Abstract

Patient education is an important factor in the management of Type 2 Diabetes. Successful outcomes can be causally related to proper patient education. Many times, patients are left with more questions than answers about their newly diagnosed condition. Some groups of patients are affected by prediabetes and diabetes more than other groups. Differences in health status or access to health care among racial, ethnic, geographic, and socioeconomic groups are referred to as health disparities. This is a major concern in the United States for health care providers.

The physiological factors that underlie T2D could have impact on specific brain pathways that support cognitive and emotional functioning. Oral health and diabetes are connected in a reciprocal relationship linked through the pathological mechanisms of bacteremia, inflammation, and the immune response. Dental patients experience a delay in receiving treatment at a university-based dental clinic secondary to obtaining required medical consultation from the patients’ private providers for complex medical conditions. Patient’s with a diagnosis of diabetes require significant lifestyle modification and education. The need for patient education has led to the development of diabetes self-management education (DSME) classes. Unfortunately, utilization of DSME classes is only 5% among Medicare beneficiaries and 6.8% among privately insured patients (Centers for Disease Control [CDC], 2018). The purpose of this evidence-based practice (EBP) project was to implement DSME via telehealth and assist with goal formation to help patients with uncontrolled diabetes achieve glycemic control by increasing access to crucial education.
