Exploring Primary Care Providers’ Compassionate Care And Their Evidence-Based Practice With Death, Grief, And Loss: Patient Experience And Perspective, Kathryn Krzykala

Research on primary care providers’ (PCPs) education regarding Death, Grief, and Loss (DGL) remains limited. Specifically, data is lacking regarding patients' perceptions and opinions on what they need and expect from their PCPs regarding grief and loss. This exploratory mixed-method study comprised 146 participants who had experienced death and non-death loss. It examined participants' perceptions of 1) PCP compassion, 2) PCP utilization of evidence-based grief and loss practices, 3) their own level of comfort discussing DGL with their PCPs during medical visits, and 4) their personal grief and loss experiences. Purpose: Medically tailored meal (MTM) programs generally demonstrate meaningful health outcomes for patients. There is a need to evaluate novel MTM delivery methods to understand participant and organizational-level impacts. The objectives of this study were to assess a shipped MTM pilot intervention, including 1) initial and ongoing client reach, 2) effects on clients’ health and behaviors, and 3) staff members’ perceptions of factors that influence intervention maintenance.
Methods: Based on the reach, effectiveness, adoption, implementation, maintenance (RE-AIM) framework, the mixed methods design included quantitative data collected from participants and qualitative data collected from MTM provider staff. The pilot took place …

Background: Effective pain management is essential for improving quality of life at the end of life. However, challenges persist globally, particularly in China, where palliative care remains underdeveloped. This study investigates pain prevalence and the associations between pain severity, place of death, and the quality of pain management among older adults in China, with a focus on regional and urban-rural disparities.
Methods: We used data from the 2014, 2016, 2018, and 2020 waves of the China Longitudinal Aging Social Survey. The final sample included 1,525 older adults who died during the study period. We conducted OLS regression analyses to examine …

Alzheimer’s disease (AD) is the leading cause of dementia in adults over the age of 65, and currently affects approximately 6.9 million people in the United States. By 2060, the number of people age 65 and older with AD is projected to reach 13.8 million, barring the development of disease-altering therapies to prevent, slow or cure AD. Thus, there is a need for progress in the understanding of AD pathophysiology, as well as in the development of diagnostic and therapeutic interventions. Introduction
Clinical prediction models assist healthcare providers in estimating disease likelihood using multiple patient-specific factors. While these models are increasingly used, few undergo the essential process of external validation, which limits their clinical utility. In toxicology, timely diagnosis is crucial and expertise scarce, making knowledge-based systems an excellent choice to support medical decision making. A knowledge-based system developed using data from the Florida Poison Information Center Network has shown promise but lacks external validation. Environmental sanitation is a fundamental component of infection prevention in long-term care facilities (LTCFs), where residents are at increased risk due to advanced age, underlying health conditions, and communal living arrangements. Despite its critical role, significant gaps remain in implementing environmental sanitation procedures and the regulatory frameworks that guide them.

Background/objectives: Family history is a known risk factor for multiple myeloma (MM) and its precursor condition, monoclonal gammopathy of undetermined significance (MGUS). Previous genome-wide association studies (GWASs) have identified 35 common loci associated with MM risk and 21 associated with MGUS. The objective of this study was to identify less common and rare genetic loci predisposing to MM/MGUS through whole-exome sequencing (WES)-based linkage analysis.

