Education For Patients And Caregivers To Reduce 30-Day Readmissions Into Acute Care, Tyler W. Taylor

The purpose of this capstone project was to reduce 30-day readmissions of patients into the acute care setting by developing, enhancing, and implementing education materials for patients and their caregivers during occupational therapy treatment sessions. Patients who experience unplanned readmissions into acute care within 30 days are subjected to disruption in occupations, functional decline, occupational deprivation, and role strain and stress on caregivers and families. All patients with occupational therapy orders from a physician that matched certain criteria received this treatment. The present capstone project and experience aimed to develop and implement a 12-week program of social skills peer groups for school-aged children with autism at an outpatient pediatric therapy clinic. The program consisted of weekly peer group sessions targeting a range of social skill topics, such as emotional regulation, conversation skills, and collaborative play. The small group format provided an opportunity for participants to practice skills, learn from others, and obtain feedback on their performance within a supportive environment. The purpose of the Capstone experience was to develop and implement a five-week peer-led building resilience and social-emotional skills training program at a public high school. This program addressed high school students' knowledge, skills, and attitudes regarding resources, social-emotional skills, and resilience. A questionnaire was administered pre and post-program to measure effectiveness. The results indicated an increase in scores, demonstrating a clearer understanding in all three areas.

This 17-week capstone project was completed at The Parker Place and virtually managed throughout the entirety of the experience. The Parker Place is a pediatric endocrinology and lifestyle management facility located in Annapolis, Maryland. Completing a virtual capstone project did not hinder the progress and outcomes of this experience. The Healthy Habits program was developed at this site to implement an occupational therapy role to supplement the served population’s health and wellness goals. Simulated patients are a resource that is often used in other health care programs to give students early exposure and practical experience with a patient before they advance onto their fieldwork. The purpose of this study is to evaluate the usefulness of simulated patients in a Doctor of Occupational Therapy program by analyzing literature regarding simulated patients in various health care education programs. This literature review reviews the results of the use of simulated patients in health care education programs and their effect on the student’s education.

Interprofessional education (IPE) is defined by AOTA (2015) as an “occasion by which students from two or more professions learn about, from, and with each other to improve collaboration and quality of care”. IPE is closely related to and often overlaps with interprofessional collaboration (IPC) within healthcare settings. IPC “occurs when multiple health workers from different professional backgrounds work together with patients, families, [caregivers], and communities to deliver the highest quality of care” (World Health Organization [WHO], 2010, p. 7). The overall aim of IPE is to promote and develop effective IPC thereby improving patient outcomes (Khan, 2016).

School-based occupational therapists support academic success within and outside the classroom through accommodations/strategies and modifications to student’s class environment. Children with learning disabilities face a variety of occupational challenges in school including completing class assignments, following school routines, tolerating unexpected/expected sounds, and developing/maintaining friendships. The purpose of this capstone experience was to gain clinical skills for school-based occupational therapy and to develop academic, social, and life skills lessons for children with learning disabilities as well as a parent newsletter with carryover activities and strategies.
