Dissociation And Sexual Trauma: The Moderating Role Of Somatization, Amineh Abbas

This study examined various types of trauma, with an emphasis on sexual trauma across the lifespan, in a clinical sample of male and female adult outpatients assessed for trauma, somatization, and dissociation. Two hundred forty-five adult outpatients at the University of Tennessee Psychological Clinic were administered the Dissociative Experiences Scale (DES), the Traumatic Experiences Checklist (TEC), and Symptom Checklist-90-Revised (SCL-90-R), as part of the routine intake procedure. Of those individuals, 200 patients completed the questionnaires correctly and were included in the final study sample. The aim of this exploratory study was to link psychoanalytic theories of trauma and its impact on the mind with psychobiological research of how trauma lives in the body. The study has expanded on prior research (Cramer, 2003) to evidence that defense mechanisms do in fact moderate the relationship between stress and physiological response, and that there are likely individual differences in physiological response to traumatic stress. This study goes further to identify the psychological concomitants of these individual differences within an adult population exposed to potentially traumatic events (PTEs), and their proclivity for using different defense mechanisms. A recent framework known as the 4D-Model of Trauma-related Dissociation (Frewen & Lanius, 2014) differentiates between symptoms of clinically significant distress based on whether the symptoms do or do not intrinsically exemplify trauma-related altered states of consciousness (TRASC). Undergraduate students (n = 342) participated in an online survey and completed several measures assessing childhood experiences and psychological symptoms. Female PTSD patients (n = 25) completed similar measures before entering treatment. Within the student sample, NWC symptoms were endorsed as occurring more frequently than TRASC symptoms. On average, symptoms of NWC were more strongly intercorrelated than symptoms of TRASC. …

Current literature on the relationship between posttraumatic symptoms and dissociation that occurs during the time of a trauma, or peritraumatic dissociation, appears to be contradictory and inconclusive. Conflicting findings in the empirical literature that disagree on the nature of this association (whether peritraumatic dissociation is a risk factor for PTSD or a neutral or even protective evolutionarily-derived phenomenon) may originate from the lack of conceptual clarity regarding the construct of dissociation, and lack of differentiation between peritraumatic dissociation and dissociation that persists after a traumatic event. No study has taken an updated and comprehensive review of Donnel Stern’s writings. An investigation of his philosophical assumptions, locating Stern’s work socioculturally and historically, along with an elucidation of Stern’s background in traditional psychoanalytic literature and clinical practice, brings out the meanings and enigmas present in his theories of dissociation, enactment, unformulated experience, multiple self-states, and reflection. Stern has offered one of the best-integrated theoretical models in relational psychoanalytic theory. An examination of his theories within the theoretical traditions to which he makes claim (psychoanalytic, interpersonal, hermeneutic, postmodern, and democratic) helps elucidate the challenge posed by relational psychoanalysis to …
