One hundred and fifty nine patients who experienced the first episode of stroke between 15 to 45 year-old is analyzed for possible risk factors. These patients whose diagnosis of stroke were all approved by the computerized tomography, were collected from the National Taiwan University Hospital (NTUH) from 1983 to 1990. They included 79 cases of cerebral hemorrhage (48 males and 31 females) and 80 cases of cerebral infarction (47 males and 33 females). The frequency of male stroke was about 30% higher than female. Atherothrombosis and rheumatic heart disease still dominate in cerebral infarction. The main etiology of cerebral hemorrhage …

The Purpose of this study was to compare the stroke patients within day-care center to patients within family-care about the function of activity of daily living. They were selected as twenty groups. Each group had the same hemiplegic side, the same motor stage, and the same score of Barthel Index. They also got the same observation duration. No significant difference at admission were found between the two groups, except that the day-care patient had higher scores in dressing lower body (P=0.04). At discharge, the day-care patient had higher functional score in feeding than the family-care patient (P=0.0036). Within-group functional change …

From April 1985, to December 1987, 1445 patients were admitted to Chang Gung Memorial Hospital, Keelung under the diagnosis of cerebral vascular accident ( hemorrhagic or infarction type). By random selection, 343 patients were analyzed by the SPSS. Of these 343 patients, 185 had died by the time this study was carried out. Thus the data available for analysis was incomplete for this deceased group. Only the data of those who survivied was analyzed in the results. The average duration of follow-up was 50.2±8.9 months (36-67 months). Among the survivors (n=158), 57% (n=90) received regular medication treatment, yet only 63% …

In order to understand the caring environment of the stroke patients and the utilization, need, demand and congition of the rehabilitation resoures, 225 stroke patients, who admitted to Kaohsiung Chang Gung Memorial Hospital from January 1986 to December 1990, 117 males (52%) and 108 females (48.0%) with the ages from 31 to 89 (mean 63.45 ±10.87) were evaluated by questionaires, telephone and home visit. From the view point of the patients, the rehabilitation resources were insufficient for them in communities (76.4%), and it was one of the main reasons causing loss of follow-up. Severe muscular spasticity in cerebral palsy not only causes functional impairment and difficulty in daily care, but also increases the incidence of musculoskeletal complication which includes joint contracture and deformity. Clinically, many ways are used to reduce muscle spasticity, such as therapeutic exercise, antispastic medication, orthosis, phenol intramuscular neuroloysis and orthopedic surgery, etc. Basically, their effects are transient and can last for variable duration. To investigate the nature of exposure factors, and effectiveness of countermeasures in head injuries in Taiwanese children with head injuries consecutively admitted to Chang Gung Memorial Hospital from Jan. 1985 to Dec. 1989. We analyzed the data by chart review. The analysis confirmed a boy to girl ratio of about 2.3 to 1. The largest number of head injuries was in children under 1 year, 4 to 5 years and more than 15 years of age. Intoxication of lithium carbonate may present as a multiple systematic dysfunction, but central and peripheral nervous involvement is rare and more troublesome. In the past, there was only one case of lithium intoxication with polyneuroparhy reported by Chang et al in Taiwan. We report here an additional 49 year-old man with manic-depressive psychosis. He received lithium carbonate medication treatment for 5 years and developed a severe cerebellar dysfunction and a mild sensori-motor polyneuropathy. Blood lithium level was 1.80meq/l two days after quitting lithium. This is a case of a 64-year-old male with difficulty in ambulation for 40 years. Physical examination revealed a defect in vibration and positional sense over the lower extremities. Muscle power of the upper extremities was normal. The lower extremities could still resist force, but there was a little atrophic change in the quadriceps muscle and weakness over the iliopsoas muscle.Laboratory data were all negative, including CSF, routine blood, urine and stool specimens. This 67 years old, male, DM patient suffered from a falling accident, resulting in traumatic cervical disc herniation with epidural abscess. After discectomy, the patient sustained quadriplegic condition and needed mechanical ventilation due to breathing difficulty. In addition to the usual rehabilitation program, we also used transcutaneous electrical stimulation on this patient's various muscle groups in order to stimulate his muscles and to help his weaning from ventilator. Diurnally fluctuating hereditary progressive dystoria is a very rare disorder. In 1975, Segawa regarded this disorder as an entity, and there are 46 cases reported till 1986 since then. The clinical manifestation of this disease are: normal development, early onset, dyotonia, diurnal change, excellent response to L-dopa. In this report, a 21-year-old female, who had had symptoms onset since 8 years old; became wheel chair bound at 15 years of age; was not diagnosed correctly until 20 years old. This study examined the effects of training and experience on counselors' ability to detect relapse precipitants in substance abuse clients. Eighty counselor trainees from Western Michigan University and the University of Northern Colorado participated. They were classified as to their previous level of experience and then exposed to either the treatment condition or the control condition. Pretest-posttest data were gathered using a modified version of the Alcohol Confidence Questionnaire (Annis & Graham, 1988) (ACQ-M).

