Hospital medicine emerged in the United States in the 1990s to cope with the growing need for integrated care for complex and ageing hospitalized patients. The hospitalist model has rapidly spread out worldwide to European and Asian countries. It was also introduced to Taiwan in 2009 and obtained the government support since 2015 with a pilot project among more than two thirds of medical centers. Hospital medicine has also involved in medical education in the US, with medical schools and residency programs offer dedicated training in hospital medicine. Purpose: The main purpose of this study is to explore (1) the palliative care and end-of-life (EoL) nursing education in Taiwan, (2) the essential EoL contents for undergraduate nursing students. Methods: We used cross-sectional, descriptive research design in this study. The 'End-of-Life Nursing Education Survey' (EoLNES) Questionnaires was used to collect data. Study subjects were nursing teachers who had delivered EoL related courses in nursing schools in Taiwan. SPSS for Windows version 19.0 was used for analyzing data. Objective: The study aimed to construct content-valid item banks of geriatric medicine examination for medical students and residents to assist the development of the computerized geriatric examination system. Methods: In 2011-2013, totally 800 items used in the Taiwan Association of Gerontology and Geriatrics (TAGG) board examinations from 2002 to 2009 were collected. Thirty experts were invited to assess the appropriateness of test items for medical students and residents, while content validity of the items was computed by Lawshe's Content Validity Ratio (CVR) approach. The medical professions, among all professions, are exposed most frequently to death and dying. Despite the clear need, the design and implementation of thanatology courses is rarely explored in Taiwan. The aim of this report is to share our experience in teaching the course ”Medical Professions and Thanatology” offered in National Cheng Kung University Medical College. We discuss the course design, teaching strategy and preliminary results indicating teaching effectiveness. 慢性阻塞性肺疾病晚期病人之安寧緩和醫療需求, 亭儒 陳, 文郁 胡, 之勛 鄭, 慶餘 陳

本文藉由文獻查證與臨床經驗，探討慢性阻塞性肺疾病(chronic obstructive pulmonary disease, COPD)晚期病人之安寧緩和照顧需求。結果顯示COPD晚期病人在生理層面，忍受複雜而困難症狀且未被適當地處理；心理靈性層面，病人出現負向情緒反應的比例相當高，靈性需求常被忽略；社會層面，出現社會隔離及家屬代為做決策情形；溝通方面，呈現病人對於生命末期臨床資訊與末期照護議題討論皆不足。文中也提出安寧療護於COPD晚期病人之應用，包括介入時機、從全人觀點進行症狀處置、全面評估心理靈性需求、運用社區資源與溝通生命末期照顧議題等。未來可考慮於醫療專業人員之養成教育或繼續教育課程增加此部份的內容，以提升對COPD晚期病人末期照顧的知識與照顧品質。

目的：本研究主要是探討護理系學生對於安寧緩和醫療知識的認知程度，以及相關教育的介入對安寧緩和醫療知識之認知程度的差異情形。方法：本研究採自填之結構式問卷，並採用事前事後比較之研究分析，針對中部某一科技大學之護理系學生進行安寧緩和醫療知識之問卷調查。最後有效問卷為205份。結果：經由學習「緩和醫療護理」之課程後，後測平均值均有明顯的提高，表示教育的介入是有幫助的。個人特質中僅學制（四技/二技）部份有顯著差異，其餘變項與安寧緩和醫療知識皆無相關，經共變數分析結果，在經過教育介入後，不同學制學生的知識差異顯著縮小。但後測部份安寧緩和醫療教育知識仍顯不足的有四個方面，分別為「安寧緩和醫療對病人的幫助」、「安寧緩和醫療的法律問題」、「住進安寧病房的時機」以及「嗎啡的使用」。結論：課程設計中應多加強安寧緩和醫療相關法律、健保給付的宣導以及嗎啡正確的使用觀念，同時考慮不同學制的教育需求。

Palliative care is an important and emerging medical specialty in the area of end-of-life care. Almost all physicians are required to take care of a terminally ill patient at some point in their care, but previous studies from around the world have identified inadequate palliative care training for both medical students and general practitioners.
