Objectives: The purpose of this study was to understand how staff responded to individualized patient falls prevention education delivered as part of a cluster randomised trial, including how they perceived the education contributed to falls prevention on their wards.
Design: A qualitative explanatory study.
Methods: 5 focus groups were conducted at participatory hospital sites. The purposive sample of clinical staff (including nurses, physiotherapists and quality improvement staff ) worked on aged care rehabilitation wards when a cluster randomised trial evaluating a patient education programme was conducted. The United States faces an immigrant population explosion with more foreignborn residents compared to any country in the world. Each immigrant enters with individual and cultural health beliefs and, as they seek health care, often prefers to receive care from someone who understands and supports their cultural beliefs and customs. Nurses comprise the largest segment of healthcare providers. Within this population of nurses, the number of ethnically diverse nurses continues to stagnate at astoundingly low levels. An evaluation of screen time and media use in children. This project looks to education healthcare practitioners about the effects of current media practices in the United States on children. A presentation was given to healthcare practitioners at a family medicine practice and a brochure was created to distribute to families in the waiting room. Practitioners reported an increase in knowledge regarding the subject of screen time in children, an increase in their comfort level in in discussing these effects with patients, and increase in their ability to effectively communicate the media guidelines of the American Academy of Pediatrics.

Physical inactivity is one of the leading causes of chronic preventable diseases, which are the leading causes of death in the United States. Vermont, specifically Washington County, is no exception: Heart disease, stroke and diabetes are 3 of the Top 6 causes of death. In Washington county, VT. Approximately 60% of residents are overweight or obese; >40% do not meet recommended physical activity guidelines. Physical inactivity is highest among patients with obesity, diabetes, hypertension and heart disease. Vaccination against Human Papillomavirus (HPV) is able to protect against 70% of strains causing cervical cancer and and 90% of strains causing genital warts, yet vaccination rates for boys and girls are still well below average vaccination against many other preventable diseases. The focus of this project was to assess issue with transportation as a potential barrier to accessing health care in Bangor, Maine. This pilot project involved interviews with community members and providers, as well as evaluated the needs in terms of transportation for accessing primary care. Research regarding this need was conducted through anonymous patient surveys, conducted by the author, and was used to guide the recommendations for future interventions that would address transportation issues in Bangor and the Penobscot County area.

As finding breakthrough small-molecule drugs gets harder, drug companies are increasingly turning to “large molecule” biologics. Although biologics represent many of the most promising new therapies for previously intractable diseases, they are extremely expensive. Moreover, the pathway for generic-type competition set up by Congress in 2010 is unlikely to yield significant cost savings.

Background
Many governments and health care providers worldwide are enthusiastic to develop medical tourism as a service export. Despite the popularity of this policy uptake, there is relatively little known about the specific local factors prospectively motivating and informing development of this sector.
Objective
To identify common social, economic, and health system factors shaping the development of medical tourism in three Central American and Caribbean countries and their health equity implications.

Background
Health inequalities disproportionally affect indigenous people in Guatemala. Previous studies have noted that the disadvantageous situation of indigenous people is the result of complex and structural elements such as social exclusion, racism and discrimination. These elements need to be addressed in order to tackle the social determinants of health. Background
Early detection of emergent influenza strains is a global health priority. However, maintaining active surveillance is economically and logistically challenging. While community-based surveillance is an attractive alternative, design and operation of an effective epidemiological surveillance program requires community engagement that can be linked to public health reporting and response. We report the results of a study in rural Guatemalan communities aimed at identifying opportunities for and barriers to community engagement in disease surveillance.
Methods

