The purpose of this Doctor of Ministry action research project was to help reduce burnout and compassion fatigue among the clinical staff at Choices Healthcare. Burnout and compassion fatigue are becoming a healthcare crisis, and hospice agencies are no different. The study was open to any member of the clinical staff, which included chaplains, social workers, nurses, home health aides, and the team leaders of each team in the organization. This doctoral research project addresses the critical issue of burnout among nursing staff at the Kansas City Veterans Administration (KCVA). Burnout is often worsened by a lack of spiritual support, thereby negatively impacting both the well-being of nurses and the quality of patient care. This project study aims to mitigate these challenges by implementing a spiritual leadership resource toolkit, tailored to equip KCVA chaplains with creative strategies to provide spiritual care for nurses.

Moral distress is endemic to healthcare in the emerging post-covid era. Moral distress is defined as the knowledge of the correct or ethical action but the repeated inability to act in that manner. First studied in nursing, it has been found in nearly every facet of healthcare and is strongly associated with clinician burnout, intent to leave position, and behaviors that can in turn compromise patient safety. Research has indicated that pain management practices amongst registered nurses is inadequate across all age groups; however, it is frequently noted that children receive less analgesia in comparable situations than adult clients leading to harmful and costly consequences. Advance care planning allows for medical care at the end-of-life that is congruent with a patient’s wishes, values, and beliefs. Even with well-known organizations such as the Institute of Medicine recommending advance care planning completion and legislative encouragement such as the Patient Self-Determination Act of 1990, only approximately one-third of American adults have completed any form of advance directive. End of life planning is critical to ensure enactment of patient wishes, ethical patient treatment, and improved family acceptance of death. As such, this project addressed integration of reviewed literature of advanced planning directives to influence end-of-life care. Implementation guidelines and incorporation of the Physician Orders for Life Sustaining Treatment (POLST) paradigm was the central focus of this project.
