A Case Study Of Cross-Jurisdiction Resource Sharing: The Merger Of Two Tuberculosis Clinics In East Tennessee., Anne Kershenbaum, Margaret A. Knight, Martha L. Buchanan, Janet Ridley, Paul C. Erwin
Frontiers in Public Health Services and Systems Research
Cross-jurisdiction resource sharing is considered a possible means to improve efficiency and effectiveness of public health service delivery. A merger of the Tuberculosis (TB) clinics of a rural and a metropolitan jurisdiction in East Tennessee provided an opportunity to study service provision changes in real time. A mixed methods approach was used, including quantitative data on latent TB treatment outcomes and qualitative data from staff interviews, as well as documentation of changes in staffing time in TB services. Results showed a mix of efficiency changes, indicating probable increased pressure on key service providers after the merger, in addition to expected …

What “Community Building” Activities Are Nonprofit Hospitals Reporting As Community Benefit?, Erik Bakken, David Kindig, Jo Ivey Boufford
Frontiers in Public Health Services and Systems Research
In 2008, the Internal Revenue Service (IRS) revised and standardized the reporting policy for community benefit expenses for nonprofit hospitals. These expenses are required for tax exemption. At that time, the IRS designated some categories of activities as non-eligible as a community benefit, but still mandated their reporting on hospitals’ Form 990, the annual tax filing for nonprofit organizations. One such category was community building, which encompasses a broad range of nonmedical determinants of health and an important potential source of population health revenue. This is the first study to analyze community-building dollars at any level, examining New York State’s …

Avoiding 30-Day Readmissions Of Acute Mi Patients Utilizing Cardiac Rehabilitation, Patricia T. Forsberg
Avoiding 30-Day Readmissions Of Acute Mi Patients Utilizing Cardiac Rehabilitation, Patricia T. Forsberg
Doctor of Nursing Practice (DNP) Projects
A significant number of Acute Myocardial Infarction (AMI) patients were readmitted to an urban San Francisco Medical Center within 5-7 days post discharge this year. Two of the main identified causes were symptom management issues and medication instructions, both of which are part of the discharge instructions. It’s not surprising that 80% of all discharge teaching is forgotten by patients by the time they hit the parking lot. With the recommended timeframe for post discharge follow up appointments at 48 to 72 hours post discharge and as those appointments are not typically available within the recommended timeframe, patients are more …

Minimizing Avoidable Interruptions During Medication Administration, Jaleel Anne Arnado
Master's Projects and Capstones
The objective of this CNL Internship Project is to improve patient outcomes and nurse satisfaction by minimizing the avoidable interruptions that occur during medication administration. The microsystem is a pediatric and adult medical-surgical overflow unit at a large, urban teaching hospital in Northern California. A series of surveys for nurses and patients and observations of medication administrations were conducted to assess barriers during mediation administration. It was determined phone calls interrupt a nurse the most during medication administration. Interruptions leave the medication administration process vulnerable to errors because it disrupts the nurse’s workflow and thought process. Because most pediatric medication …
Examination Of Capital Murder Jurors’ Deliberations: Methods And Issues, Keith Price, Susan Coleman, Gary R. Byrd
Examination Of Capital Murder Jurors’ Deliberations: Methods And Issues, Keith Price, Susan Coleman, Gary R. Byrd
Administrative Issues Journal
The study of capital juries remains a subject of critical interest for the public and for legislative and judicial policy makers as well as legal scholars and social scientists. Cowan, Thompson, and Ellsworth established one of the standard methodologies for examination of this topic in their 1984 seminal study by observing the subjects’ debate about conviction in a death penalty case utilizing mock juries; other scholars employed different techniques to add more information in the late 1990s. Yet, the question of death qualification and prosecutorial bias remains open to inquiry. This preliminary study found evidence to support bias toward conviction …

Health Management and Policy Presentations
Successful strategies to scale up and spread complex community-level interventions require an understanding of the resources required for implementation, how best to distribute them among supporting institutions, and how resource consumption and distribution varies across settings. This session reviews methods and early findings from the RWJF’s Public Health Delivery and Cost Studies (DACS) Initiative, which includes 12 inter-related studies examining the causes and consequences of variation in the costs of delivering complex community-level prevention strategies across more than 300 community settings in 12 states. Findings from these studies highlight the value of studying the economics of implementation, the measurement and …