วัตถุประสงค์: เพื่อเปรียบเทียบสมรรถภาพปอดของผู้ป่วยสูงอายุโรคปอดอุดกั้นเรื้อรังก่อนและหลังของการเข้าร่วมโปรแกรมการจัดการตนเอง และเปรียบเทียบระหว่างกลุ่มที่เข้าร่วมและไม่ได้เข้าร่วมโปรแกรมการจัดการตนเอง\n \nแบบแผนงานวิจัย: กาวิจัยแบบกึ่งทดลอง\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่างเป็นผู้ป่วยสูงอายุโรคปอดอุดกั้นเรื้อรังที่มีระดับความรุนแรงของโรคอยู่ในระดับ 2-3 จำนวน 40 คน ที่มารับการรักษาแผนกตรวจผู้ป่วยนอก หน่วยโรคปอด โรงพยาบาลจุฬาลงกรณ์ แบ่งเป็นกลุ่มทดลอง 20 คน และกลุ่มควบคุม 20 คน จับคู่ในด้าน เพศ อายุ ระดับความรุนแรงของโรค และชนิดของยารักษาโรคปอดอุดกั้นเรื้อรัง กลุ่มทดลองได้รับโปรแกรมการจัดการตนเอง เป็นระยะเวลา 8 สัปดาห์ และกลุ่มควบคุมได้รับการพยาบาลตามปกติ เครื่องมือที่ใช้ในการทดลอง ได้แก่ โปรแกรมการจัดการตนเองที่สร้างขึ้นโดยใช้แนวคิดการจัดการตนเองของ Bourbeau (2008) ประกอบด้วย แผนการสอน คู่มือการจัดการตนเอง และวีดิทัศน์การบริหารการหายใจและการออกกำลังกาย ซึ่งผ่านการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิ 5 คน เก็บรวบรวมข้อมูลโดยใช้เครื่องตรวจสมรรถภาพปอด วัดความเร็วสูงสุดของลมหายใจออก (Peak expiratory flow rate: PEFR) วิเคราะห์ข้อมูลโดยหาค่าเฉลี่ย ร้อยละ ส่วนเบี่ยงเบนมาตรฐาน และสถิติทดสอบที\n \nผลการวิจัย: 1) สมรรถภาพปอดของผู้ป่วยสูงอายุโรคปอดอุดกั้นเรื้อรัง ภายหลังได้รับโปรแกรมการจัดการตนเองสูงกว่าก่อนได้รับโปรแกรมจัดการตนเอง อย่างมีนัยสำคัญทางสถิติที่ระดับ .05\n2) สมรรถภาพปอดของผู้ป่วยสูงอายุโรคปอดอุดกั้นเรื้อรังภายหลังการทดลอง กลุ่มที่ได้รับ\nโปรแกรมการจัดการตนเองสูงกว่ากลุ่มที่ได้รับการพยาบาลปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05\n \nสรุป: โปรแกรมจัดการตนเองส่งผลให้ผู้ป่วยสูงอายุโรคปอดอุดกั้นเรื้อรังมีสมรรถภาพปอดดีขึ้นและดีกว่าการพยาบาลตามปกติ\n

