The amount of time nurse managers and senior leadership spend with frontline or bedside staff impacts their ability to empower, engage and retain staff. Improving nurse satisfaction in relationship to teamwork, perception of senior leadership and attitude about direct management should be a priority of all organizations. Leaders that recognize the power of an intergenerational and interdisciplinary workforce will be influential for dynamic change and visionary development. Advancing overall teamwork as a nursing department, developing a better rapport with senior leadership and improving the relationship between direct management and front-line nurses is in the best interest of patient care.

OBJECTIVE: To identify and evaluate available measures that can be used to examine the effects of management innovations in five important areas: autonomy, conflict, job satisfaction, leadership, and organizational climate. BACKGROUND: Management interventions target the context in which care is delivered and through which evidence for practice diffuses. These innovations need to be evaluated for their effects on desired outcomes. However, busy nurses may not have the time to locate, evaluate, and select instruments to measure expected nursing administration outcomes without research-based guidance. In the third in a series of articles exploring working conditions and quality improvement in nursing homes, 31 nursing homes were surveyed using an adaptation of the Competing Values Framework (CVF) Organizational Assessment. The CVF provides information about the organizational culture through describing dominant perceived values, distribution of values across organizational characteristics, and orientation of values toward flexibility. Staff reported a dominant group culture, reflecting a family and team orientation within their settings. Leaders, however, were more often reported to reflect a hierarchy value orientation, emphasizing efficiency of operations and following rules and procedures.

Leadership and clinical staff were surveyed to explore communication and leadership in nursing homes. Registered nurses and other professionals perceived communication as better than their nursing colleagues did. Overall, results suggest all factors of communication could improve. In terms of leadership, licensed practical nurses perceived less clarity of expectations, encouragement of initiative, and support than other groups. The study provides insight into what is organizationally necessary to improve quality of care in nursing homes.

The purpose of this study is to explore the relationship between nursing home staffs' perceptions of organizational processes (communication, teamwork, and leadership) with characteristics (turnover, tenure, and educational preparation) of the nursing home administrator (NHA) and director of nursing (DON). NHAs and DONs rate communication, teamwork, and leadership significantly higher than direct care staff do (registered nurses, licensed practical nurses, certified nurse aides [CNAs]). CNAs have the lowest ratings of communication and teamwork. Turnover of the NHA and DON is significantly and negatively associated with communication and teamwork. There are differences in perceptions of safety culture between healthcare leaders and staff. Evidence suggests that an organization's actual safety performance is more closely reflected in staff perceptions suggesting that frontline staff may be more aware than the leadership of actual patient safety challenges within their organization. Closing the perception gap between healthcare leaders and staff is critical to aligning the resources and strategies required to create a true culture of safety.

TOPIC: A culture of safety. PURPOSE: To explore the current culture of blame and what organizational elements must be impacted to move toward a culture of safety in the nursing home setting. METHODS: A mixed-method approach incorporating a case study and staff member survey results were used to explicate the organizational elements impacting the current nursing home culture. CONCLUSION: Nurse leaders can create an environment in which every member of the team feels a responsibility and an ability to insure that residents are safe by improving communication and participation in decision making.

Historically, graduate nurses are not attracted to older adults or geriatric care. Innovative strategies, such as the Young Gerontological Nurse Clinician Program (YGNCP) initiated by the John A. Hartford Center for Geriatric Nursing Excellence at the University of Iowa, are needed to promote geriatric nursing as a challenging and rewarding practice focus. The YGNCP, which provides early, positive, self-directed experiences for nursing students under the direction and supervision of faculty and expert clinical mentors, shows excellent potential for recruiting nurses to clinical and leadership positions in geriatric and geropsychiatric care.

The nursing and medical literature reveals considerable debate over whether graduate or continuing education is the appropriate level for nurse practitioner preparation. These arguments have been rhetorical since the research literature lacks data on the scope of practice of master's- and nonmaster's-prepared nurse practitioners. The purpose of this study was to examine possible differences between the two types of practitioners in one area--well-child care provided by pediatric nurse practitioners (PNPs). A total of 236 master's- and nonmaster's-prepared PNPs completed a background questionnaire and checklists of nursing activities involving infant and preschool well-child care. The study used a mixed methods research design, employing the triangulation convergence model, to investigate the possible factors contributing to successful transitions into the role of a new superintendency. Participating superintendents indicated that the training and education they received had adequately prepared them for the role of a new superintendent. Also, participants indicated that school board relations were important during the entry period and that their interaction with the board played an important part in the entry plan. INTRODUCTION: It has been suggested that the use of opinion leaders in the dissemination of information may be an effective method of changing clinical practice. Recent reviews on this topic, however, have found mixed results and have concluded that further research is needed to explore the circumstances that effectively utilize opinion leaders. A Connection Between Moral Imperative And Women’S Leadership?, Sarah W. Nelson, Patricia L. Guerra, B. Genise Henry

This paper describes a qualitative study of two school districts’ efforts to support school leaders in developing the knowledge and skills to analyze, understand, and transform system inequities. The results suggest that, despite being armed with knowledge and skills, leaders may be reluctant to act even when doing so is within the scope of their authority. This work draws from Fullan’s (2003) concept of moral imperative and examines the connection between moral imperative and women’s ways of leading. The findings have implications for educational leadership programs that seek to prepare school leaders capable of challenging and changing inequitable systems.

The organizational benefits of emotionally intelligent leaders and leadership are well documented. Likewise, the potential organizational gains from an investment in programmes designed to develop the emotional intelligence of leaders are strongly supported. The critical factor is whether the programmes for developing emotional intelligence will be effective and achieve the desired outcomes. This presentation is based on a study that explored how emotional intelligence can be developed and a subsequent program designed to develop emotional intelligence in a senior management group of a State Emergency Service in New South Wales (NSW), Australia.
