Caring For Children With Severe Cerebral Palsy: The Lived Experience Of Parents, Jason Eric Cook

The purpose of this study is to explore the lived experience of parents who care for their child withcerebral palsy who function at Gross Motor Function Classification System (GMFCS) Level V. Additionally, the study also explores the parent’s understandings and beliefs about pain and non-pain related emotional distress, and the meaning of caring, providing care, and providing comfort for their child with GMFCS Level V cerebral palsy. The study was conducted using a qualitative, hermeneutic phenomenology approach with semi-structured interviews and participant-initiated journaling. Cerebral palsy (CP) is a common developmental neurological disorder affecting about 2-3 children out of 1,000. CP is the result of infant brain damage or abnormal development resulting in impaired muscle control, coordination, tone, reflex, posture, and balance. These patients are unable to control motor movements of their muscles of mastication and facial expression, causing excessive drooling, clenching, bruxism, and other oral health-related issues. This lack of motor control affects their ability to swallow and often limits these patients to a liquid diet. This can lead to vitamin deficiencies and result in further developmental problems. Evidence Based Design (EBD) is an informed approach to design where designers base their decisions on quantitative and qualitative research. The design of the Universal Access Center for United Cerebral Palsy, discussed in this paper, encapsulates this approach to design. The five stages of EBD and how they were achieved within this project is defined. In the first phase, programming, current trends in healthcare as well as issues surrounding design for adults with Cerebral Palsy (CP) are discussed. To evaluate the relationship between dental erosion (DE) and scintigraphically detected gastroesophageal reflux disease (GERD) in patients with cerebral palsy (CP). Materials and methods: Included in the study were 21 CP patients with dental erosion and a control group consisting of 16 CP patients without dental erosion (total of 37 children; 19 male, 18 female; mean age: 12.1 ± 2.8 years). The severity of DE was scored and all patients underwent gastroesophageal reflux scintigraphy. Botulinum toxin is a promising treatment option for the management of drooling in patients with cerebral palsy. However, the effect of botulinum toxin type A in treating drooling of Taiwanese children with cerebral palsy has not been reported thus far. We injected single dose botulinum toxin into bilateral parotid glands of three children with cerebral palsy, and followed the results over a 9-month period. Drooling improved in all three patients without significant adverse effects during follow up. There were individual differences in terms of efficacy and duration of effect. Two patients had longer duration of effect than that reported in …