วัตถุประสงค์: เพื่อศึกษาประสบการณ์การเป็นพยาบาลผู้ป่วยเด็กวิกฤต\n \nรูปแบบงานวิจัย: การวิจัยเชิงคุณภาพแบบปรากฏการณ์วิทยาเชิงตีความ\n \nวิธีดำเนินการวิจัย: ผู้ให้ข้อมูล คือ พยาบาลวิชาชีพที่ปฏิบัติงานในหอผู้ป่วยวิกฤตกุมารศัลยศาสตร์ โรงพยาบาลตติยภูมิแห่งหนึ่ง ในเขตกรุงเทพมหานคร ติดต่อกันเป็นระยะเวลานาน 5 ปีขึ้นไป จำนวน 14 ราย เก็บรวบรวมข้อมูลโดยการสัมภาษณ์แบบเจาะลึก และการบันทึกเสียงสนทนา ข้อมูลที่ได้นำมา วิเคราะห์ข้อมูลเชิงเนื้อหาตามวิธีการของ Colaizzi\n\nผลการวิจัย: พยาบาลผู้ป่วยเด็กวิกฤตให้ความหมายการเป็นพยาบาลผู้ป่วยเด็กวิกฤตไว้ว่า จะต้องเก่ง มีความรู้ ความเชี่ยวชาญ เป็นคนที่มีความละเอียด รอบคอบ ช่างสังเกต และจะต้องมีใจรักและใส่ใจในการดูแลผู้ป่วยและครอบครัว สำหรับประสบการณ์การเป็นพยาบาลผู้ป่วยเด็กวิกฤต พบว่า ประกอบด้วย 4 ประเด็นหลัก ประเด็นแรก คือ ความสามารถเฉพาะทาง ซึ่งเป็นสมรรถนะในบทบาทหน้าที่เชิงวิชาชีพของพยาบาลผู้ป่วยเด็กวิกฤต ประเด็นที่ 2 ความเครียด ซึ่งเกิดจากการขาดความรู้ และทักษะในการปฏิบัติงาน เครียดจากผู้ร่วมงาน และเครียดจากการติดต่อประสานงานกับแพทย์ ประเด็นที่ 3 การปรับตัว พบว่า พยาบาลผู้ป่วยเด็กวิกฤตจะมีการปรับตัวเข้ากับลักษณะงานที่ทำ ผู้ร่วมงาน และตารางการทำงาน เมื่อปรับตัวได้ ทำให้ความเครียดลดลงและมีความมั่นใจในการทำงานมากขึ้น ประเด็นสุดท้าย พบว่า สิ่งที่ทำให้พยาบาลผู้ป่วยเด็กวิกฤตคงอยู่ในงานได้ เพราะบรรยากาศในการทำงานที่อบอุ่นเป็นกันเอง มีความเอื้อเฟื้อต่อกัน รับรู้ถึงคุณค่าของการเป็นพยาบาล มีความภาคภูมิใจในองค์กรและมีความสุขในการทำงาน และการทำงานเป็นความมั่นคงของครอบครัว\n\nสรุป: ข้อมูลที่ได้จากการศึกษาในครั้งนี้ทำให้เข้าใจถึงประสบการณ์การเป็นพยาบาลผู้ป่วยเด็กวิกฤตได้ชัดเจนมากขึ้น ความรู้ที่ได้รับสามารถนำไปเป็นพื้นฐานให้ผู้บริหารทางการพยาบาลนำไปพัฒนาเป็นกลยุทธ์สร้างความพึงพอใจและเพิ่มอัตราการคงอยู่ในงานของพยาบาลผู้ป่วยเด็กวิกฤต\n

Background : Radical cystectomy is the treatment of choice for muscle-invasive and refractory superficial bladder carcinoma. Objectives : To report the perioperative outcomes and complications of open radical cystectomy. Materials and Methods : We retrospectively reviewed all medical records of patients who underwent open radical cystectomy for bladder carcinoma at our hospital between January 2003 and June 2013. เหตุผลของการทำวิจัย : ปัจจุบันน้ำยาตรวจหมู่โลหิต anti-M ของศูนย์บริการโลหิตแห่งชาติ สภากาชาดไทย ผลิตขึ้นมาจากการฉีดกระตุ้นกระต่าย (rabbit polyclonal antibody) แอนติบอดีที่ผลิตได้มีความจำเพาะไม่คงที่และความแรงที่ได้ ค่อนข้างต่ำ วัตถุประสงค์ : เพื่อพัฒนาการผลิตน้ำยาตรวจหมู่โลหิตโมโนโคลนัล anti-M วัสดุและวิธีการ : ใช้เทคนิคการเชื่อมเซลล์ระหว่าง B-lymphocyte ที่มีการสร้าง anti-M กับเซลล์มะเร็งสายพันธุ์เดียวกัน (JMS-3) ทำให้ได้เซลล์สายพันธุ์ที่คงทน ในการสร้าง anti-M จากนั้นนำน้ำเลี้ยงเซลล์ที่ได้จากโคลนใหม่ (clone 1F9) มาศึกษาคุณสมบัติต่าง ๆ โดยเปรียบเทียบกับ rabbit polyclonal anti-M ของศูนย์บริการโลหิตแห่งชาติเดิม และ monoclonal anti-M ของบริษัท ต่างประเทศ ผลการศึกษา : พบว่าน้ำยา human monoclonal anti-M (clone 1F9) ที่พัฒนาขึ้นใหม่ มีความแรงของแอนติบอดี (potency) ดีกว่าน้ำยาตรวจหมู่โลหิต rabbit polyclonal anti-M ของศูนย์บริการโลหิตแห่งชาติเดิมและเทียบเท่าหรือ ดีกว่า monoclonal anti-M ของบริษัทต่างประเทศ ด้านความเร็วของ แอนติบอดี (avidity) ในการเกิดปฏิกิริยากับเม็ดเลือดแดงหมู่โลหิตที่มี M-antigen positive บนสไลด์ พบว่ามีความเร็วเฉลี่ย 1 - 2 วินาที ซึ่งไม่แตกต่างกันในน้ำยาทั้ง 3 ชนิด ด้านความจำเพาะของแอนติบอดี (specificity) พบว่าให้ผลถูกต้องแม่นยำร้อยละ 100 โดยให้ปฏิกิริยา ผลบวกกับเม็ดเลือดแดงหมู่โลหิตที่มี M-antigen positive จำนวน 800 ราย ให้ปฏิกิริยาผลลบกับเม็ดเลือดแดงหมู่โลหิตที่มี M-antigen negative จำนวน 400 ราย และยังให้ปฏิกิริยาผลลบทั้งหมดกับ papainized identification panel cells (Lot …

