cell pertussis, oral polio are given at 2, 3, 4 months, and hepatitis B virus (HBV) vaccine is given at 3, 4, 9 months of age. A survey of abomasal parasites in cervids from Central Spain was conducted at three sites, Quintos de Mora (Toledo), Maluéñez de Arriba (Cáceres), and La Herguijuela (Cáceres). Commonly occurring helminths belonged to three polymorphic species of the Ostertagiinae: Spiculopteragia asymmetrica/S. quadrispiculata, Ostertagia leptospicularis/O. kolchida, and O. drozdzi/O. ryjikovi. Trichostrongylus axei was found in very few cases. Ostertagia drozdzi/O. ryjikovi and the minor male morphotype, S. quadrispiculata, are reported for the first time in red deer from Spain. Asthma is characterized by infiltration and shedding of the bronchial epithelium. The Th2 cytokines IL-4 and IL-13 are involved in the cellular recruitment and infiltration seen in asthma. The effects ofIL-4 and IL-13 on cell-matrix interactions, gap formation, increased monolayer permeability and epithelial shedding are unknown. Experiments described in this report test the hypothesis that bronchial airway epithelial cells (BAEC) express paxillin, a structural focal adhesion protein, and down-regulation of paxillin by Th2 cytokines lead to BAEC hyperpermeability. There is considerable debate about the fundamental mechanisms that underlie and restrict acquisition of human immunodeficiency virus type 1 (HIV-1) infection. In light of recent studies demonstrating the ability of C type lectins to facilitate infection with HIV-1, we explored the potential relationship between polymorphisms in the DC-SIGN promoter and risk for acquisition of HIV-1 according to route of infection. Using samples obtained from 1,611 European-American participants at risk for parenteral (n = 713) or mucosal (n = 898) infection, we identified single-nucleotide polymorphisms in the DC-SIGN promoter using single-strand conformation polymorphism. The serum autoantibody repertoire from cancer patients is currently being exploited for the identification of tumor associated antigens (TAA) and the design of TAA arrays. Such arrays would facilitate autoantibody profiling and potentially aid in the serological diagnosis and prognosis of tumors. The goal of this study was to identify and characterize prostate-specific TAA. We observed that while the general frequency and titers of autoantibodies in PCa patients were relatively similar to those in matched controls, significant differences could be detected between the two groups in the autoantibody response to the lens epithelial-derived growth factor p75 (LEDGF/p75).
