Hypermobile Ehlers-Danlos syndrome (hEDS) is an inherited connective tissue disorder, often under-diagnosed, and presenting with frequent chronic pain and severe musculoskeletal symptoms that can drastically reduce the quality of life during one’s life span. While there are limited quantitative approaches in the literature on adult movements, the biomechanics of movements during activities of daily living (ADLs) in children have not been investigated comprehensively. Therefore, the primary purpose of this dissertation was to characterize the biomechanics of the musculoskeletal system and investigate the biomechanics of hEDS by quantifying joint dynamics and muscle activations during ADLs and gait in the pediatric population. …

Manual wheelchair (MWC) users with spinal cord injury (SCI) rely heavily on their upperextremities to complete daily occupations. Due to repetitive shoulder use during wheelchair mobility and propulsion, MWC users are at greater risk of shoulder pain and shoulder pathology, and thus decreased independence, and lower quality of life. The relative fit of the wheelchair and its parameters are critical and can further impact the user’s propulsion biomechanics. Parameters such as seat angle and axle position may put the user in detrimental shoulder positions for longer periods of time, impacting health outcomes even more. Overhead sports such as baseball, tennis, volleyball, softball and swimming are known to cause rotator cuff imbalances and injuries. These injuries are typically caused by the repeated use of “throwing motion,” where the humerus is externally rotated and shoulder abducted and elevated. Previous research has shown that the function of the upper trapezius muscle in overhead athletes is likely to have improper activation (timing, magnitude) during various shoulder motions and/or activities of daily living. Musculoskeletal Injuries are the most common cause of severe long-term pain and physical disability. Push-Ups and Pull-Ups are effective dynamic exercises that mimic high level function activities, such as those used in the military. The model developed allows for researchers to analyze the forces and moments associated with the shoulder, elbow and wrist, to further assess function in military personnel, athletes and the active population. The model also follows the guidelines set forth by the International Society of Biomechanics (ISB).

From 1994-2006, 1.5 million baseball injuries were treated within emergency departments across the United States while the upper extremity was the second most commonly injured body part. The purpose of this collection of studies was to further develop the knowledge of shoulder motion in order to better understand joint function through direct measurement of 3D scapulohumeral joint kinematics using a technique of high accuracy. Markerless, bi-planar fluoroscopic radiostereometric analysis using a generic shoulder model was developed in this thesis, reducing the amount of radiation exposure to subjects. The studies compared kinematic data of the scapulohumeral joint in six degrees of freedom with a precise, in-vivo measuring technique. Data were collected on young and older healthy individuals, individuals with a torn supraspinatus and post-surgical intervention.

The use of electromyography (EMG) techniques offer strong evaluation of musculature fatigue during activity in different exercises. Studies show that EMG can be used to monitor fatigue patterns in muscles composing the upper extremities and thoracic areas of the human body by analyzing median frequency values. Advances in muscle fatigue research have been critical for improving rehabilitation programs for patients with musculoskeletal conditions. Water polo is a highly dynamic and physically demanding sport that has heretofore not inspired nearly as much research as other overhead sports such as baseball or swimming. As in almost all overhead sports, water polo puts its players at high risks for shoulder injuries since the motions required to perform the sport at an optimum level push the limits of normal shoulder function.

Specific injuries at the elbow and glenohumeral joints have been linked to several kinetic variables that occur throughout the throwing motion. Ulnar collateral ligament (UCL) sprains have been linked to excessive elbow valgus and shoulder external rotation torques occurring during the late-cocking phase of throwing. Shoulder external rotation torque during the late arm-cocking phase and shoulder distraction forces during the deceleration phase contribute to tears of the labrum. Additionally, it is believed that the peak distraction force generated during the arm deceleration phase also contributes to SLAP tears and rotator cuff pathologies. The rotation angle between the proximal and distal axes of long bones is known as torsion, and it is thought to be indicative of the forces applied to the bone during growth. The humerus, for example, develops an internal twist or antetorsion, as daily activities are anterior to the body. However, the strong posterior stress induced by an overhead throwing motion may counteract this internal twist in young athletes and cause prominent bilateral dimorphism.

Forward head and rounded shoulder posture (FHRSP) is theorized to contribute to alterations in scapular kinematics and muscle activity leading to the development of shoulder pain. However, reported differences in scapular kinematics and muscle activity in those with forward head and rounded shoulder posture are confounded by the presence of shoulder pain. Therefore, the purpose of this study was to compare scapular kinematics and muscle activity in individuals free from shoulder pain, with and without FHRSP.