During pregnancy, women frequently anticipate birth and motherhood with unrealistic images and expectations which are formed by personal experience and society’s portrayal of motherhood (e.g., books and movies). How these expectations impact the postpartum period for women has not been well studied. However, research has identified a link between maternal expectations, reality, and negative emotional responses. When expectations are met, the experience of motherhood may be a positive one. "According to the Institute of Medicine report medications are the most common medical intervention. However, drug-related morbidity and mortality now represents a major public health challenge due to the ineffective and unsafe consequences of medication use. Morbidity in this case relates to disease and illness associated with drug therapy whereas mortality relates to death associated with drug therapy. It is also estimated in the US that drug-related morbidity and mortality cost about $200 billion annually in health care expenditure.

Inhibitor of differentiation (Id) proteins are DNA-binding transcription factors involved in cellular proliferation, migration, inflammation, angiogenesis and fibrosis. However, their expression and role in the cornea is unknown. The present study was undertaken to characterize the expression of Id proteins and their interactions with the pro-fibrotic cytokine Transforming Growth Factor β1 (TGFβ1) and anti-fibrotic cytokine, bone morphogenic protein 7 (BMP7) in human cornea. Human donor corneas procured from Eye Bank were used. Id proteins were localized in human corneal sections using immunofluorescence. วัตถุประสงค์: เพื่อศึกษาผลของโปรแกรมส่งเสริมการรับรู้สมรรถนะแห่งตนต่อพฤติกรรมการดูแลตนเองของเด็กวัยเรียนโรคธาลัสซีเมีย\n \nแบบแผนงานวิจัย: การวิจัยกึ่งทดลอง\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่างเป็นเด็กวัยเรียนโรคธาลัสซีเมีย อายุ 7-12 ปี ที่มารับบริการที่คลินิกโรคเลือด แผนกผู้ป่วยนอก โรงพยาบาลมหาราช นครศรีธรรมราช โดยแบ่งเป็นกลุ่มควบคุม และกลุ่มทดลอง กลุ่มละ 25 คน กลุ่มควบคุมได้รับการพยาบาลตามปกติ กลุ่มทดลองได้รับโปรแกรมส่งเสริมการรับรู้สมรรถนะแห่งตน ซึ่งประกอบด้วย 4 ขั้นตอน คือ 1) การให้คำแนะนำและใช้คำพูดชักจูง 2) การใช้ตัวแบบ 3) การจัดประสบการณ์ของการประสบความสำเร็จในการลงมือทำกิจกรรมด้วยตนเอง และ 4) การประเมินสภาพทางร่างกายและการกระตุ้นทางอารมณ์ เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล ได้แก่ แบบสอบถามพฤติกรรมการดูแลตนเองของเด็กวัยเรียนโรคธาลัสซีเมีย และแบบสอบถามการรับรู้สมรรถนะแห่งตนในการมีพฤติกรรมการดูแลตนเองของเด็กวัยเรียนโรคธาลัสซีเมีย ซึ่งผ่านการตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ จำนวน 5 คน มีค่าความเที่ยงเท่ากับ .84 และ .78 ตามลำดับ วิเคราะห์ข้อมูลโดยหาค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และการทดสอบที\n\nผลการวิจัย: 1) คะแนนเฉลี่ยพฤติกรรมการดูแลตนเองของเด็กวัยเรียนโรคธาลัสซีเมียหลังได้รับโปรแกรมส่งเสริมการรับรู้สมรรถนะแห่งตนสูงกว่าก่อนได้รับโปรแกรมฯ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (t =9.518p < .05) 2) คะแนนเฉลี่ยพฤติกรรมการดูแลตนเองของเด็กวัยเรียนโรคธาลัสซีเมียในกลุ่มที่ได้รับโปรแกรมส่งเสริมการรับรู้สมรรถนะแห่งตนสูงกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (t =9.402p < .05)\n\nสรุป: โปรแกรมส่งเสริมการรับรู้สมรรถนะแห่งตนส่งผลให้เด็กวัยเรียนโรคธาลัสซีเมียมีพฤติกรรมการดูแลตนเองพฤติกรรมการดูแลตนเองที่ดีขึ้นและดีกว่าการพยาบาลตามปกติ\n

