Current versions of the DSM and ICD have facilitated reliable clinical diagnosis and research. However, problems have increasingly been documented over the past several years, both in clinical and research arenas (e.g., 1, 2). Diagnostic categories based on clinical consensus fail to align with findings emerging from clinical neuroscience and genetics. The boundaries of these categories have not been predictive of treatment response. And, perhaps most important, these categories, based upon presenting signs and symptoms, may not capture fundamental underlying mechanisms of dysfunction. One consequence has been to slow the development of new treatments targeted to underlying pathophysiological mechanisms.

Objective: To evaluate the statistical accuracy of estimates of current HIV incidence rates from cross-sectional surveys, and to identify characteristics of assays that improve accuracy.

Methods: Performed mathematical and statistical analysis of the cross-sectional estimator of HIV incidence to evaluate bias and variance. Developed probability models to evaluate impact of long tails of the window period distribution on accuracy.

Results: The standard cross-sectional estimate of HIV incidence rate is estimating a time-lagged incidence where the lag time, called the shadow, depends on the mean and the coefficient of variation of window periods. Equations show how the shadow increases with the …

Genicular collateral arterial circulation is required for perfusion of the lower leg should the superficial femoral artery (SFA) become obstructed by trauma or disease. When describing the anatomy, textbooks only provide a schematic representation of the genicular collateral circulation and suggest that the descending branch of the lateral circumflex artery (DBLCFA) plays a critical role. Purpose: To evaluate the applicability of an evidence-based video intervention to promote informed decision making for prostate cancer (CaP) screening among African-American men with different levels of health literacy.

Methods: Forty nine African-American men participated in interviewer-administered, pretest-posttest interviews between January and March 2008. Health literacy status was assessed with the Test of Functional Health Literacy in Adults (TOFHLA). Repeated measures analysis of covariance (ANCOVA), McNemar or binomial distributions were computed to assess pretest/posttest differences in knowledge. Descriptive statistics were produced to describe participants’ perceptions of the information presented in the video.

The analysis of recurrent failure time data from longitudinal studies can be complicated by the presence of dependent censoring. There has been a substantive literature that has developed based on an artificial censoring device. We explore in this article the connection between this class of methods with truncated data structures. In addition, a new procedure is developed for estimation and inference in a joint model for recurrent events and dependent censoring. Estimation proceeds using a mixed U-statistic based estimating function approach. New resampling-based methods for variance estimation and model checking are also described. The influence of cancer on elderly is examined in a Home and Community Based Waiver program during 2002-2007. A comparison of with cancer (865) to without cancer (8617) on falls, fractures and healthcare use occurred. Data were obtained from Minimum Data Set assessments, vital records, and claims. Mean age was 77.1 years, 67.8% female, 74.0% Caucasian. Cancers was 92.7% > stage 2. Falls were 35.9% (304) with cancer and 25.3% (2,174) with non-cancer, a significant difference (p-value 0.01); hip fractures were 4.6% (38) and 5.3% (449) p-value 0.38; service use: ER 35.4% (295) to 22.2% (1904) and hospitalization 13.1% (109) …

Cancer survivors are diagnosed at an older age, with treatment often complicated by comorbidities which may exacerbate functional decline. The purposes of this study were to examine1) the effect of cancer diagnosis on frailty (activities of daily living, cognition, depression, incontinence, weight loss, vision, and hearing) and 2) differences in falls by site, stage, or cancer treatment. The hypothesis is that elderly cancer survivors experience the influence of the disease or treatment of cancer, and have increased frailty and falls. The objectives of this study were to (a) pilot test instruments measuring fatigue and quality of life (QOL); (b) pilot test an exercise intervention; and (c) estimate the effect size of this intervention relative to completion of combination therapy, fatigue, QOL, and walking distance in 20 patients with chronic hepatitis C about to begin interferon alpha and ribavirin treatment. Alpha reliabilities for both the Schwartz Cancer Fatigue Scale and Hepatitis Quality of Life Questionnaire were moderately high. Veterinary medicine has undergone dramatic, rapid feminization while in many ways remaining gendered masculine. With women constituting approximately half of its practitioners and nearly 80 percent of students, veterinary medicine is the most feminized of the comparable health professions. Nevertheless, the culture of veterinary medicine glorifies stereotypically masculine actions and attitudes. This article examines how women veterinarians understand the gender dynamics within the profession. Our analysis reveals that the discursive strategies available to women sustain and justify the status quo, and thus preserve hegemonic masculinity. This potent modifiable host factor has been disregarded in the cancer treatment plan for decades, despite its role in cancer control. The purpose of this study was to explore perception of physical activity among women with breast cancer. We investigated how induced blur affects performance on the Trail Making Test and Digit Symbol Test routinely used in occupational therapy cognitive evaluations. The study used a factorial design with both age (young and old adults) and simulated blur levels of near visual acuity (20/50 and 20/100) manipulated between participants. A sample of 124 healthy, community-living adults was used in the final analysis. Significant differences (p < .05) were found in performance for young participants between 20/50 and 20/100 blur level as well as between 20/20 and 20/100 blur level for the Digit Symbol Test. Abstract: Prevalence of tender points (TP), and widespread pain and fibromyalgia, as well as the relationship between TP and widespread pain and mobility, was examined in 585 community-dwelling older adults (mean age 78.2 years, 63.4% female). Pain was based on location (none, single site, multisite, widespread). Mobility was measured by the Short Physical Performance Battery (SPPB), gait speed, and self-reported (S-R) mobility difficulty. Tender-point count and health characteristics (ie, BMI, chronic conditions, analgesic use, number of medications, depression, and blocks walked per week) were assessed.
