Primary care providers are often the first contact for patients with neurodegenerative illnesses, however, they may not be aware of the relationship of certain diseases that may have an impact on their patients' longevity. This case report reminds clinicians of the association between frontotemporal dementia (FTD) and amyotrophic lateral sclerosis (ALS). Physicians should be aware of the association, because FTD commonly occurs first and may prepare clinicians to be alert to the signs of ALS in these patients, leading to earlier detection of ALS and the prescription of disease-modifying medication that may extend the lifespan of people with these diseases. …

Suicide loss impacts every aspect of the lives of those left behind, creating shattered assumptions and feelings of meaninglessness. In this perspective piece, the authors attempt to describe how the use or rituals in sessions can be therapeutically beneficial to suicide loss survivors across the lifespan. This piece provides a brief overview of the impact of suicide loss, theoretical approaches to suicide loss, and examples of rituals that can be used in sessions. COVID-19 closed colleges and forced faculty and students to learn a new way of teaching and learning. Like most institutions, Marshall University’s College of Education and Professional Development struggled to provide the practicum experience required for teacher preparation when most public schools shifted to remote learning. This qualitative study investigated the reactions of, and ongoing impact on, a university’s college of education’s administrators, faculty, undergraduates, the institution’s instructional designers, and the Counseling Center’s director when the campus closed spring of 2020 due to the pandemic. This qualitative study aimed to address the gap in the research related to end-of-life counseling by exploring the experiences of counselors working with clients at end of life. While counseling literature and education are lacking regarding end of life, many counselors will work alongside clients approaching death. Licensed professional counselors (LPCs) are now eligible to provide mental health counseling to older adults as Medicare providers. Many older adults receive home delivered (HDM) and congregate (CM) meal services through the Aging Services Network, though their mental health impact remains under examined. This study compared the mental health and suicide risk of older adults receiving HDM or CM services. Our cross-sectional study of 221 HDM and 150 CM recipients from 2018-2020 found significant group differences in psychological distress, t(366) = 3.28, p = .001, perceived burdensomeness; t(357.31) = 2.77, p = .006, and thwarted belongingness, t(357.96) …

Quetiapine is an atypical antipsychotic that is used in many clinical scenarios; it has been approved and has shown to be effective in the treatment of schizophrenia and bipolar symptoms. Additionally, it has several off-label uses, including the treatment of insomnia. In this case report, an elderly veteran was given quetiapine as a sleep aid. After the first dose, he demonstrated severe agitation. To the best of the authors’ knowledge, there have been limited reports of this reaction. Introduction

Advance Directives (AD) allow patients to maintain autonomy during incapacitation. Patients and their caregivers benefit from these documents in times of crisis. Overcoming barriers to AD completion and documentation can improve patient care quality.

Methods

A retrospective chart review was performed initially, after consolidation of the electronic health record (EHR) and after alteration of the EHR, to evaluate the availability of a patient’s medical power of attorney (MPOA), living will (LW), and code status.

Results

Baseline documentation of MPOA (7.33%), LW (6.00%), and code status (5.33%) within the outpatient EHR was low. After 2 cycles, this improved to 13.10%, …

Multigenerational legacy projects and activities facilitate meaning making and instill hope for further generations. Hospice and palliative care entities can use legacy projects to help terminally ill clients share how they want to be remembered. Legacy projects come in many forms including but not limited to scrapbooks, holiday rituals, and storytelling. In this perspective piece, examples of legacy projects will be explored as ways to process loss.

Telebehavioral health offers a unique opportunity to expand access to mental health services for older clients by addressing systemic barriers that often render mental health care inaccessible in later life. Although health interventions facilitated by technology, including telebehavioral health approaches, proliferated at the start of the COVID-19 pandemic, little guidance exists for counselors seeking to provide such services to clients in later life. In this manuscript, we describe challenges accessing mental health services, how telebehavioral health services can address these barriers, and practical consideration for delivering telebehavioral health approaches for counselors who work with older clients.

This article presents a case study about a fictional client’s experience of complicated grief. This article differentiates between complicated grief and normal (adaptive) grief. It also distinguishes between bereavement, grief, and mourning. The counseling process is described using developmental theory and narrative reconstruction to illustrate how to implement creative techniques.

Medicare fraud has been the cause of up to $60 billion in overpaid claims in 2015 alone. Upcoding occurs when a healthcare provider has submitted codes for more severe conditions than diagnosed for the patient to receive higher reimbursement. The purpose of this study was to assess the impact of Medicare and Medicaid fraud to determine the magnitude of upcoding inpatient and outpatient claims throughout reimbursements.
