ABSTRACT Saponin-based vaccine adjuvants have attracted considerable attention owing to the clinical success of QS-21, a potent immunostimulant capable of inducing balanced humoral and cellular immune responses, yet its widespread application is limited by supply constraints, chemical instability, and dose-dependent toxicity. To address these challenges, we previously developed two semisynthetic saponin adjuvants, VSA-1 and VSA-2, derived from abundant Momordica saponins through a single-step modification, which exhibit QS-21–comparable immunopotentiating activity with improved accessibility and stability. Expected practice interventions are a type of antimicrobial stewardship intervention in which local guidelines regarding antimicrobial prescribing are developed and released at an institutional level to establish an institutional standard of care. An expected practice intervention to encourage use of oral and long-acting IV (PO/LAIV) antibiotics for serious infections was performed at the University of Alabama at Birmingham Hospitals in November of 2022. Allergens are non-infectious environmental proteins that can trigger T helper 2 (Th2) cell-mediated inflammation in atopic individuals. Allergens are structurally diverse and typically lack pathogen-associated molecules patterns (PAMPs), which are recognized by pathogen recognition receptors (PRRs). PRRs are the canonical sensors of the innate immune system that typically detect PAMPs in microbes, initiating signaling pathways that drive pathogen clearance. Because allergens generally do not activate PRRs in the same way as infectious agents, alternative detection mechanisms are likely responsible for their recognition and the subsequent initiation of Th2 cell-mediated inflammation. T follicular helper (Tfh) cells are a specialized subset of CD4+ T cells that sup-port germinal center (GC) B cells and promote antibody (Ab)-mediated immunity. There is growing recognition of the heterogeneity and functional diversity of Tfh cells, supported by accumulating evidence that highlights their context-specific roles across a range of disease settings. While their contribution to B cell responses is well-established, our data demonstrate that Tfh cells have a broader and more dynamic function. Air Pollution And Environmental Injustice Are Related To Respiratory Complications In Individuals With Sickle Cell Disease, Kelsey M. Maclin

Sickle cell disease (SCD) is a genetically inherited blood disorder affecting people of African descent. SCD is characterized by painful episodes and an acute lung injury known as acute chest syndrome (ACS). Patients with SCD have a life expectancy <19 years compared to Blacks without SCD. There is minimal research investigating air pollution and health complications in SCD. The aims of this research were to investigate whether pulmonary function is affected by exposure to ambient air pollution, whether higher air pollution concentrations are associated with hospitalizations for ACS, and whether environmental injustice impacts the respiratory health of children and young adults with SCD. Air pollution data on the criteria air pollutants (CAPs) was obtained from the US Environmental Protection Agency (EPA). Patient health data were obtained from Children’s Hospital of Alabama electronic medical records. Logistic regression, distributed lag non-linear regression, Granger Causality, and Getis-Ord Gi* hot spot analysis were used to analyze our data. The EPA’s EJScreen tool was used as a measure of exposure to environmental injustice. ArcGIS was used to map the EJScreen data. Exposure to short-term air pollution is significantly associated with a reduction in pulmonary function in children and young adults with SCD. Children and young adults exposed to a one-unit increase in the monthly average concentrations of sulfur dioxide (SO2) experienced an increase in the odds of having abnormal pulmonary function. We observed the trends of each criteria air pollutant in our study to be closely aligned with the trends in the hospitalizations for ACS. Most of our study participants with a history of ACS lived in areas burdened by greater environmental injustices. Specifically, the hot spots for ACS overlapped with the hot spots for PM2.5. Unequal exposure to environmental injustices leads to increases in hospitalizations for respiratory complications in individuals with SCD. Reductions in air pollution could be greatly beneficial to the pulmonary function and overall respiratory health of individuals with SCD. Further research is needed to examine associations between air pollution and hospitalizations for ACS in individuals with SCD.

