Many professional organizations publish clinical practice guidelines for the use of probiotics in gastrointestinal disorders. Generally, no two guidelines align perfectly, and some differ markedly. These discrepancies occur because clinical practice guidelines are not purely mechanical outputs of data synthesis. They are shaped by structured interpretation, value judgments, and predefined thresholds for uncertainty. These elements become particularly visible when effect sizes are small, heterogeneity is high, and outcomes vary in clinical relevance-as is the case with probiotic research. Background: Dietary management is key to preventing and controlling type 2 diabetes mellitus (T2DM). However, few studies have examined differences in Healthy Eating Index (HEI-2020) scores and components among individuals with and without diagnosed T2DM. This study aimed to assess gender differences in HEI-2020 scores and its 13 components by T2DM status. Methods: Using National Health and Nutrition Examination Survey (NHANES) data from 2011 to 2018, this cross-sectional study analyzed 19,464 adults. Purpose of Review: This narrative review provides discussion on current evidence regarding the relationship between sodium intake and cognitive function in animal and human studies, as well as potential mechanisms underlying this relationship. Recent Findings: Recent evidence suggests high sodium intake is associated with development of cognitive impairment and neural dysfunction. Additionally, studies have proposed that high sodium intake is associated with increased aggregation of Aβ-amyloid and that hypertension (for which high sodium intake is a risk factor) modulates the relationship between Aβ-amyloid and development of cognitive impairment and Alzheimer’s disease. Summary: This cross-sectional study of 300 older adults (aged ≥ 50 years) found that less education, lower income, and health care card ownership are associated with reduced bone, muscle, and physical function measures. This underscores the need for targeted preventive strategies for osteoporosis and sarcopenia that address socioeconomic-related disparities. Purpose: The prevalence of chronic diseases follows a social gradient, although this is unclear in musculoskeletal conditions. The external school food environment, comprising food outlets and marketing near schools, shapes what young people eat. Yet little is known about how Australian adolescents perceive and navigate these environments. Focus groups were conducted with 31 school students (Years 7–12) from Perth, Western Australia, to explore their experiences of the external school food environment and its influence on their food-related behaviours. Transcripts were thematically analysed and six themes were identified. Accessibility emphasised proximity, transport links, opening hours and speed of service, with older students using outlets as social spaces. Retail food outlets are commonly located in close proximity to schools, providing students with opportunities to purchase and consume food enroute to or from school. These outlets are typically unhealthy and disproportionately clustered near schools, and this trend has been increasing over time. Background: Prebiotic dietary supplementation has been shown to improve glucose homeostasis in type 2 diabetes patients. The aim of this analysis was to determine whether pre-pregnancy body mass index (BMI) modifies the effect of prebiotic supplementation from mid-pregnancy on reducing the risk of gestational diabetes mellitus (GDM). Methods: In a double-blinded, randomised controlled trial, pregnant women < 21 gestational weeks were randomly assigned (1:1) to consume daily prebiotics (14.2 g galacto-oligosaccharides and fructo-oligosaccharides) or placebo (8.7 g maltodextrin) powder. An effect modification analysis was performed to assess the heterogeneity of the effect of prebiotic supplementation in relation to pre-pregnancy BMI on GDM diagnosis. Results: Between June 2016 and November 2021, 329 women were assigned to the prebiotic group (50.4%), and 323 (49.5%) were assigned to the placebo group. Overall, 288 of 652 women (44.2%) were classified as overweight/obese prior to pregnancy (BMI ≥ 25 kg/m2). The distribution was balanced, with 146/329 (44.4%) randomised to the prebiotics group and 142/323 (44.0%) in the placebo group. Pre-pregnancy BMI modified the effect of prebiotic supplementation, with the intervention reducing GDM rates in women with a BMI ≥ 25 kg/m2 …

Background: Data on the external validation of current dementia risk prediction models has not yet been systematically synthesised. This systematic review and meta-analysis collated results from three previous reviews to evaluate the predictive discriminative performance of dementia risk models when validated in population-based settings. Background: Transitioning from independent living to a residential care facility often results in a significant loss of independence, autonomy, and connectedness, leaving new residents with profound feelings of loss and grief. In light of these challenges, this study explored the potential benefits of a tailored in-house exercise program for new residents. To examine, among people with type 2 diabetes (T2D), whether cardiovascular disease (CVD) phenotypes and cardiovascular medications are associated with any fracture as the primary outcome and major osteoporotic fracture (MOF) as a secondary outcome. Nationwide registry-based matched case–control study in Denmark (2013–2021) among people with T2D. Cases were individuals with T2D who sustained an incident fracture (n = 12,390), and controls were fracture-free individuals with T2D matched 1:3 by age, sex, T2D onset date, and weighted Charlson Comorbidity Index (n = 37,170). Objective: Police force recruits have a high musculoskeletal injury burden, which results in a substantial economic burden and can lead to attrition. The objective of this study was to identify and prioritise the strategies perceived as important and feasible to reduce the prevalence, incidence, and burden of injury in police force recruits. Background: Legumes can substitute for meat and are thus a particularly important food group for environmental sustainability. As the use of legumes among Finnish children is marginal, there is potential for a substantial increase in their consumption. Background: The proliferation of fast-food outlets (FFOs) in residential areas has raised growing concerns about their impact on community health, safety, and wellbeing. This qualitative study examined both pre-development community consultation submissions and post-development resident experiences surrounding the development of a McDonald’s outlet in a newly established suburb on the urban fringe of Perth, Western Australia (WA). Methods: Using a qualitative descriptive design, the study analyzed 183 community submissions lodged with the local council prior to development and conducted semi-structured interviews with 11 residents living within 300 m of the outlet after construction. The development of plant-based meat analogues (PBMAs) that accurately replicate meat texture and flavor remains limited by fundamental structural differences between plant and animal proteins. Most notably, the globular conformation of plant proteins contrasts with the fibrous anisotropy of animal muscle, thereby limiting viscoelasticity. Additional challenges include beany off-notes arising from lipid oxidation products such as hexanal and anti-nutritional factors (ANFs) that reduce bioavailability. Problem/Research Question: Taste alteration affects up to 70% of oncology patients receiving chemotherapy, leading to decreased dietary intake, malnutrition, and poor quality of life. It is often under reported due to lack of taste assessments during toxicity screening and has no known treatment. Researchers have shown that breast cancer patients were able to assess and control alterations with a researcher-delivered intervention. Research is needed to translate this promising strategy for clinical use with populations beyond breast cancer.

