Purpose

Access site complications following peripheral vascular intervention (PVI) are associated with prolonged hospitalization and increased mortality. Prediction of access site complication risk may optimize PVI care; however, there is no tool designed for this. We aimed to create a clinical scoring tool to stratify patients according to their risk of developing access site complications after PVI.

Methods

The Society for Vascular Surgery’s Vascular Quality Initiative database yielded 27,997 patients who had undergone PVI at 131 North American centers. Clinically and statistically significant preprocedural risk factors associated with in-hospital, post-PVI access site complications were included in a multivariate logistic regression …

Background: A monoclonal antibody, trastuzumab targets the human epidermal growth factor receptor 2 (HER2) oncogene that is overexpressed in 25–30% of breast cancers. In combination with first-line therapy, trastuzumab resulted in significant improvement in survival outcomes for those with HER2-positive metastatic breast cancer. Due to its improvement in outcome and prolonged survival, trastuzumab has been established as standard of care in both adjuvant and metastatic settings. However, along with common adverse events, trastuzumab has been found to be associated with cardiotoxicity. Insulin resistance (IR) and inflammation are associated with an increased risk of cardiovascular disease and may contribute to obesity cardiomyopathy. The earliest sign of obesity cardiomyopathy is impaired left ventricular (LV) diastolic function, which may be evident in obese children and adolescents. However, the precise metabolic basis of the impaired LV diastolic function remains unknown. Description Multicenter, randomized, open-label, active-controlled study in which women undergoing an urgent or elective medical procedure to open a blood vessel in the heart (PCI) will be randomized to either transradial (through the arm) or transfemoral (through the leg) access site

Eligibility Inclusion Criteria-including but may not be limited to:
o Must be older than 18 years of age
o Must be female
o Undergoing urgent or elective PCI
o Appropriate performance scores

Purpose The primary study purpose is to assess the success and complication rates of the transradial approach to PCI in women compared with the transfermoral approach.

Background: The principal determinants of oxygen uptake (VO2) kinetics are controversial, with dynamic changes in central and peripheral factors mediating oxygen supply and utilisation suggested to be limiting. Background As late provision of specialist care, prior to starting dialysis therapy, is believed to be associated with increased morbidity and mortality, a systematic review was undertaken to evaluate clinical outcomes relating to early versus late referral of patients to nephrology services.

Methods: Cochrane Central Register of Controlled Trials (CENTRAL), MEDLINE and EMBASE were searched up until September 2008 for studies of early versus late nephrology referral in adult (>18 years) patients with chronic kidney disease. Early referral was defined by the time period, at which patients were referred to a nephrologist.

The aim of this work was to systematically review the effect of exercise training on BNP and NT-pro-BNP levels in patients with left ventricular dysfunction.
