Managed care plans may hesitate to participate in programs for uninsured persons because they fear adverse selection, whereby only the sickest people or highest users would choose to join the program. We studied this issue in Washington State's Basic Health Plan, a demonstration program that provides subsidized health insurance for families earning less than 200% of the poverty level. We interviewed people in three counties who enrolled in the program, and compared them to people in the same counties who were eligible but did not enroll. There were substantial differences between enrollees and eligibles in education, age, income, employment, race, …

A group of complement-resistant, virulent avian Eschberichia coli isolates were compared with a group of complement-sensitive, avirulent avian isolates for the presence of K-1 capsule, smooth lipopolysaccharides (LPS), the traT gene, and Colicin V (ColV) production. These parameters were selected because of their reported association with complement resistance and virulence in E. coli. Lethality in chicken embryos has also been shown to be correlated with virulence of avian E. colifor chickens. The complement-resistant, virulent E. coli isolates did not possess a K-1 capsule. Production of ColV and the presence of smooth LPS were significantly correlated with embryo lethality. There was …

Breed predispositions for some types of congenital heart disease were recognized 30 years ago and led to breeding experiments that confirmed the genetic transmission of specific congenital heart defects in most instances. As a result, certain breeds were associated with certain diseases, and this became a legitimate consideration in the differential diagnosis of some abnormalities in the absence of more definite diagnostic studies. As an example, a significant systolic murmur in a young Newfoundland dog means sub-aortic stenosis (SAS) until proven otherwise. The diagnosis of SAS is usually confirmed by Doppler echocardiography or angiocardiography.

Spirituality Among Terminally Ill Hospitalized Adults And Healthy Non-Hospitalized Adults In A Military Environment, Richard M. Holt

The purpose of this study was to identify whether or not terminally ill hospitalized adults indicated greater spirituality and well-being than healthy non-hospitalized adults. Reed's (1986b & 1987) studies are the bases for this treaty although this is not a replicate study. The conceptual framework of this research is founded on the developmental life-span, and energy field patterns (Rogers, 1980) impacting spirituality and well-being. It was hypothesized that terminally ill hospitalized adults indicate greater spirituality than healthy nonhospitalized adults in a military environment. A terminally ill and healthy adult group of N=30, (n=15 each) were selected for a convenience sample. …

The Occurrence Of Alexithymia In Children And Adolescents With Posttraumatic Stress Disorder, Michael S. Routh

Previous studies of the relationship between alexithymia and posttraumatic stress disorder (PTSD) have utilized samples of combat veterans with PTSD. However, children and adolescents who have experienced trauma and are diagnosed with PTSD exhibit similar or identical symptomology. The current study examined alexithymia in a sample of children and adolescents diagnosed with PTSD. It was hypothesized that the sample, when compared to non-PTSD and control samples, would reveal a significantly greater severity of alexithymia and PTSD. Although overall results were not consistent with previous research, trends of group means supported a relationship between alexithymia and PTSD. 1

Diagnostic reasoning is an essential feature of the occupational therapy treatment process. Acquisition of diagnostic reasoning skills is difficult for students, however, because of paradoxical developmental limitations faced during transition from classroom to clinical learning environments. Educators in pre-professional occupational therapy curriculums are searching for educational methods to prepare students for diagnostic reasoning tasks inherent in practice.
This experimental study compared a computer-simulated diagnostic reasoning activity with a more traditional case study reviewing activity in terms of effect on accuracy of problem identification among occupational therapy students. Grammatical Morpheme Acquisition In 4-Year-Olds With Normal, Impaired, And Late-Developing Language, Rhea Paul, Sally Alforde

The production of the grammatical morphemes studied by Brown and his colleagues was examined in free speech samples from a cohort of 4-year-olds with a history of slow expressive language development (SELD) and a control group of normal speakers. Results suggest that children with SELD acquire morphemes in an order very similar to that shown in previous acquisition research. Children who were slow to begin talking at age 2 and who continued to evidence delayed expressive language development by age 4 showed mastery of the four earliest acquired grammatical morphemes, as would be expected, based on their MLUs, which fell …
