Malingering includes the intentional feigning of psychiatric symptoms combined with motivation for external gain (American Psychiatric Association, 2013). Malingering is likely to occur in the justice system in both criminal and civil contexts, as there is increased opportunity for secondary gain in these settings. When individuals successfully malinger, it can be very costly for the United States. The rate of college students attempting to obtain Attention-Deficit/Hyperactivity Disorder (ADHD) diagnoses in order to gain access to prescription stimulant medications has shown a large increase over the past 20 years. Research has also shown that college students are frequently able to convincingly present themselves as meeting clinical criteria for ADHD with relative ease, meaning that the barrier to false presentation for the purpose of obtaining inappropriately prescribed medication is minimal. Many reliable tools have been established for the detection of malingered neurocognition; however, research on how pain may affect these tools is sparse. Noncredible performance and the intentional faking of symptoms during psychological evaluations have been observed in those seeking to obtain personal benefits. Cognitive deficits, such as impairments in attention are common in mental health settings and many seek an evaluation to rule out an attention-deficit/hyperactivity disorder. Previous literature establishes a base rate for malingering to be between 22-47% in adult ADHD evaluations (Sullivan, 2007; Suhr et al., 2008; Marshall et al., 2010). However, those faking or exaggerating ADHD can go unnoticed on self-report measures. An increase in the number of adults seeking ADHD evaluations as college students in recent years raises concerns of malingered or exaggerated attentional impairments. Some students may falsely report ADHD symptoms in an attempt to obtain academic accommodations as well as prescriptions for stimulant medications. Meanwhile, little attention has been given to the development of either self-reported measures or performance-based tests that can ensure the validity of reported or exhibited attentional symptoms. Performance validity test (PVT) inaccuracies can be explained by both test and extra-test (e.g., research design components) factors. Eye-tracking is a promising technology to enhance assessment of performance validity. Prior research has established that ocular behaviors are reliable biomarkers of (un)conscious cognitive processes. Experimental research on deception has shown that ocular behaviors reliably distinguish feigned concealment of information from honest responding. The present study examined the effectiveness of a Mild Traumatic Brain Injury (mTBI) coaching (i.e., providing information about mTBI symptoms) and motivational incentive (i.e., a $50 gift card lottery) on the Automated Neuropsychological Assessment Metrics (ANAM) test performance. The sample included a total of 162 undergraduate and graduate students enrolled in an introductory educational psychology course. The study examined the applicability of various validity measures and their ability to identify patterns of invalid responding (i.e., malingering) in an archival sample of civil forensic litigants. A latent clustering approach was used to create profiles comprised of the response patterns from validity measures of different domains including three performance validity tests (PVT), five symptom validity scales (SVT), and an embedded validity indicator (EVI). Latent class analysis (LCA) was used to enumerate subgroups of malingering and to assess differences between the subgroups.

One obstacle to the accurate diagnosis of ADHD in college students is malingering, although many symptom self-report measures do not contain feigning validity scales. The Infrequency Index (CII) for the Conners’ Adult ADHD Rating Scale–Self-Report: Long Version (CAARS-S:L) was developed for this purpose, although further validation of the index is needed. Another topic of interest in ADHD malingering research is the increasing use of online assessments. Little is known about how ADHD is malingered in an online format, particularly on the CAARS-S:L. Malingering is the purposeful fabrication of symptoms for secondary gain. Memory problems are the most reported symptom, and object recognition tests are often used in clinical settings to evaluate these claims. Past research has shown that eye movements can indirectly index memory, in that greater viewing is directed at studied stimuli 500-750 ms after display onset. The present study evaluated eye movements as a potential method of detecting feigned memory impairment. Forty-eight participants, half simulators, studied standardized images and took a memory test. Threats to performance validity test (PVT) security and utility have increased efforts to develop covert measures of performance validity. Response time (RT) is a promising covert measure to distinguish between honest and feigned performance; however, research investigating RT patterns on PVTs is sparse and troubled by methodological problems. Due to increased concern about malingered self-report of symptoms of attention-deficit/hyperactivity disorder (ADHD) in college students, there is a need for instruments that can detect feigning. The present study provided further validation data for a recently developed validity scale for the Conners’ Adult ADHD Rating Scale (CAARS), the CAARS Infrequency Index (CII). The sample consisted of 139 undergraduate students; 21 individuals with diagnoses of ADHD, 29 individuals responding honestly, 54 individuals responding randomly (full or half), and 35 individuals assigned to malinger. The CII demonstrated modest sensitivity to malingering (.31-.46) and excellent specificity to ADHD (.91-.95). Sequential application of validity …

This study used subtests of the WAIS-IV to detect evidence of malingering. Developing reliable tests for malingering could significantly reduce costs paid to malingering individuals. A within-group known-group design was used. There were 3 known-group conditions. The first group (G1) was instructed to take the tests honestly. The second group (G2) was asked to fake a cognitive disability while taking the tests. The third group (G3) took the tests while undergoing the cold-pressor method (hand immersed in cold water) of inducing pain. Analysis of variance was performed. BACKGROUND: Forensic neuropsychiatric assessment requires thorough consideration of malingering and response bias. Neuropsychiatric evaluations are complicated due to the multiple domains in which symptoms and impairment present. Moreover, symptom exaggeration in these evaluations can also present along various symptom domains (e.g., psychological, neurocognitive, somatic). Consequently, steps must be taken to ensure adequate coverage of response bias across all three domains of function.
PURPOSE: The following article reviews the conceptualization of malingering in neuropsychiatric settings, as well as various approaches and measures that can be helpful in the assessment of malingering and response bias.

College students may feign symptoms of ADHD to gain access to stimulant medications and academic accommodations. Research has shown that it can be difficult to discriminate malingered from genuine symptomatology, especially when evaluations are based only on self-report. The present study investigated whether the average student given no additional information could feign ADHD as successfully as those who were coached on symptoms. The current study examined the efficacy of various neuropsychological measures for differentiating ADHD and comorbid ADHD from malingered ADHD in a large state university sample. The sample consisted of 23 nonclinical individuals assigned to malinger ADHD (NLM), 9 nonclinical individuals responding honestly (NLH), 22 individuals with diagnoses of ADHD only (ADHD-H), 9 individuals with comorbid ADHD/Learning Disorder presentations (ADHD-LD), and 13 individuals with comorbid ADHD/Anxiety presentations (ADHD-ANX). The present study investigated the utility of the MMPI-2-Restructured Form (MMPI-2-RF) over-reporting scales (F-r, Fp-r, Fs, and FBS-r) and the Response Bias Scale (RBS) to detect symptom exaggeration among litigants claiming chronic pain. Utilizing the Bianchini et al. (2005) multi-method criteria for Malingered Pain Related Disability (MPRD), patients were classified along a continuum ranging from incentive only to definite malingering. Malingering classification was found to have a significant effect on MMPI-2-RF over-reporting scales, particularly the RBS, Infrequent Symptoms Scale (F-r), and Infrequent Somatic Responses scale (Fs), which supports the use of the MMPI-2-RF in forensic disability evaluations.

Malingering is a frequently encountered problem of faking psychological or physiological symptoms or exaggerating existing conditions for external gain. Malingerers typically are seen in clinical and forensic settings and create a burden to our society due to loss of economic resources or professional time.