วัตถุประสงค์: เพื่อศึกษาปัจจัยทำนายความล่าช้าในการมารับบริการที่โรงพยาบาลของผู้ป่วยกลุ่มอาการหัวใจขาดเลือดเฉียบพลัน\n\nรูปแบบการวิจัย: การวิจัยความสัมพันธ์เชิงทำนาย\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่างเป็นผู้ป่วยกลุ่มอาการหัวใจขาดเลือดเฉียบพลัน จำนวน 148 คน คัดเลือกกลุ่มตัวอย่างแบบเจาะจง เก็บรวบรวมข้อมูลโดยการสัมภาษณ์ผู้ป่วยกลุ่มอาการหัวใจขาดเลือดเฉียบพลันที่มาตามนัดและนอนโรงพยาบาลด้วยภาวะหัวใจขาดเลือดเฉียบพลันไม่เกิน 2 เดือน โดยใช้แบบสอบถามการตอบสนองต่ออาการต่างๆ มีค่าสัมประสิทธิ์อัลฟ่าของครอนบาคเท่ากับ .67 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา และสมการถดถอยโลจิสติก\n\nผลการวิจัย: พบว่า ผู้ป่วยกลุ่มอาการหัวใจขาดเลือดเฉียบพลันส่วนใหญ่ (64.2%) เป็นเพศชาย อายุเฉลี่ย 53.59 ปี ประมาณ 2 ใน 3 (64.9%) ของผู้ป่วยกลุ่มอาการหัวใจขาดเลือดเฉียบพลันมีความล่าช้าในการมารับบริการที่โรงพยาบาล โดยรวมตัวแปรที่ศึกษาทั้งหมดร่วมกันทำนายความล่าช้าได้ร้อยละ 52.1 (Pseudo R2 = 0.521, p <0.05) ประกอบด้วย อายุ เพศ การตอบสนองด้านการรับรู้ การตอบสนองทางอารมณ์ การตอบสนองทางพฤติกรรม และการตอบสนองทางอารมณ์หรือพฤติกรรมของบุคคลอื่น โดยมี 5 ตัวแปร ที่สามารถทำนายความล่าช้าในการมารับบริการที่โรงพยาบาล ได้แก่ การตอบสนองทางพฤติกรรมโดยพยายามผ่อนคลาย (OR = 35.566, B = 3.571, p <.05) อยู่กับครอบครัวในขณะเกิดอาการ (OR = 0.041, B = -. 3.191, p <.05) การตอบสนองทางอารมณ์หรือพฤติกรรมของบุคคลอื่นแนะนำให้พักผ่อนและทานยา (OR = 0.056, B = -2.883, p <.01) อยู่ที่บ้านขณะเกิดอาการ (OR = 8.738, B = 2.168, p <0.01) รอให้อาการหายไป (OR = 2.722, B = 1.019, p <0.01)\n\nสรุป: พยาบาลควรส่งเสริมให้ประชาชนมีความรู้ ความเข้าใจเกี่ยวกับอาการแสดงของภาวะหัวใจขาดเลือดเฉียบพลันเพื่อป้องกันความล่าช้าในการมารับบริการที่โรงพยาบาล\n

