Background During voluntary limb movement, feedforward postural activation of the trunk and hip musculature occurs to maintain postural stability. This is called an anticipatory postural adjustment (APA). Onset of trunk muscle APAs is delayed in symptomatic individuals with persistent low back pain (LBP). It is not clear how spatial APA organization is affected, or if adaptations in APAs persist between symptomatic episodes. We used two versions of a supine active leg raise task to examine central modulation of APAs. One version of the task, the supported leg raise (SLR) does not elicit trunk/hip APAs. Modic changes (MCs), defined as bone marrow lesions adjacent to the vertebral endplate seen on MRI, have been associated with back pain. There are different types of MCs, with different histopathological correlates. This community-based study sample comprised 110 men from the Twin Spine Study, with baseline, 5-year, and 15-year follow-up MRIs. Following training and reliability testing, 1320 endplates (T12-S1) were evaluated for MC presence, type, and dimensions at each time point. Results demonstrated that Type 2 Modic Changes (MC2) were the most common type, with approximately half located at the two lowest disc levels. However, Type 1 Modic Changes (MC1) …

A. Background of Knowledge: Nevada ranks 8th highest in uninsured population in the United States. People who are uninsured may be more affected by low back pain (LBP) than their insured counterparts. Volunteers in Medicine of Southern Nevada (VMSN), a non-profit organization, serves uninsured people in Nevada. VMSN partnered with UNLV Physical Therapy (UNLVPT) to provide a back-school for patients. UNLVPT proposed a service-learning model to provide both education and treatment for VMSN patients with LBP. Purpose: The purpose of the study was to investigate the temporal relationship between psychogenic stress and perceived intensity of chronic low back pain (LBP). LBP was chosen as a pilot condition prefacing a larger research effort to determine which variants of chronic pain may manifest latent psychogenic modulation based on previous studies of neuropathic pain conditions.
Methods: The study consisted of five subjects, one male and four females, ranging from 19-32 years old and chronicity of pain ranging from 5 months to 6+ years since diagnosis. Over 12-15 weeks, participants completed daily visual analog pain, stress, and pain-related function scales. …

Background and Purpose: The number of people living with an amputation in the United States is rising at a rate of almost 200,000 cases a year [1]. The incidence of low back pain (LBP) in individuals with lower limb amputation (LLA) has been shown to be much greater than in the general population at 52%- 72% [2-4]. Low back pain in collegiate athletes has been reported at a rate of 37% from a wide array of sports including soccer, volleyball, football, swimming, and baseball. Whereas, in a military population the prevalence of low back pain is 70% higher than the general population. Compensatory movement strategies are often used as an attempt to reduce pain. Though compensatory movement strategies may effectively reduce pain, they are often associated with altered lower extremity loading patterns. The purpose of this study is to highlight the use of power training in a patient with low back pain and Parkinson’s disease.
Power training demonstrated a more significant change in gait speed, mobility, and function compared to other phases in rehabilitation. After power training was implemented at visit 11, there was a dramatic increase gait speed with the 10-meter walk test, a reduction in Oswestry Disability Index score (ODI), and a decrease in Timed Up and Go (TUG) time. The increase in 10 MWT gait speed and reduction in ODI and TUG score were greater than their MDC values …

The purpose of this case study is to determine the effects of core stabilization and pelvis mobility training for a 28-week pregnant female to improve function and decrease pain.
Low back pain during pregnancy is a common occurrence that affects a majority of the population.1 Over the five treatment sessions, the patient was able to tolerate a full ten-hour work shift and sleep through the night. She also showed improvements in muscle strength and flexibility. As seen in non-pregnant females, the patient was able to have a decrease in pain and improvement in function with these techniques.4

Mental health problems and psychosocial stressors have been increasing in Soldiers and are also top causes for medical discharge. The Dead Bug exercise is performed in physical therapy clinics to restore lumbar spine stability and core strength in patients with lower back pain (LBP). The aim of this study was to evaluate the efficacy of using electromyography (EMG) feedback to enhance proper mechanics during the Dead Bug exercise. Sixteen healthy, college age students volunteered as subjects for the study. Objective: To measure the effects of dry needling to the lumbar multifidus (MF) muscle for any change in resting and contracted thickness of the transversus abdominis (TrA) muscle as well as symptoms and disability in individuals with low back pain (LBP). Background and Purpose. Currently, stationary bicycles do not incorporate exercise for the abdominal and low back musculature. An experimental stationary bicycle, the Magnus Cycle, has been developed to increase trunk muscle activation and, at the same time, provide aerobic conditioning. The purpose of this study is to assess the activity of the rectus abdominus, external oblique, erector spinae, rectus femoris and biceps femoris muscles during a stationary cycling setting and during a tilt-in-space setting of the Magnus Cycle.

The purpose of this study was to assess the association between innominate torsion (asymmetric anteroposterior positioning of the pelvic innominates) and the Gillet, standing forward flexion, sitting forward flexion, and supine-to-sit tests. A sample of 21- to 50-year-old patients with low back pain (n=150) and a comparison group of patients with upper-extremity impairments (n=138) were recruited from outpatient physical therapy facilities. The purpose of this independent study is to define presently existing theories linking cigarette smoking and low back pain. It also provides a detailed description of nicotine and its physiological and psychological effects.
There are four theories outlined in this study which could serve as explanations for the correlation of cigarette smoking and low back pain. The first is the chronic cough associated with smoking. This cough increases mechanical stress including higher disc pressures and poor union and healing following spinal fusion surgery. At one time or another most adults have back pain. Despite the fact that back pain is as prevalent as the common cold, it has been as puzzling as the Bermuda Triangle. Low back pain has an enormous socioeconomic impact in this country. Even with advancement in diagnostic technology, the incidence and severity of low back pain continues to increase. The intervertebral disc plays an important causative role in the production of low back pain.