Neonatal infection remains a leading cause of both neonatal morbidity and mortality worldwide, particularly in preterm infants. Probiotic bacteria have been demonstrated to protect against the development of neonatal intestinal dysbiosis, however, formulations and efficacy vary substantially, highlighting a critical gap in the current understanding of the mechanisms governing probiotic efficacy in this population. This gap is exacerbated by regulatory standards which favor economic expansion over scientific rigor. ORGANIZATIONAL FACTORS THAT IMPACT EMERGENCY ROOM SEPSIS BUNDLE COMPLIANCE IN U.S. HOSPITALS JASON L. KIMBRELL HEALTH SERVICES ADMINISTRATION ABSTRACT This in-depth study examines the organizational factors within U.S. hospital emergency rooms (ERs) that impact care teams’ adherence to established, time-sensitive sepsis protocols. The regression analysis revealed a concerning trend: for each 1% increase in annual patient walk-out rates, compliance with sepsis protocols declines by 2.10%. Additionally, the impact on poor ER throughput where the ESI level 3 length of stay increases by 1 minute, a hospital's compliance with the 3hr POA sepsis bundle decreases by .08%. COVID-19 is caused by the novel and easily transmissible SARS-C0V-2 virus. SARS-C0V-2 was first reported in the Chinese Providence of Hubei in December 2019 and in the United States in January 2020. After nearly 120,000 confirmed global cases and 4,200 deaths in 114 countries, the WHO declared COVID-19 a worldwide pandemic in March of 2020. By fall of 2020, COVI9-19 had become the third leading cause of death in the United States. Streptococcus mutans (S. mutans) is the primary etiological agent involved in the initiation and progression of dental caries. S. mutans forms robust biofilms on the tooth surface, and current preventative treatments, such as oral rinses, eliminate both pathogenic and commensal oral species. Therefore, there is an urgent need for therapeutics which selectively inhibit S. mutans biofilms while keeping the oral microbiome intact. The diadenylate cyclase (DAC) enzyme in S. mutans plays a key role in biofilm formation. DAC converts adenosine triphosphate (ATP) into cyclic di-AMP (c-di-AMP), a secondary messenger involved in several cellular pathways. Increased levels of c-di-AMP have shown …

Vaccine adjuvants are indispensable substances that help induce a fast, potent, and durable immune response when administered with vaccine antigens. Among the FDA-approved vaccine adjuvants for human use, three of the most recently approved ones contain saponin adjuvant QS-21. To address QS-21’s inherent drawbacks, we investigated the adjuvant activity of a series of saponin-based semisynthetic vaccine adjuvants, including VSA-1 and VSA-2, prepared from natural Momordica saponins I and II in one-step. The natural saponins precursors are isolated from the widely available and inexpensive seeds of Momordica cochinchinensis SPRENG. Climate change is set to profoundly impact urban infrastructure, causing increased flooding and storm damage, necessitating the reevaluation and adaptation of design methodologies to account for increasing precipitation extremes. This dissertation aims to enhance the resilience of urban infrastructure in Alabama by updating Precipitation Intensity-Duration-Frequency (IDF) curves, which are crucial for designing stormwater management systems, drainage networks, and flood control structures. Chronic myeloid leukemia (CML) and acute myeloid leukemia (AML) are blood malignancies characterized by clonal expansion and a compromised immune system. Tyrosine kinase inhibitors (TKI) revolutionized CML treatment; however, TKI discontinuation leads to relapses in half of the cases. Further, up to 10% of patients are TKI-resistant and develop a blast crisis resembling AML. Natural killer (NK) cells can clear malignant cells, representing a promising immunotherapy. However, NK cell activity is suppressed in leukemia, and the microenvironmental drivers of this impairment remain understudied. Current treatments mainly focus on symptom management rather than reversing the disease. Sepsis remains one of the most frequent causes of mortality and life-altering morbidity in humans of all ages globally. Though sepsis pathobiology is incompletely understood, it is clear that a unifying feature of all sepsis phenotypes is systemic vascular endotheliopathy and damage to the luminal glycocalyx of endothelial cells, or eGC, that leads to organ injury.
