Pediatric Emergency Care: A Survey Of Michigan Emergency Departments, David J. Treloar, Gregory Preston, Fred Lamb

Recent studies suggest a lack of preparation in emergency medical systems for pediatric patients. Michigan emergency departments were surveyed to determine their degree of preparedness regarding equipment, space, and pediatric on-call coverage. More than 50% of responding emergency departments did not have a pediatrician on call and lacked a pediatric "crash cart," and 34% had to send their personnel to other areas in the hospital to obtain pediatric equipment during resuscitations. This survey suggests a lack of preparation for pediatric emergencies in Michigan.

Food allergy occurs in as many as 5% of infants less than 1 year old. Most food sensitivities disappear by age 3 to 4. Most immunologically mediated food allergy reactions are due to type I IgE mechanisms, with skin, gastrointestinal, and respiratory signs and symptoms the most common manifestations. U sing the double-blind, placebo-controlled food challenge (DBPCFC) technique and atopic dermatitis as a model, the most common foods to which children are allergic are cow's milk, eggs, peanuts, wheat, fish, and soy proteins. Anaphylactic sensitivity in older children often involves crustacean seafoods (eg, shrimp), nuts, peanuts, fish, and eggs. Such …

Among the many challenges of providing care for children with multiple physical and developmental handicaps is the proper management of their respiratory difficulties. This study was designed to evaluate the prevalence of respiratory symptoms in children attending a multidisciplinary clinic for handicapped children. We also used several methods to estimate the prevalence of allergic sensitivity in children with respiratory complaints. Questionnaires concerning respiratory symptoms were completed by the parents of 104 patients. The most frequent complaint reported was snoring in 41% of the children. More than 1 million teenagers become pregnant every year in the United States. Nearly one-half of the pregnancies are carried to term, while a similar number end in abortion. In Michigan this amounts to approximately 34,000 teenage pregnancies (ages 15 to 19). These adolescent patients have a high incidence of sexually transmitted diseases (STDs). Of the pregnant teens at Henry Ford Hospital. 60% present with one or more STDs. We discuss the case of a 13-year-old pregnant girl who presented at five weeks estimated gestation with gonorrhea, chlamydia, and appendiceal perforation, resulting in subsequent spontaneous abortion. Teenagers in the United States experience significant mortality and morbidity because of high-risk behaviors. Adolescents need routine health supervision aimed at reducing these risks. A Teen Clinic within the Pediatrics Clinic at the Henry Ford Hospital Fairlane Center offers separate facilities, longer appointments, and an emphasis on adolescent issues. This plan is a model for practitioners in various office settings who seek to improve adolescent care.

The functional outcome of 34 cases of digital ampution with toe-to-digit reconstruction in Chang-Gung Memorial hospital, from Jan 1984 to Jan 1987, were evaluated by questionaires.The age of the patients were from 16 to 48 years old, with male in majority (82.3%). Nearly all of them were occupational injury. There were 20 cases with right hand injury, most of them should change hand dominance even after reconstruction surgery (63.2%). The prosthetic workshop of National Taiwan University Hospital was established in 1970. There were 665 amputees recieved prosthetic fitting at the workshop from the beginning of 1971 to the end of 1985. Among these amputees, 307 patients were the victims of black-foot-disease, and has ben reported previously. Therefore, the present study included the rest of 358 amputees, 278 males and 80 females with male/female ratio 3.5: 1. The site of extremity involved were 160 in right and 198 in left, which revealed no significant difference statistically (p>0.05). Hearniation of lumbar disc (HIVD) causing compression of the cauda equina is rare but needs urgent diagnosis and surgical treatment. Cauda equina syndrome (CES) has been described as a complex of low back pain, bilateral or unilateral sciatica, saddle anesthesia and motor weakness in the lower extremities that may progress to paraplegia with bowel and urinary bladder incontinence. The onset of bladder and rectal paralysis with saddle anesthesia should be viewed with a high index of suspicion in a patient with backache and sciatica. We examined the use of oesphageal-pharyngeal (0/P) samples for the isolation of Foot and Mouth Disease (FMD) virus in animals from which it was difficult to obtain a conventional epithelial tissue specimen. The virus was detected in 42 (28%) of 150 samples, and only 5 (12%) of the 42 cases had positive results when the samples were tested directly with ELISA. The preferred technique for examination of 0/P samples was thus by culturing representative cell samples. The 150 samples came from 22 cases of the disease, with the virus being detected in 16 cases. ปรากฎการณ์การดื้อยาของเชื้อ Escherichia Coli ที่แยกจากไก่เนื้อ, พิณทิพย์ น้อมไสว, มาลิน จุลศิริ, อรษา สุตเธียรกุล

