Assessing Social Determinants Of Health Training In Graduate Medical Education: A Narrative Review Using Kirkpatrick’S Model, Surya Namboodiri, Bharathi Gadad, Juan Lopez-Alvarenga, Chelsea Chang

Social determinants of health (SDOH) training is a critical component of medical education, equipping physicians to address factors such as economic stability, education, and health care access. Despite its importance, the evaluation of SDOH training often focuses on lower-level outcomes, such as learner satisfaction and knowledge assessment, with limited emphasis on behavioral change and patient outcomes. Kirkpatrick’s model offers a validated framework for addressing this gap by categorizing training outcomes into four levels: Reaction, Learning, Behavior, and Results.

Most prior research on resilience to HIV/AIDS has utilized quantitative tools (e.g., scales and surveys) to examine individual-level assets (e.g., self-efficacy, hope, optimism) that researchers believe represent or approximate resilience to HIV/AIDS with minimal consideration for the perspectives of men who have sex with men (MSM), the population that has remained at greatest risk of, and the most impacted by HIV/AIDS in North America since the 1980s. Roughly forty years since the beginning of the HIV/AIDS epidemic, gay, bisexual, and other men who have sex with men (MSM) continue to be the population most at risk of, and most affected by, HIV/AIDS in Canada, the United States, and innumerable low to high income countries worldwide. Despite this, many MSM remain HIV-negative today, and among MSM living with HIV, many have not only survived the clinical and social impacts of HIV/AIDS, but have also thrived, lived full lives, and fiercely advocated for their health care and social service needs.

Partnered research may help bridge the gap between research and practice. Community-based participatory research (CBPR) supports collaboration between scientific researchers and community members that is designed to improve capacity, enhance trust, and address health disparities. Systems science aims to understand the complex ways human-ecological coupled systems interact and apply knowledge to management practices. The Community Research Fellows Training (CRFT) Hattiesburg program took place between January 12th, 2016 and May 17, 2016. This is the third Mississippi cohort of CRFT and the first Mississippi cohort outside of Jackson, Mississippi. This report reflects the implementation and evaluation of a community based participatory training (CBPR) program for community members in Hattiesburg. The report provides data on the assessment of the program’s effectiveness in promoting the role of underserved populations in research by enhancing the capacity for CBPR. BACKGROUND: The purpose of this study was to learn community members' perspectives about digital storytelling after viewing a digital story created by a Community Health Aide/Practitioner (CHA/P).

METHODS: Using a qualitative research design, we explored digital storytelling likeability as a health-messaging tool, health information viewers reported learning and, if viewing, cancer-related digital stories facilitated increased comfort in talking about cancer. In addition, we enquired if the digital stories affected how viewers felt about cancer, as well as if viewing the digital stories resulted in health behaviour change or intent to change health behaviour.

CONTEXT: Many encourage service learning and health advocacy training in medical student education, but related evaluation is limited.

OBJECTIVES: To assess (1) impact of a required community health advocacy training for medical students on student attitudes, knowledge, and skills; (2) student characteristics associated with higher advocacy knowledge and skills; and (3) perspectives of community-based organizations (CBOs).

DESIGN: Cross-sectional surveys.

SETTING: University of Miami Miller School of Medicine (UMMSM) Regional Medical Campus and main campus.

Objective(s): The purpose of this study was to explore the effectiveness of CHW/ nursing student teams in promoting secondary protection and improving access to care for residents of three urban underserved neighborhoods. The study also sought to measure CHW and resident satisfaction of such a program. Design and Sample: Quasi-experimental and nonexperimental designs were used for the study. Convenience samples consisted of residents who participated in the CHW program during 2005–2006, CHWs and residents who participated in the CHW program during 2005–2007 and a systematic random sample of residents across the three neighborhoods. Objective: The authors report the outcomes of a community-based, barber health adviser pilot intervention that aims to develop customized educational materials to promote knowledge and awareness of prostate cancer (CaP) and informed decision making about prostate cancer screening (PCS) among a predominantly African American clientele.

Method: First, the authors implemented a series of learner verification processes with barbershop clients (n=15) to adapt existing CaP health promotion materials. Following intervention implementation in the barbershop, they conducted structured surveys with barbershop clients (n=40) to evaluate the intervention. Barbers Against Prostate Cancer: A Feasibility Study For Prostate Cancer Education In An Urban African American Community, John S. Luque, Brian M. Rivers, Maisha Kambon, Ronald Brookins, B. Lee Green, Cathy D. Meade

The goal of this pilot study was to assess the feasibility of training barbers to deliver a brief culturally and literacy appropriate prostate cancer educational intervention to urban African American men. Eight barbers received training to deliver a 2-month educational intervention in the barbershop and completed pre- and posttest training assessments. The training workshops led to a significant increase in mean prostate cancer knowledge scores among the barbers (60% before vs. 79% after; P < 0.05). The barbers also reported positively on the intervention in terms of satisfaction and relative ease of engaging clients. Training barbers to deliver a prostate cancer educational intervention is a feasible strategy for raising prostate cancer awareness of the disease among a priority population.

Schedule H: New Community Benefit Reporting Requirements For Hospitals, Eileen Salinsky

In recent years, some policymakers have questioned whether not-for-profit hospitals benefit the communities they serve at a level commensurate with the tax exemptions they receive as charitable organizations. This background paper reviews the new community benefit reporting requirements hospitals will face in 2009 under Schedule H of the Internal Revenue Service's revised Form 990 (the return used by organizations exempt from federal income tax). The paper provides a descriptive summary of the quantitative and qualitative information to be reported on Schedule H, such as charity care, bad debt, and the unreimbursed costs of Medicaid and Medicare.
