According to the Annals of Family Medicine, the amount of babies born via cesarean section has increased from 4.5% in 1965 to 26.1% in 2002 and nearly 40% of all cesarean sections are repeats. After an extensive literature review, results showed that patients need to be educated about the risks and benefits of vaginal delivery and cesarean delivery. It was also found that there needs to be policy changes to decrease the amount of cesarean sections done and increase the labor and delivery support without using interventions. Further study should be focused on morbidity and mortality very low birth weight …

Abstract

Objectives: To assess risk factors for cerebrospinal fluid (CSF) leak after microvascular reconstruction of extensive cranio-orbitofacial resection with orbital exenteration (CFOE).

Study Design: Retrospective Case Series

Methods: 70 consecutive patients at a tertiary hospital underwent 76 procedures with microvascular reconstruction of CFOE defects. Patients were stratified by extent of skull base exposure and presence or absence of dural resection. Patients with exposure of the orbital apex and roof alone were classified as minimal skullbase exposure (MSE, n=32). Those with exposure beyond the orbital apex and roof were classified as significant skullbase exposure (SSE, n=38), including those with dural resection …

Introduction:

The Sidney Kimmel Medical College (SKMC) at Thomas Jefferson University (TJU) is creating a new medical curriculum (JeffMD), in which more learning will take place in small groups and individual activities.

In their small groups, students will focus on core case studies that will become increasingly complex, each building on the prior phase of the curriculum.

Abstract

Metabolic dysregulation within the tumor microenvironment (TME) is critical to the process of tumorigenesis in various cancer types. Thyrocyte metabolism in papillary and anaplastic thyroid cancer, however, remains poorly characterized, and studies analyzing the role of multicompartment metabolism in thyrocyte oncogenesis are sparse. We present a review of the current knowledge on cellular metabolism in non-cancerous and cancerous thyroid tissues, focusing on the monocarboxylate transporters MCT1 and MCT4, and on a transporter of the outer mitochondrial membrane TOMM20. Understanding the metabolic phenotype of tumor cells and associated stromal cells in thyroid cancer can have profound implications on the use …

  • Innovations Decrease Toxic Effects of Radiation Therapy for Mediastinal Tumors from Hematological Malignancies: Anterior mediastinal masses caused by lymphoma or leukemia can be difficult to treat because sensitive tissues in the area are vulnerable to damage from radiation therapy. To treat these tumors while sparing healthy tissue, radiation oncologists at The University of Texas MD Anderson Cancer Center use a combination of advanced technologies and new techniques.
  • Transoral Robotic Surgery Enables Less Toxic Treatment of Early Throat Cancers: For decades, many patients with hard-to-reach throat cancers received high-dose radiation and chemotherapy, with their often severe toxic effects, because conventional surgery …

Background:

  • Methemoglobin (MetHb) is formed when the iron moiety of hemoglobin (Hgb) is oxidized from ferrous (Fe2+) to ferric (Fe3+) state à impaired O2 delivery to tissues
  • Acetaminophen Metabolism:

~90% is metabolized in liver via sulfation and glucuronidation

remainder is metabolized via CYP450 to a toxic oxidizing agent, N-acetyl-p-benzoquinone imine (NAPQI), which is detoxified via glutathione1

  • Acute Acetaminophen Overdose:

saturates sulfation and glucuronidation pathways

depletes glutathione stores

leads to excess NAPQI à oxidative stress à methemoglobinemia1

  • MetHb Reduction Pathway:

cytochrome b5 reductase removes 95% to 99% of endogenous MetHb

nicotinamide adenine dinucleotide phosphate …

Case Description:

  • A 36-year-old man presented with a large epidural collection, shown on MRI to extend from C1-S5 and resulting in spinal canal narrowing (Figures 1-3). The patient was admitted for acute inpatient rehabilitation after neurosurgical treatment and stabilization s/p C7-T1, T7-8, L4-5 laminectomies.
  • He presented with impaired mobility and activities of daily living, and reported moderate pain in the back and lower extremities, but without specific distribution, during PT/OT therapies.
  • Throughout his 7-day length of stay, the patient was closely monitored for methicillin-susceptible S. aureus bacteremia, and he received IV oxacillin and oral rifampin. Active medical problems …