The present study was designed to examine possible neuropsychological deficits in migraine and tension headache subjects. Past research has been inconclusive, with some studies indicating that chronic migraine headache sufferers do exhibit some neuropsychological deficits such as short-term memory difficulties, gross motor slowing, and verbal memory deficits, while other studies have indicated that no deficits are seen in this group Also examined were headache precipitant and headache-related behaviors that these groups partake in when experiencing headache pain. Past research has suggested that migraine headache sufferers tend to deal with their headache pain differently than tension headache sufferers.

The osteoclast, a multinucleated giant cell, has as its role the resorption of bone matrix with consequent release of calcium. The osteoclast is thought to derive from a pluripotent stem cell which also gives rise to monocytes and macrophages. The development, activation, and inhibition of osteoclasts are influenced by a number of systemic and local factors. The release of these factors is influenced by changes in the dietary status of the animal. Calcium deficiency is known to stimulate the recruitment and activity of large numbers of osteoclasts. พิษเฉียบพลันของเมททิลพาราไธออนในกุ้งกุลาดำ, สถาพร สุวรรณรักษ์, สุพัฒตรา ศรีไชยรัตน์, จิรศักดิ์ ตั้งตรงไพโรจน์

ศึกษาความเป็นพิษของเมททิลพาราไธออนที่ขนาดความเข้มข้น 1-90 ไมโครกรัม ต่อลิตร (ppb) ในกุ้งกุลาดำขนาด 10.9 ± 0.5 เซนติเมตร 7 กลุ่ม โดยวิธีแบบแช่ พบว่าค่า LC50 ภายใน 96 ชั่วโมง เท่ากับ 54 ppb อาการแสดงของความเป็นพิษและอัตราการตายขึ้นอยู่ กับความเข้มข้นของสารเคมีกำจัดศัตรูพืชที่ให้ สมรรถนะของเอนไซม์โฆลีนเอสเทอเรสในเลือดกุ้งที่ได้รับสารเคมีกำจัดศัตรูพืชไม่สอดคล้องกับความเป็นพิษที่ได้รับ ในขณะที่สมรรถนะของเอนไซม์ โฆลีนเอสเทอเรสในเส้นประสาทและกล้ามเนื้อลดลงอย่างมีนัยสำคัญทางสถิติ (p<0.05) เปอร์เซ็นต์ ของสมรรถนะของเอนไซม์ที่ลดลงขึ้นอยู่กับความเข้มข้นของเมททิลพาราไธออนที่ได้รับ ผลทางจุลพยาธิวิทยาแสดงว่ามีการตายของเซลล์ตับและตับอ่อนและเซลล์กล้ามเนื้อ ความรุนแรงขึ้นกับความเข้มข้นของเมททิลพาราไธออนที่ได้รับ (1-50 ppb) และระยะเวลาที่สัมผัส ส่วนกลุ่มทดลอง 75-90 ppb ไม่พบริการดังกล่าว ผลการทดลองชี้ให้เห็นว่าการวัดสมรรถนะของเอนไซม์โฆลีนเอสเทอเรสในกล้ามเนื้อและในเส้นประสาทของกุ้งกุลาดำสามารถใช้เป็นตัวบ่งชี้ถึงความเป็นพิษของสารเคมีกำจัดศัตรูพืชกลุ่มออกาโนฟอสเฟตได้

