The Latino community in Monterey County faces significant obstacles to accessing hospice care, including language barriers and cultural misunderstandings. Many community members are unfamiliar with available hospice services or hold common misconceptions, such as equating hospice only with dying or abandonment. This capstone project, in partnership with the Central Coast Visiting Nurse Association (VNA), investigated these barriers through interviews conducted at a local senior event. Montage Health is a nonprofit healthcare provider in Monterey County, offering many services, including the Hospice of the Central Coast. This hospice agency supports older adults and their families with end-of-life care. Although Monterey County has a large Hispanic/Latino population, only a small number use hospice services—mainly due to language barriers, lack of awareness, and insurance concerns. This project focuses on helping the Hispanic community learn more about hospice care through surveys and Spanish-language presentations in the future. VNA Hospice of the Central Coast is a nonprofit that was created back in 1951. The staff at VNA are visiting about 75,000 homes per year. Throughout the years the staff have been seeing patients who reached the requirement of having 6 months or less to live. Working with Hospice patients can be hard for everyone but, the staff at VNA are seeing the patients from good days and bad days. Hospice of Santa Cruz County is regarded as the most experienced hospice program in the region. Currently, terminally ill patients who are not enrolled in any type of hospice care program are not receiving resources for services soon enough. Also, the transitions and palliative care patients at Hospice aren’t receiving adequate social or emotional support. This project piloted a Transitions and Palliative Care Services Volunteer Program with volunteers providing direct services through biweekly or monthly visits to patients that included companionship, emotional support and cognitively stimulating activities. The Central Coast VNA Visiting Nurse Association provides the highest quality health care to patients through physicians, hospice medical directors, nurses, medical social workers, spiritual care counselors, and volunteers. VNA currently does not have a volunteer program specifically for palliative care patients for their Palliative Care Team. Palliative care is specialized medical care for people living with a serious illness. The goal is to improve quality of life for both the patient and family by addressing physical, emotional, social, and spiritual needs (VNA, 2019). 94.9% of patients are referred to VNA because they need support for the patient and family …
