Primary root canal therapy has been shown to be a predictable procedure with a high degree of success 1-4, however failures can occur after treatment. Treatment options for the preservation of teeth that have had previous endodontic treatment but demonstrate persistent disease include non-surgical (orthograde) or surgical (retrograde) endodontic retreatment, assuming the tooth is restorable, periodontally sound, and the patient desires to retain the tooth. The purpose of this project was to study the antibacterial effect of the short exposure of 5.25% NaOCl and 17% EDTA on eight strains of Enterococcus faecalis in extracted human teeth. The crowns of 240 single rooted teeth were removed and the canals were instrumented to ISO 0.36. The canals were irrigated with 5.25% NaOCl and 17% EDTA, autoclaved and separated into eight groups of 30 teeth consisting of 20 experimental teeth, 5 positive and 5 negative controls. This study evaluated the effect of residual gutta percha on the retention strength of prefabricated post cemented with four different cements. Forty intact mandibular single canal canines or premolars were selected for standardized size and quality, endodontically treated, and decoronated to the cementoenamel junction. A post space was prepared to accommodate Para post XP size 4 (Coltene, Whaledent Inc., Cuy Falls, Ohio) to a depth of 8 mm. The purpose of this investigation was to determine the antimicrobial effect of Biopure MTAD (a mixture of a tetracycline isomer, an acid, and a detergent) as a final irrigant on eight strains of Enterococcus faecalis (E. faecalis) in contaminated root canals of extracted human teeth. In addition, the Minimum Inhibitory Concentration (MIC) and Minimum Lethal Concentration (MLC) of MTAD were also measured on the same strains of bacteria. Bacterial lipopolysaccharide (LPS) has been implicated in the pathogenesis of pulpal and periapical diseases. Several studies have evaluated the ability of various medicaments and irrigants used during root canal therapy to chemically detoxify LPS. The purpose of this study was to evaluate the ability of some endodontic sealers to detoxify LPS. This investigation compared the quantities of TNF-α released by human monocytes exposed to LPS or LPS pre-treated with sealers. Diabetes Mellitus (DM) has been implicated as a factor affecting healing. The purpose of this study was to use the healing of experimentally exposed pulps subsequent to pulp capping as a model to determine the effect of DM on healing.

Twenty-two Sprague-Dawley rats were divided into two groups of eleven animals each. In one group, DM was induced by injection of 70 mg/KG of streptozotocin. Periradicular surgery is a viable option to save natural teeth when non-surgical treatment fails or when endodontic retreatment is not feasible or contraindicated. Laboratory and animal studies have demonstrated that MTA is biocompatible, provides an excellent seal against penetrating bacteria, and promotes hard tissue healing. The purpose of this study was to provide long term (>3 years) clinical evidence for its use as a root-end filling material in endodontics. Root perforations during root canal therapy have a significant negative impact on long-term prognosis of teeth. Proper treatment is critical in managing these accidental procedures. Two variations of MTA (Gray and White) have been introduced to the dental market. The purpose of this study was to histologically compare the in vivo response to immediate furcation perforation repair using gray or white MTA in beagle dogs.

While previous studies have shown that presence of bacteria in systemic circulation can cause infective endocarditis, there is no information on the specific amount of bacteria necessary to cause this condition. The purpose of this study was to establish the minimum level of circulating bacteria that will cause colonization of damaged heart valves in rats. Fifty-two Sprague-Dawley rats were anesthetized and their heart valves were damaged using an established protocol. Bacteria have been shown to play an important role in the development of pulp and periradicular disease. The source of the infective bacteria is most often the oral cavity. Previous studies have detected bacteria in teeth that are structurally intact that seemingly have no communication intraorally. Anachoresis has been proposed as a mechanism for localizing circulating bacteria to such teeth. In several previous bacteriologic anachoresis studies, bacteria have been found to localize in inflamed dental tissues. The criticism most commonly associated with these investigations has been the potential contribution of contaminant bacteria resulting in false positive findings.

Previous studies have evaluated the effects of various hemostatic agents on radicular tissues following apical surgery. The purpose of this study was to examine the early osseous responses to seven hemostatic agents under identical conditions in rabbits. Sixteen adult male New Zealand white rabbits were used for this study. Following anesthesia, utilizing a midline submandibular incision, standardized bony defects were prepared on the lateral-superior regions of the mandible. Test materials were placed using standardized volumes and exposure times. Replacement resorption (RR) occurs following severe injury to the periodontal ligament (PDL). It has been suggested that inflammatory reactions precede RR. The purpose of this study was to examine and compare histologically the effect of topical application of two anti-inflammatory medications and stannous fluoride (SF) on the development of RR. Thirty-four roots of beagle dog mandibular 2nd, 3rd, and 4th premolars received root canal therapy. They were then extracted and left to bench dry for one hour. Periradicular lesions develop as a result of the interaction between root canal irritants and the immune system. The purpose of this study was to investigate the periradicular response of immunodeficient rats to their oral flora. Twenty Sprague-Dawley rats received a total body irradiation dose of 750 cGy one week prior to the investigation. The pulps of the mandibular first molars of these animals and six nonirradiated rats were exposed mechanically with a ½ round bur and left open to the oral cavity. Mineral Trioxide Aggregates (MTA) was recently developed at Loma Linda University. It has been shown to possess good sealing ability when tested with dye, bacteria, and a fluid filtration technique.

Endotoxin is a component of the cell wall of Gram negative bacteria. It has been proposed that it may play a role in the pathogenesis of periradicular lesions. Previous studies have demonstrated that MTA can prevent the leakage of bacteria. However, the bacterial products were not tested during these studies.

Several animal models have been proposed to develop pulpal inflammation. Procedures recommended in these models have many variables and have not produced consistent results. The purpose of this study was to create pulpal inflammation using the Nd:YAG laser.

Forty-five cuspids of 12-18 month old beagle dogs were lased at 5, 7.5, and 10 watts (W) for 20 seconds with a 600 micron contact probe. Five cuspids were not lased but were used as controls. The purpose of this study was to evaluate the effects of the resorbable membrane Guidor™ and the osteogenic protein hOP-1 on hard tissue healing following periradicular surgery in cats.

Surgical endodontic therapy was performed on the maxillary canine teeth of twelve cats. After orthograde root canal therapy, flap reflection, osteotomy, resection of the root end, root end cavity preparation and the placement of a root end filling, eight of the surgical sites were treated with Guidor™ and eight with hOP-1. In general it is accepted that oral tissues have an abundant blood supply; however, there is little information quantifying blood flow to these regions.