รายงานการเกิดลูกกระบือแฝดจำนวน 3 คู่จากแม่กระบือที่ได้รับการปรับขนานการ เป็นสัดด้วยฮอร์โมนนอร์เกสโตเมท ร่วมกับการฉีดฮอร์โมน PMSG จำนวน 69 ตัว อุ้มท้องจนครบกำหนดคลอด 24 ตัว คิดเป็นอัตราการคลอดลูกแฝด 12.5 % ลูกแฝด 3 คู่ที่เกิดนี้ เป็นแฝดต่างเพศ 2 คู่ แฝดคู่เมีย 1 คู่ ลูกแฝดนี้มีน้ำหนัก แรกคลอดต่ำกว่าลูกกระบือที่คลอดตัวเดียว 5-10 กก. ลูกกระบือ 3 ตัวตายเมื่ออายุ 1,3 และ 36 วัน ลูกกระบือทั้ง 3 ตัวที่เหลือ เพศเมีย 1 ตัวที่เลี้ยงรอดชีวิตจนถึงปัจจุบันเกิดจากแฝดต่างเพศเป็น freemartin

Peripheral neuropathy is an important complication of diabetes mellitus. However, neuropathological changes in diabetic neuropathy with axonal degeneration or segmental demyelination are still unclear. By using a newly developing electrodiagnostic method of single fiber electromyography (SFEMG) with a computer-aided program, mean jitter value and muscle fiber density were measured in extensor digitorum communis (EDC) and anterior tibial (AT) muscles from 36 diabetic patients with neuropathy (group 1) and 12 patients without neuropathy (group Ⅱ) proved by nerve conduction study. The survey of dancing injury in the special dancing class of a primary school was done on 1990. From 108 effective questionare, the prevalence of dancing injury was 77.78% in which 11.54% became chronic.In the 59 girls of grade 5, 6 students, the most frequent injury area was ankle 34.48%, next was the knee 20.2% and inner thigh muscle 11.72%. The causes of the injury were 1) unenough warm up (47.61%) 2) poor dancing floor (29.76%) 3) over training (13.09%). For the better understanding of problems of patients with spinal dysraphism, 51 cases were studied retrospectively by questionnaire, telephone and clinical followup. They had been admitted to Taipei Area Chang Gung Memorial Hospital from Jan 1983 to Dec. 1989Among them 24 patients were male and 27 were female. Their age distribution were from 2 to 39 years old. Sarmiento introduced the functional fracture brace for the management of tibial shaft fracture in 1963. However, tibial angulation with varus deformity may still occur, especially in the presence of an intact fibula. We report here a case of tibial shaft fracture with varus angulation treated with a modified below knee fracture brace. Our modification included a wooden rod embedded in the under-side of the anterior shield with nod positioned laterally between tibia and fibula. Trimming of rod was done for fitting comfortably against the varus angulation. Good bridging callus formation with sound alignment was noted at the twentieth week. The …

A 53 year-old male patient, who sustained infarction of the right medial frontal lobe with extension into the corpus callosum, demonstrated inattention, slow initiation and left hemiparesis initially. As the strength recovered, forced grasping of the left hand, and intermanual conflict appeared. Besides, left ideomotor apraxia, left constructional apraxia, left apraxic agraphia, and abnormalities of cross-replication of hand posture were detected after specific clinical testing. Most of these deficits improved eight weeks after stroke. St. Mary's is run by a religious order attempting to provide charitable care in a nondemanding environment. Many demands are made, however. The lack of privacy and respect for individuality inherent in institutional life tends to erode the "inmate's" very conception of self.
