This dissertation examines wellbeing, mental health, and natural resource (NR) dependency in rural Utah. The following questions are asked: How do wellbeing and mental health vary throughout the state of Utah, including by rural-urban location? What concerns do applied practitioners have regarding mental health in rural parts of the state? How do chronic stress and social dislocation contribute to mental health outcomes in rural NR dependent communities in the state? Four studies were conducted to pursue these research questions.

This dissertation includes two studies of rural age-related services in the state of Utah. The first study combines geospatial, demographic data (number of people over 55) and age-related services (hospitals, hospice providers, nursing homes, senior centers, and Areas Agencies on Aging) at the county level and census-tract level to determine localized differences in proportional access to age-related services. Higher and lower proportions are then predicted by contextual factors including rural/urban gradient, economic industry, and broadband access. Millard County, Utah, rural and sparsely populated, continues to experience challenges in recruiting and retaining primary health care providers. My study addressed the lack of a rigorous and systemic analysis of this problem by collecting and analyzing data from a series of semi-structured interviews conducted between January and March 2019. These interviews were with nineteen of the twenty-four known health care providers who began practice in the county from the mid1980s to 2018, as well as with four administrators.

Health disparities exist between rural and urban areas but geographic comparisons of mental health are less studied and conclusive. Maternal depression has not been examined by region or rurality in the United States but might be influenced by geographic locations due to the variance of social support and healthcare available in some locations compared to others. The research focuses on (1) whether rurality increases a mother’s risk of experiencing depression and (2) if region impacts a mother’s risk of depression. Rural residents experience rates of Type 2 Diabetes Mellitus (T2DM) that are considerably higher than their urban or suburban counterparts. Two primary modifiable factors, self-management and formal clinical management, have potential to greatly improve diabetes outcomes. “Community to Clinic Navigation to Improve Diabetes Outcomes,” is the first known randomized clinical trial pilot study to test a hybrid model of diabetes self-management education plus clinical navigation among rural residents with T2DM. Forty-one adults with T2DM were recruited from two federally qualified health centers in rural Appalachia from November 2014–January 2015. The purpose of this study was to examine change in symptoms of anxiety and satisfaction experienced by participants who received acceptance and commitment therapy (ACT) by using technologically assisted psychotherapeutic interventions (TAPI). TAPI utilizes the internet as a medium to make mental health services available and accessible to people residing in rural communities. The participants in this study were women who were experiencing severe levels of anxiety and lived in a rural community. Measures were taken at three different points in the study (pretherapy, posttherapy, and 6-months posttherapy). The purpose of this study is to assess the prevalence of low birth weight among non-Hispanic Blacks and non-Hispanic Whites along the rural/urban continuum, as well as the combined effect of being both non-Hispanic Black and residing in a completely rural county. Degree of social isolation and lack of support are proposed mechanisms for explaining disparities in low birth weight for Blacks in rural counties.

Using data from the National Longitudinal Survey of Youth 1979 (NLSY79) and the National Longitudinal Survey of Youth 1979 Child (NLSY79-C) datasets, logistic regression models were used to estimate the odds of low birth weight. …