Rare variants affecting the epigenetic regulator KDM2B cause a recently delineated neurodevelopmental disorder. Interestingly, we previously identified both a general KDM2B-associated episignature and a subsignature specific to variants in the DNA-binding CxxC domain. In light of the existence of a distinct subsignature, we set out to determine if KDM2B CxxC variants are associated with a unique phenotype and disease mechanism. We recruited individuals with heterozygous CxxC variants and assessed the variants' effect on protein expression and DNA-binding ability. Outcomes in patients with relapsed/refractory (RR) AML are poor. We sought to investigate if CPX-531 in combination with venetoclax (CPX + VEN) was tolerable and effective in RR AML. This was a single institution phase 1b/2 trial of CPX + VEN. Patients aged ≥ 18 years with RR AML who were fit for intensive chemotherapy were eligible. Prior venetoclax exposure was allowed. The phase 1b portion followed a 3 + 3 design to identify the recommended phase 2 dose (RP2D) for the expansion cohort. Type I interferons are secreted in response to various stimuli and are used as a treatment for many diseases, including infections with the hepatitis B virus (HBV) and its satellite virus, hepatitis delta (HDV). HDV significantly aggravates HBV-mediated liver damage and is - in contrast to HBV - a strong inducer of interferon responses, including IFN-β. As the role of IFN- β in liver metabolism is so far ill explored, we studied its impact on hepatocyte metabolism in HDV-infected cultures. Introduction: Patients with advanced cancer and their spousal caregivers who parent minor children report unmet parenting concerns and increased psychological distress. Seeking to address these important supportive care needs, this RCT examined the feasibility, acceptability, and initial evidence for the efficacy of a novel psychosocial intervention.
Patients and methods: Patients with a metastatic solid malignancy and their spouses completed self-reported validated assessments of psychological symptoms and cancer-related parenting outcomes and were then randomized to the parent support intervention or a usual care (UC) group. Alzheimer’s disease (AD) is an irreversible and devastating neurodegenerative disease characterized by progressive synaptic, dendritic, glial, and neuronal loss, collectively leading to impairments in learning, memory, and deterioration of cognitive and behavioral functions. AD is the most common form of dementia accounting for 60-80% of all cases. AD affects approximately 7 million Americans and is the 6th leading cause of death in the US. Furthermore, by 2050, the US is projected to spend over $1.1 trillion on AD-related treatments. Chimeric antigen receptor (CAR) T-cell therapy is a novel immunotherapy modality that has shown remarkable response rates in refractory hematologic malignancies, including multiple myeloma (MM). Cytokine release syndrome (CRS) and neurotoxicity are well-described side effects of this therapy. CAR T-cell therapy recipients are also at increased risk for infections due to immune dysfunction, history of multiple lines of therapy, history of lymphodepleting chemotherapy prior to cell infusion, and prolonged B-cell aplasia. This research explores the evolution of the neo-XX/XY sex chromosome system of Amblyophallus exaltatus, a treehopper insect in the family Membracidae. Neo-XX/XY sex chromosome systems can evolve from an XX/X0 sex chromosome system via fusion events between the X chromosome and an autosome. Although cytological work has indicated the presence of a neo-XX/XY system within A.exaltatus, the chromosomal identity of the fused autosome and the molecular drivers behind this chromosomal fusion remain poorly understood. Assessing Medicare payment trends for cervical cancer care is important to mitigate the financial impact on Medicare. This multiyear cross-sectional study included 65 years and older cervical cancer patients in SEER registries diagnosed between 2010 and 2019 who had continuous Part A and B Medicare coverage at least 6 months before diagnosis and at least within the first year of diagnosis and were not enrolled in any Health Maintenance Organization (HMO) in this duration. This study investigates the common house fly (Musca domestica), a globally important insect that serves as both a decomposer and a mechanical vector for pathogens. The emergence of insecticide resistance, particularly knockdown resistance (kdr) mutations, has been reported in house fly populations, raising concerns for pest control. Additionally, house flies exhibit diverse bacterial communities, influencing pathogen transmission and environmental microbial dynamics. Importance: Molecular techniques, including next-generation sequencing, genomic copy number profiling, fusion transcript detection, and genomic DNA methylation arrays, are now indispensable tools for the workup of central nervous system (CNS) tumors. Yet there remains a great deal of heterogeneity in using such biomarker testing across institutions and hospital systems. Background: Nivolumab plus ipilimumab (nivo/ipi) is a standard of care first-line (1 L) therapy for patients with metastatic clear-cell renal cell carcinoma (ccRCC), but its role in patients with metastatic, non-ccRCC has not been fully defined. We report a single-institution experience with nivo/ipi in non-ccRCC.
Methods: Between November 2017 and February 2024, 55 patients with metastatic non-ccRCC received nivo/ipi at MD Anderson Cancer Center. The tumor response was assessed by blinded radiologists using RECIST v1.1. The overall response rate (ORR), progression-free survival (PFS), PFS milestone, duration of response (DoR), and overall survival (OS) were determined. Ayyubid And Mamluk Pottery From A Crusader-Period Subterranean Reservoir At Moẓa (Pp. 193–204), Benjamin J. Dolinka

Excavations in the underground Crusader water reservoir at Moẓa yielded a ceramic assemblage dating primarily to the Ayyubid period (1187–1250 CE), as well as some later Mamluk sherds (fourteenth to fifteenth centuries CE). The assemblage includes bowls, cooking pots, frying pans, amphorae, store jars, jugs, a juglet, a flask and lamps. The ceramic assemblage has strong affinities with contemporaneous Crusader and Frankish sites from the rural hinterland of Jerusalem. Other vessels of a later, Late Ottoman or British Mandate date were associated with an intrusive burial.

The Muristan is located in the heart of the Christian quarter of the Old City of Jerusalem. During the twelfth century CE the area was given to the Hospitaller Order of St. John, and it included the following major buildings: a hospital, ecclesiastical buildings and various large secular buildings. During excavations at the site, remains pertaining to these architectural units were discovered, including massive piers and ashlars, displaying typical Crusader diagonal tool marks, and stones bearing mason's marks. The meager finds from these excavations are an important contribution to the reconstruction of the Hospitaller Quarter of Jerusalem during the twelfth …

Grounded in a case study in Barbuda in the Caribbean, this research examines sustainability from the perspective of what arts and heritage can contribute to community engagement and local and broader understandings about the environment. This article documents a growing body of literature recognizing the role of arts and culture, including local knowledge and traditional ecological knowledge (TEK), in climate change endeavors. Art and TEK present expansive world views. Contextual information situates research done on the island of Barbuda pre- and post-Hurricane Irma. The first 5 decades of research in acute myeloid leukemia (AML) were dominated by the cytarabine plus anthracyclines backbone, with advances in strategies including allogeneic hematopoietic stem cell transplantation, high-dose cytarabine, supportive care measures, and targeted therapies for the subset of patients with acute promyelocytic leukemia. Aim: Neurofilament light chain (NfL) is a nonspecific sensitive biomarker of axonal damage.
Methods: This case series identified cancer patients with neurological complications who had serum NfL measurements and paired these results to outcomes.
Results: NfL serum levels were available in 15 patients with hematological malignancies or solid tumors. The neurological complications studied were immune effector cell-associated neurotoxicity syndrome, immune checkpoint inhibitor-related encephalopathy, anoxic brain injury, Guillain-Barre syndrome, hemophagocytic lymphohistiocytosis, transverse myelitis, paraneoplastic syndrome, central nervous system demyelinating disorder and chronic lymphocytic inflammation with pontine perivascular enhancement responsive to steroids. Over 81% of players have reported injuries linked to disc golfing, predominantly due to overuse. Since disc golf originated, there is a lack of research on the proper biomechanics of the disc golf throw. This descriptive, quantitative study seeks to be the first. We investigated 6 females and 35 males, aged 21-66, registered to compete in disc golf. The goal was to examine the kinetic parameters of healthy disc golfers to identify biomechanics likely associated with safety.
