Building successful kinship partnerships and coalitions requires intentional effort to establish trust, align goals, and foster inclusivity. These collaborations are essential for addressing the unique and evolving needs of kinship families, who often face limited access to resources due to the informal nature of many caregiving arrangements. Effective partnerships prioritize clear missions, open communication, and active engagement with stakeholders, particularly those with lived experiences such as kinship caregivers. Research demonstrates that coordinated efforts among organizations enhance resource availability, improve advocacy capabilities, and strengthen service delivery. The hospital readmission rate is highest among underserved communities, and providing a continuum of care to these populations can yield a greater reduction in the readmission rate. Church and faith-based clinics often help underserved communities, and equipping them with the transitional care model (TCM) and an Advanced Practice Registered Nurse (APRN) with specialized training as a Faith Community Nurse (FCN) can implement whole-person care, improve the health of the community, and reduce hospital readmission.

Disparities in health and mental health service utilization among racial and ethnic groups in the U.S. are well documented, with many studies having identified accessibility to linguistically and culturally informed services as a key barrier to service utilization. Korean Americans in particular, being the most recent immigrant group of Asian Americans, reportedly suffer from higher rates of depression and anxiety than other Asian American groups. The teen pregnancy rate in Henry County, Tennessee has increased over the years. The purpose of this project was to develop an evidence-based family life education policy for possible adoption by the board of education to address the persistent high teen pregnancy rate for girls aged 15-17 in the county. This present study resulted in a revised policy that was based on a comprehensive policy termed Abstinence-Centered Plus Contraception.
