Extension educators across the United States are being asked to expand their direct education efforts to include policy, systems, and environmental (PSE) changes. However, professional development opportunities and tools are needed to familiarize Extension professionals with PSE change approaches, build their capacity to implement PSE change, and make the process relevant to their work. We describe a planning tool developed for a unique multistate PSE change intervention training and designed to facilitate the process of PSE change implementation at the local level. An example of the tool and recommendations for others wishing to use it are included.

A sustainable Extension model for promoting health and managing lifestyle-related chronic diseases in Hispanic populations would help mitigate health disparities in Hispanic communities. Hispanic Balanced Living with Diabetes (HBLD) is a type 2 diabetes lifestyle management program that we tested in Virginia. Through postintervention focus group discussions, we assessed barriers faced by Hispanics when accessing health care services, satisfaction with HBLD, and feasibility of training members of the Hispanic community to become promotoras, individuals who help facilitate adoption of healthful behaviors. Building The Foundation For A Health Education Program For Rural Older Adults, Seung Eun Jung, Stephany Parker, Janice Hermann, Joshua Phelps, Yeon Ho Shin

We explored rural older adults’ perceptions of health to inform health promotion program development, using social marketing as our framework. Participants in seven focus groups viewed independence and holistic health as indicators of health and identified healthful eating and physical activity as actions to promote health. Barriers to these actions included physical limitations, social factors, financial considerations, motivation issues, and information confusion. Participants desired education that improves knowledge and skills, provides socialization opportunities, and occurs in familiar, affordable locations. The tool kit Mobilizing Rural Communities to Prevent Childhood Obesity is the product of a seven-state multidisciplinary research project focused on enhancing obesity prevention efforts by integrating community coaching into the work of rural community coalitions. The interactive tool kit is available at no cost both in print form and online, and it consists of five tutorials that present best practices and lessons learned throughout the research project. Extension professionals working within health promotion coalitions may wish to use or promote the tool kit. Coalition members can complete the activities contained in the tool kit individually or as a group.

Extension programming that incorporates both physical activity and dietary behaviors is necessary for the prevention of certain chronic diseases, including obesity. The purpose of the study presented here was to systematically identify the presence of physical activity objectives in the strategic plan for each Extension system in the United States. Few Extension systems (13) included physical activity in their strategic plans, yet lack of inclusion of physical activity in a strategic plan may limit the degree to which a system supports physical activity objectives and outcomes. African American women have been disproportionately impacted for decades by the obesity epidemic, which frequently leads to severe chronic diseases such as cardiovascular disease, diabetes and hypertension. The Systematic Screening and Assessment method (SSA) could be a useful approach to identifying Extension programs being implemented, to describing the nature of those programs, and to highlighting those that have proven effective. After a general discussion of SSA, we provide an application of SSA as recently used to identify 4-H Healthy Living programs with evidence of improving diet; physical activity; and alcohol, tobacco, and other drug use prevention outcomes. Our goal is to increase awareness about SSA and to demonstrate that it can be used to expand the knowledge and reach of Extension programs with evidence of impact.

This article examines the role of multi-level partnerships in implementing Faithful Families Eating Smart and Moving More, a faith-based health promotion program that works with low-resource faith communities in North Carolina. This program incorporates a nine-lesson individual behavior change program in concert with policy and environmental change. We offer an overview of the program, an evaluation of its effects on individual behavior changes and policy, and environment and practice changes and provide suggestions on lessons learned for Extension professionals who wish to partner with faith communities to promote nutrition and physical activity.

Rural Community Members' Perspectives On Mental Health And Aging: An Ecological Approach To Interpreting And Applying Focus Group Results, Tina Kruger, Deborah Murray, Faika Zanjani

The Mental Healthiness and Aging Initiative was created to develop resources addressing the mental health needs of older adults, particularly those in rural areas. Community members participated in focus groups discussing mental health and aging. Themes that emerged included the desire to maintain independence, the importance of emotional health and intergenerational activities, the value of sharing one's life history, remaining active and engaged, being respected, and educating younger community members about aging. The results are discussed in terms of ecological applications. Extension agents should find this data useful for developing programs and materials to promote mental healthiness in their communities

Worksite Wellness: Investing In Healthy Employees And Economies, Patricia Case

Worksites are structured environments with social norms and policies. These "norms" positively or negatively influence health habits such as physical activity and food choices. This article describes a worksite wellness program conducted and evaluated by Extension. The program demonstrated significant changes in body weight, waist circumference, body mass index, amount of exercise, and stages of change for exercise and intake of fruits and vegetables. This article challenges Extension to partner with employers who have a growing interest in contributing to a healthy work force. Also included are suggestions and tools for conducting worksite wellness programs.
