Too many psychiatric patients leave the hospital without a follow-up appointment, a gap that may cost lives. Research has consistently shown that the days immediately after discharge from psychiatric inpatient or emergency care are among the most dangerous in a patient’s trajectory, marked by elevated suicide risk, relapse, and avoidable readmission. Yet, across behavioral health settings, completion of outpatient follow-up within 7 days of discharge remains inconsistent; typically, standardized processes are often missing. In this quality improvement project, I evaluated a pilot implementation of a standardized bioidentical hormone replacement therapy (BHRT) screening and monitoring tool embedded within the electronic health record (EHR) at an integrative medicine practice in the southeastern United States. Grounded in the Johns Hopkins evidence based practice (EBP) model, the project addressed a documented gap in consistent screening, initiation, and monitoring of hormone replacement therapy (HRT) for women ages 35 to 50 presenting with menopausal symptoms. This quantitative, non-experimental study was an examination of the perceptions and experiences of board certified behavior analysts (BCBAs) and board certified behavior analysts doctorate (BCBA-Ds) regarding the comparative utility of traditional functional behavior assessment (FBA) and practical functional assessment (PFA). This study examines leadership in a community policing context. It addresses a long-standing problem: injustice, inequality, and unfair treatment. Specifically, it examines community policing and the impact of law enforcement leadership, particularly servant leadership, on policing outcomes. Greenleaf’s Theory of Servant Leadership guided this study. Participants included mid- to senior-level law enforcement officers with community policing experience, at least three years of leadership experience, and at least 18 years of age. The purpose of the project was to implement and evaluate a structured fall risk assessment process that promoted the early identification of patients at increased risk for falls and supported interventions designed to prevent fall-related events. A pre- and post-implementation evaluation design was used to assess the intervention's effectiveness by comparing fall rates before and after implementation of the standardized assessment protocol. De-identified data obtained from the electronic health record (EHR) were analyzed using descriptive statistics to evaluate changes in fall incidence and protocol adherence. This organization change process evaluation addressed continued reliance on outdated no-suicide and no-harm contracts in an outpatient psychiatric setting, despite evidence that such contracts do not reduce suicide risk and may create a false sense of security. MAT was available, but staff education was informal and inconsistent, contributing to variation in knowledge, confidence, patient education, documentation, and discharge planning. African American women experience a disproportionate burden of hypertension and hypertension-related complications, making timely follow-up and treatment intensification essential quality and equity priorities. This organization change process initiative addressed a critical safety practice gap regarding catheter-associated urinary tract infection (CAUTI) prevention in a 450-bed acute care facility. This gap was recognized based on evidence that the facility’s CAUTI Standardized Infection Ratio (SIR) was above the national benchmark—and daily necessity assessment compliance of only approximately 50%. This gap was attributed to an organizational interprofessional culture, which hindered timely device removal. Prolonged catheter dwell times increase patient morbidity, extend hospital stays, and expose the organization to severe financial penalties. Ineffective leadership transitions in for profit higher education erode employee morale, trust, and retention. Higher education business leaders recognize that ineffective leadership transitions can contribute to employee disengagement, reduced organizational commitment, and increased turnover, undermining institutional stability, workforce continuity, and organizational performance. Grounded in transformational leadership theory, the purpose of this qualitative pragmatic inquiry was to identify and explore the effective leadership strategies used by for-profit education institution leaders to retain key employees after a change in leadership.