การศึกษากระทำในเชื้อ Escherichia coli ที่แยกจากไก่ฟาร์ม 5 แห่ง ซึ่งตั้งอยู่ในเขตกรุงเทพฯ และจังหวัดใกล้เคียง ภายหลังวิเคราะห์ดูการดื้อยาของเชื้อเหล่านี้ ต่อยา ampicillin, chloramphenicol, colistin, cotrimoxazole, neomycin และ tetracycline พบว่ามีเชื้อ E. coli ที่แยกจากลูกไก่และไก่กระทงดื้อยาสูงถึง 91.2% (343/376 isolates) และ 97.3% (322/331 isolates) ตามลำดับ ซึ่งส่วนใหญ่จะดื้อยา tetracycline เพื่อเปรียบเทียบผลการศึกษา จึงได้ทำการทดสอบดูปรากฏการณ์ การดื้อของเชื้อ E. coli ที่แยกจากคนทั้งเด็กและผู้ใหญ่ ผลที่ได้เปิดเผยว่า 43.2% (19/44 isolates) และ 82.1% (55/67 isolates) ของเชื้อดังกล่าวที่แยกจากเด็กและผู้ใหญ่ ตามลำดับดื้อยา และการดื้อต่อ tetracycline ก็พบมากที่สุดเช่นกัน เมื่อนำเชื้อดื้อยาบางตัวอย่างที่สุ่มมาเพื่อตรวจสอบทางน้ำเหลืองวิทยาหาเชื้อที่ อาจก่อให้เกิดโรคทางเดินอาหารของคน ผลปรากฏว่าเชื้อ 200 ตัวอย่างที่เลือกมา ได้พบ 2 ตัวอย่าง (1%) ที่มี serotype 020a 20b:K84 ของกลุ่ม enteropathogenic E. coli (EPEC)

This study was designed to determine whether students with hearing impairments could be taught to write more mature personal narrative stories. An experimental group of junior high students received intervention focused on teaching the elements of story grammar and their application to the students' written narrative stories. A matched control group of students spent equal time with the researcher and received intervention focused on improvement of basic language skills. Pretest, posttest-1, posttest-2, and generalization samples were taken. Students' metatextual skills for telling how to write a story were also probed.

Research has demonstrated several factors among sexually abused males such as males sexually abused as children becoming adult sexual abusers; one in four males reporting sexual abuse; and adult emotional trauma, drug abuse, and sexual dysfunction. To break this abuse pattern, we need to focus on increasing males' reports of sexual abuse and initiating treatment.
The present study evaluated the effectiveness of an oral educational presentation in increasing males' reports of sexual abuse. It also sought prevalence information and treatment needs. Theoretically, perceptual information from the interaction with an object and its surfaces (an affordance) facilitates the development of motor skill. This study investigated the use of an affordance in learning control of flexion and extension of an above-elbow training prosthesis. Fifty-two male college students were randomly assigned to two training procedures: two, 1.5-minute periods of a joining dots activity with a flashlight attached to the hook of the prosthesis (affordance group), and practice moving the prosthesis at the elbow joint (no-affordance group) for the same amount of time. It is a common belief that a low self concept is related to deviant behavior. The purpose of this study was to compare the self concepts of juvenile recidivists with those of non-recidivists.
A sample of 162 subjects was taken from juveniles who had been treated at a Michigan detention facility. Measures of self concept, using the Tennessee Self Concept Scale (Fitts, 1965), were taken both prior to and following treatment. Point-biserial correlations were computed for pre-test scores, post-test scores, and changes in scores from pre-test to post-test.

Previous research has shown that ethosuximide in high enough doses will disrupt operant responding in pigeons. Whether or not these same doses will protect against seizure activity in this species has not been previously tested.
To test this question a system for scoring pentylenetetrazol-induced seizures in pigeons was developed and the effects of ethosuximide on such seizures were evaluated. In Experiment 1 pentylenetetrazol at 15, 27 and 47 mg/kg reliably induced seizures. In Experiment 2 non-behaviorally active and behaviorally active doses of ethosuximide were tested for their seizure controlling effectiveness.

Several risk factors have been linked to cardiovascular diseases; among them are exposure to psychological stressors and caffeine consumption.
The objective of this study was to examine the effects of caffeine consumption on cardiovascular reactivity to role play simulations of social stressors. A double-blind alternating treatment design was employed. Eight subjects (five female and three male) received either caffeine-containing or placebo beverages. Each subject received four samples of each beverage over a total of eight sessions. The purpose of this study was to examine the amount of lower back muscle stress produced during lifting tasks, using variations in weight and postures. Electromyographic activity of the erector spinae muscles at the L4-5 vertebral level was measured.
Thirty healthy female subjects participated in the study. Each subject lifted loads of two given weights using both the stooped and squat lifting postures. Electromyographic activity was recorded and averaged throughout each lift.
Results indicated that increased weight resulted in increased stress on the erector spinae muscles, regardless of posture. The squat lifting posture was found to be more stressful on …

Calcium/diacylglycerol-dependent protein kinase, most often referred to as protein kinase C, is of interest to research groups across many scientific disciplines for two major reasons. First, protein kinase C is of general importance in signal transduction because its second messenger activator, diacylglycerol, is generated by phosphoinositide metabolism in response to a host of biologically active molecules. Second, protein kinase C has been identified as the cellular receptor for tumor-promoting phorbol esters.
