Doctor of Nursing Practice Projects

Hypertension affects approximately 70% of adults aged 65 years and older and remains a leading contributor to cardiovascular morbidity and mortality in the United States, highlighting the need for effective community-based hypertension management interventions. Despite the availability of evidence-based guidelines, gaps in education and self-management persist among older adults. This Doctor of Nursing Practice (DNP) quality improvement project, Senior Heart Health: Facilitating Hypertension Management, was designed to improve hypertension-related knowledge among adults aged 55 years and older through a structured, community-based health coaching intervention.

Objective(s): The purpose of this study was to explore the effectiveness of CHW/ nursing student teams in promoting secondary protection and improving access to care for residents of three urban underserved neighborhoods. The study also sought to measure CHW and resident satisfaction of such a program. Design and Sample: Quasi-experimental and nonexperimental designs were used for the study. Convenience samples consisted of residents who participated in the CHW program during 2005–2006, CHWs and residents who participated in the CHW program during 2005–2007 and a systematic random sample of residents across the three neighborhoods.