วัตถุประสงค์: เพื่อเปรียบเทียบอุบัติการณ์ความคลาดเคลื่อนในการบริหารยาและความพึงพอใจในงานของพยาบาลวิชาชีพก่อนและหลังการใช้หลักฐานเชิงประจักษ์ในการบริหารยาในหอผู้ป่วยวิกฤต\n \nรูปแบบงานวิจัย: การวิจัยแบบก่อนทดลอง\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ พยาบาลวิชาชีพที่ปฏิบัติงานในหอผู้ป่วยวิกฤต โรงพยาบาลพญาไท 2 จำนวน 18 คน เครื่องมือที่ใช้ในดำเนินการทดลองประกอบด้วย แผนการอบรม คู่มือการใช้หลักฐานเชิงประจักษ์ในการบริหารยาในหอผู้ป่วยวิกฤต แบบกำกับการทดลอง เครื่องมือที่ใช้ในการรวบรวมข้อมูลเป็นแบบบันทึกอุบัติการณ์ความคลาดเคลื่อนในการบริหารยาในหอผู้ป่วยวิกฤต และแบบสอบถามความพึงพอใจในงานของพยาบาลวิชาชีพ ตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ และหาค่าความเที่ยงของแบบสอบถามความพึงพอใจในงานด้วยสูตรสัมประสิทธิ์แอลฟาครอนบาคได้เท่ากับ .91 วิเคราะห์ข้อมูลโดยใช้สถิติค่าร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และการทดสอบสถิติค่าที\n \nผลการวิจัย: 1) จำนวนอุบัติการณ์ความคลาดเคลื่อนในการให้ยาผิดขนาดภายหลังการทดลอง (2.44 ครั้งต่อ 1000 วันนอน) น้อยกว่าก่อนการใช้หลักฐานเชิงประจักษ์ในการบริหารยา (8.47 ครั้ง ต่อ 1000 วันนอน) และความคลาดเคลื่อนในการให้ยาผิดเวลาภายหลังการทดลอง (2.44 ครั้ง ต่อ 1000 วันนอน) น้อยกว่าก่อนการใช้หลักฐานเชิงประจักษ์ในการบริหารยา (5.65 ครั้งต่อ1000 วันนอน) 2) คะแนนเฉลี่ยของความพึงพอใจในงานของพยาบาลวิชาชีพหลังการใช้หลักฐานเชิงประจักษ์ในการบริหารยาในหอผู้ป่วยวิกฤตสูงกว่าก่อนการใช้หลักฐานเชิงประจักษ์ อย่างมีนัยสำคัญทางสถิติระดับ .05 (t = 5.43, p <.05)\n\nสรุป: การใช้หลักฐานเชิงประจักษ์ในการบริหารยาในหอผู้ป่วยวิกฤตช่วยลดอุบัติการณ์ความคลาดเคลื่อนในการให้ยาผิดขนาดและผิดเวลา ส่งผลให้พยาบาลมีความพึงพอใจในงานมากขึ้น \n

BACKGROUND AND AIMS: Understanding the molecular pathogenesis of hepatocellular carcinoma (HCC) is essential to identify therapeutic targets. A hepatitis B virus (HBV) related double transgenic murine model was developed. METHODS: Liver specific expression of HBV X protein (HBx) and insulin receptor substrate 1 (IRS1) was achieved and transgenic mice were followed from birth to age 21 months. Liver and tumor tissue were assessed for histologic changes as well as activation of signal transduction pathways by qRT-PCR and multiplex ELISA protein assays. RESULTS: Overexpression of HBx and IRS1 stimulates liver cell proliferation in the double transgenic mice. Only the male mice …