We specify a mindful-eating and gratitude-meditation application in which a food-image recognition model resolves a meal photograph into its source species; each species is shown individually, at species level, with its typical farmed age at harvest and its typical natural lifespan, with uncertainty stated and no total computed. The application then recommends a walking or sitting meditation in a fixed form, issued by an automated agent and never scaled to what was eaten. We specify a hybrid community food network rooted in religious institutions: a distributed network of monastery-based sites, each combining a farm, a kitchen and free food distribution, operated by AI and robotic caretakers that hold no ordained status or ritual authority. Supply has two layers: produce donated from participants' home gardens, which the shared kitchen turns into meals for many, and food purchased through ordinary channels that guarantees a supply floor, so that people with no land, time or skill are never excluded; over decades the intended ratio shifts toward donated produce without cutting anyone off. Objective

Medicaid recipients in rural populations experience higher rates of food insecurity. Produce prescription (PRx) programs subsidize the cost of fruits and vegetables (F/V) and may improve health outcomes. This study was designed to test whether a PRx program using a community supported agriculture (CSA) model would improve health outcomes and reduce healthcare utilization among rural Medicaid patients with/at risk of chronic illnesses.

Methods

BACKGROUND: Hypophosphatasia (HPP) is a metabolic disorder characterized by deficient alkaline phosphatase (ALP) activity, defective mineralization of bones and teeth, and non-skeletal manifestations. We compared disease burden between patients with skeletal and non-skeletal manifestations.

METHODS: Data from US adults enrolled in the Global HPP Registry through July 2024 were analyzed to compare baseline characteristics, genetic testing results, functional outcomes, and patient-reported outcomes between participants with and without overt skeletal manifestations. Patients who had only dental manifestations were excluded.

BACKGROUND: Neonates with kidney failure require placement of a peritoneal dialysis catheter (PD-C) shortly after birth. PD-C malfunction and peritonitis, both common in this population, are associated with PD failure. The aim of this study was to describe our center's PD-C-related practices and outcomes in neonates with kidney failure.

METHODS: We performed a retrospective chart review of neonates who required kidney replacement therapy (KRT) within the first 30 days of life from 01/2018-12/2023. We evaluated for PD-C duration, PD-C replacement or revision, and peritonitis.

This article describes innovative elementary science instructional approaches based on cultural wisdom developed by preservice teachers enrolled in a teacher preparation program at a university in the U.S.-Mexico border region. As a part of a larger study, three Latina preservice elementary teachers were a part of a teacher residency in conducting community interviews and reflection activities to uncover cultural backgrounds that could inform culturally relevant STEM lesson planning, centering on family and home knowledge. Nanoplastic (NP) fouling remains a key challenge for ultrafiltration (UF) membranes, yet systematic, controlled comparisons of how membrane functional groups govern NP-membrane interactions are still limited. Here, the role of membrane surface chemistry was systematically investigated using a controlled comparative framework based on plasma-grafted polyethersulfone (PES) UF membranes tailored with structurally comparable methacrylate monomers bearing distinct terminal functionalities, including mono-2-(methacryloyloxy)ethyl succinate (MMES, -COOH), N-[3-(dimethylamino)propyl] methacrylamide (DMAPMA, -N(CH3)2), and [2-(methacryloyloxy)ethyl]dimethyl-(3-sulfopropyl)ammonium hydroxide (SBMA, zwitterionic). Considerable advances have been made in our understanding of Bardet-Biedl syndrome (BBS), particularly in its core clinical features and molecular genetics, warranting an update to the existing diagnostic criteria framework. Using a rigorous, evidence-based, and consensus-driven process, a multidisciplinary group of international experts and patient-led organizations developed an updated diagnostic algorithm. This algorithm provides practical, updated guidance for clinicians, including a pathway for accurately incorporating genetic findings into the diagnostic process. We recommend that a clinical diagnosis requires either 4 major criteria or 3 major and 2 minor criteria. Revised major criteria are retinal dystrophy, obesity (or overweight in individuals <  2 years old), congenital anomalies of the kidney and urinary tract or chronic kidney disease, hypogonadism/genital anomalies, neurodevelopmental/neurocognitive manifestations, and postaxial polydactyly. The diagnosis can also be established with a positive genetic testing result in patients exhibiting ≥ 1 major criterion, provided that genetic findings should be interpreted in the context of the patient's clinical presentation, age, family history, and overlap with related ciliopathies. These consensus criteria offer a simple algorithm incorporating updated definitions for major and minor criteria and genetic testing to support a timely and accurate diagnosis of patients with BBS, inform genetic counseling, and potentially facilitate earlier access to treatment.