Development And Mental Health Care Services: A Case Study At Rtccd, Hanoi, Michelle Lafortune
Independent Study Project (ISP) Collection
Vietnam’s rapid development since 1986 has had both positive and negative impacts on Vietnam’s mental health services. A health system was put into place and is expanding to overcome many challenges. Conversely, development has also brought with it new amenities and technologies that, in some cases, are being abused – one such is example is video games. As Vietnam deals with disturbing violent video game related episodes, parents and community members are searching for answers. This study project focuses on understanding the mental health care system in Vietnam and its current growth and advances. Three weeks were spent in Hanoi, …

Effective workplace violence (WPV) prevention programs are essential, yet challenging to implement in healthcare. The aim of this study was to identify major barriers to implementation of effective violence prevention programs. After reviewing the related literature, the authors describe their research methods and analysis and report the following seven themes as major barriers to effective implementation of workplace violence programs: a lack of action despite reporting; varying perceptions of violence; bullying; profit-driven management models; lack of management accountability; a focus on customer service; and weak social service and law enforcement approaches to mentally ill patients. The authors discuss their findings …

Correlating Coworker Relations, Employee Involvement, And Leadership With Associate Commitment In Select Healthcare Companies Using A Mixed Sequential Explanatory Design, Kimberly Ann Lee
Theses & Dissertations
Increasing patient demands and decreasing reimbursement require better efficiency and effectiveness in health care systems and, subsequently, in health care teams. These environmental and societal factors are further complicated by the complex initiatives set forth by the Affordable Care Act (2010). In this study, the researcher sought to examine and identify the strongest variables of health care teaming and to explore resulting themes through the perception of groups of health care associates. This mixed sequential explanatory design first examined the relationships between coworker relations, employee involvement, and leadership with associate commitment and with each other. An existing database of associate …

Understanding, Selecting, And Integrating A Theoretical Framework In Dissertation Research: Creating The Blueprint For Your “House”, Cynthia Grant, Azadeh Osanloo
Administrative Issues Journal
The theoretical framework is one of the most important aspects in the research process, yet is often misunderstood by doctoral candidates as they prepare their dissertation research study. The importance of theory-driven thinking and acting is emphasized in relation to the selection of a topic, the development of research questions, the conceptualization of the literature review, the design approach, and the analysis plan for the dissertation study. Using a metaphor of the “blueprint” of a house, this article explains the application of a theoretical framework in a dissertation. Steps for how to select and integrate a theoretical framework to structure …

That, That, But Not That… Using A Cafeteria Plan To Enhance Writing Skills, Tina T. Fields, Jeffrey J. Hatala
Administrative Issues Journal
College students have difficulty in written communication, despite attempts by universities to place English courses in the “core curriculum.” Although many companies indicate that writing is an expected competency, and many companies consider writing when they promote, students still enter the workforce with poor grammar skills. Clear and concise communication is especially important in the health professions, where life-and-death decisions may be made based on written communication. Instructors in a large southwestern university used the concept of “selection” to provide more opportunities for students to practice their writing skills. Students could self-select up to 19 written assignments throughout the semester, …

Principals’ Leadership Practices And Mathematics Pass Rate In Jamaican High Schools, David Palmer, Douglas Hermond, Carl Gardiner
Principals’ Leadership Practices And Mathematics Pass Rate In Jamaican High Schools, David Palmer, Douglas Hermond, Carl Gardiner
Administrative Issues Journal
This research was intended to explore the degree to which leadership practices impacted Jamaican schools’ mathematics achievement. More specifically, the researchers examined Jamaica’s high school students’ CSEC mathematics performance in relation to principals’ instructional leadership behaviors as measured by teachers’ perceptions, using Kouzes and Posner (2003) Leadership Practices Inventory (LPI). Data were gathered from 2-4 teachers from 42 high schools in Jamaica. The results indicated that those principals who had high LPI scores also lead schools with higher mathematics pass rates than those principals who had low LPI scores. More specifically, “enabling others to act” was established as the leadership …
