Background: Multiple factors have created a perfect storm of health care provider shortages in the United States. Advanced practice registered nurses (APRNs), long established as high-quality, cost-effective health care providers, are meeting health care needs across the nation in a variety of settings, and in Wisconsin will be needed to augment the primary care workforce. With 5.7% of its registered nurses credentialed as APRNs, Wisconsin lags behind the national average of 8.7%. However, current capacity to educate this workforce is strained, requiring innovative data-driven clinical education models. The Physician Orders for LifeSustaining Treatment (POLST) form provides choices about end-of-life care and gives these choices the power of physician orders. The POLST form assures end-of-life choices can be implemented in all settings, from the home through the health-care continuum. The use of the POLST form was evaluated in a pilot study in nursing homes in two eastern Washington counties. Chart reviews and template analysis of interviews revealed the POLST form accurately conveyed end-of-life wishes in 19 of 21 cases. An informed consent process was evidenced in 16 of 21 cases, and the POLST form was congruent with residents' …

Relatively few Catholic couples in the United States use modern methods of natural family planning (NFP). So too, few Catholic physicians and health professionals prescribe the use of NFP methods for their patients. Reasons for low use of NFP methods include their perceived low efficacy; the complexity of learning, using, and teaching these methods; and the prolonged (and often unnecessary) required abstinence. Newer and simplified methods of NFP have been developed by physicians and scientists that are less complex and use modern technologies of detecting fertility and communicating instructions. Enhanced access and continuity are key components of patient-centered care. Existing studies show that several interventions such as providing same day appointments, walk-in services, after-hours care, and group appointments, have been used to redesign the healthcare systems for improved access to primary care. However, an intervention focusing on a single component of care delivery (i.e. improving access to acute care) might have a negative impact other components of the system (i.e. reduced continuity of care for chronic patients). Severe stress reactions in female patients presenting for annual pelvic examination may occur for a variety of reasons, including previous sexual assault, post-traumatic stress disorder, and /or intimate partner violence. The clinician's approach to care delivery is critical for establishing trusting relationships with female patients exhibiting these types of reactions. A case study is used to describe assessment and communication techniques to improve care delivery.

Screening For High-Risk Alcohol Use On Campus - A Translational Research Project, Janette Neuman

High-risk alcohol use occurs on college campuses across the United States. A 2008 Core Alcohol and Drug Survey performed on the campus of Indiana University-Purdue University Fort Wayne (IPFW) in northeast Indiana, found an annual prevalence rate of alcohol consumption by students of 82.3%. A commitment to reduce high-risk alcohol use by IPFW students is supported by IPFW leadership in congruence with Healthy Campus objectives. A translational research project using the Alcohol Use Disorders Identification Test (AUDIT) was implemented at IPFW’s nurse-managed health and wellness clinic in order to increase student awareness of high-risk drinking behaviors, administer risk-reducing interventions, and …

A variety of barriers contribute to delays in accessing health care in Indiana. Medically underserved areas and health professional shortage areas further potentiate barriers creating additional delays. Reduction in barriers to access is critical for promoting optimal health for Hoosiers. Military Veterans seek healthcare in many settings. Some qualified for services may use Veterans Administration hospitals and clinics, but many receive care in other private sector offices and facilities. In 2012, the White House’s Joining Forces initiative to support service members, veterans, and their families was extended to include education for nursing students. The National League for Nursing (NLN) with the support from the Laerdal Medical Corporation commissioned the development of four sets of simulation scenarios to help educate nursing students about the culture and needs of Veterans from certain eras.

A child's pattern of linear growth is one of the most sensitive indicators of health and well-being. However, many health care personnel use casual techniques and faulty instruments to measure children's growth and keep imprecise growth charts, making interpretation of growth patterns problematic.
