Automating global and segmental (regional) assessments of cardiac Left Ventricle (LV) function in Magnetic Resonance Images (MRI) has recently sparked an impressive research effort, which has resulted a number of techniques delivering promising performances. However, despite such an effort, the problem is still acknowledged to be challenging, with substantial room for improvements in regard to accuracy. Furthermore, most of the existing techniques are labour intensive, requiring delineations of the endo- and/or epi-cardial boundaries in all frames of a cardiac sequence.

This thesis aimed to determine the psychometric properties and applications of sensory threshold tools and outcome measures in patients with Carpal Tunnel Syndrome (CTS). The first chapter is a psychometric study that defines clinically important difference (CID), construct validity and responsiveness of touch and vibration threshold tools and in the Symptoms Severity Scale (SSS). The study found the CID for the PSSD and for the SSS was 0.15g/mm2 and 0.50 respectively. Use of inertial sensory systems to monitor and detect seizure episodes in patients suffering from epilepsy is investigated via numerical simulations and experiments. Numerical simulations employ a mathematical model that is able to predict human body dynamic responses during a typical epileptic seizure. An optimized inertial sensor placement procedure is developed to address achievement of highest possible sensing resolution in determining angular accelerations with minimal errors. The Circulating Fluidized Bed Biofilm Reactor (CFBBR), a bioparticle technology designed for biological nutrient removal, has been implemented to achieve considerable biodegradation efficiency and low sludge production, compared with activated sludge system and typical biofilm technology. The inherent advantages of bioparticle technology are enhanced substantially by the CFBBR, for example, decoupling of hydraulic retention time (HRT) from solids retention time (SRT), large specific surface area, ideal conditions for biofilm ecosystem.

The Tympanic Membrane (TM), also known as the eardrum, includes layers of organized collagen nanofibres which play an essential role in sound transmission. Perforations that are caused by infection or accident must be repaired in order to restore hearing. Tympanoplasty is performed using grafts that are prepared from bladder, cartilage, temporal fascia and cadaveric skin. However, since mechanical properties of these grafts do not match those of the original TM, normal hearing is not fully restored. Bone defects are a prevalent problem in orthopedics and dentistry. Calcium phosphate-based coatings and nanocomposites offer unique solutions towards producing scaffolds with suitable physical, mechanical and biological properties for bone regeneration.

We developed a novel method to synthesize hydroxyapatite (HA) particles with high aspect ratio using sol-gel chemistry and hydrothermal treatment. We obtained tunable pure-phase carbonated-HA in the form of micro/nanorods and nanowires (diameters 25-800 nm). To mimic the structure of bone, HA nanowires were homogenously mixed within poly(ε-caprolactone) (PCL) to produce nanocomposites with improved mechanical properties as determined by uniaxial tensile testing.

In microsurgery, the human hand imposes certain limitations in accurately positioning the tip of a device such as scalpel. Any errors in the motion of the hand make microsurgical procedures difficult and involuntary motions such as hand tremors can make some procedures significantly difficult to perform. This is particularly true in the case of vitreoretinal microsurgery. The most familiar source of involuntary motion is physiological tremor. Post-traumatic osteoarthritis commonly occurs as a result of a traumatic event to the articulation. Although the majority of this type of arthritis is preventable, the sequence and mechanism of the interaction between joint injury and the development of osteoarthritis (OA) is not well understood. Although teaching and learning surgical microscope manoeuvring is a fundamental step in middle ear surgical training, currently there is no objective method to teach or assess this skill. This thesis presents an experimental study designed to implement and test sets of metrics capable of numerically evaluating microscope manoeuvrability and qualitatively assessing surgical expertise of a subject during a middle ear surgery called myringotomy. Implant loosening following joint replacement surgery is a health-care concern. The role of implant-cement debonding on the propensity of loosening has received limited attention. This thesis examines changes in strains within the cement mantle and bone surrounding distal ulnar implants, as a function of cement-stem interface bonding.

A method to embed strain gauges within the cement mantle of the restrictive distal ulnar canal was developed. In this thesis, a computational framework for patient-specific preoperative planning of Robotics-Assisted Minimally Invasive Cardiac Surgery (RAMICS) is developed. It is expected that preoperative planning of RAMICS will improve the rate of success by considering robot kinematics, patient-specific thoracic anatomy, and procedure-specific intraoperative conditions. Spinal fusion is currently the gold standard for surgical intervention of intervertebral disc (IVD) diseases leading to neck and back pain failing conservative treatments. However, fusion removes motion between the vertebrae and can result in adjacent level degeneration. Total disc replacement (TDR) is an emerging treatment alternative that preserves motion, but materials found in clinically available devices bear little resemblance to the properties of the native IVD. A number of commercially available radial head (RH) implants are used for the management of RH fractures. The optimal shape of a RH implant to restore joint mechanics to the native state has not been established. This work compares radiocapitellar contact and kinematics for three implant designs as well as the native RH. These implants include an axisymmetric, a quasi-anatomic and a patient-specific design. When compared to the native RH, only the axisymmetric implant was significantly different in contact area (p=0.008). The reduced access conditions present in Minimally Invasive Surgery (MIS) affect the feel of interaction forces between the instruments and the tissue being treated. This loss of haptic information compromises the safety of the procedure and must be overcome through training. Haptics in MIS is the subject of extensive research, focused on establishing force feedback mechanisms and developing appropriate sensors. The introduction of transcatheter aortic valve implantation (TAVI), an innovative stent-based technique for delivery of a bioprosthetic valve, has resulted in a paradigm shift in treatment options for elderly patients with aortic stenosis. While there have been major advancements in valve design and access routes, TAVI still relies largely on single-plane fluoroscopy for intraoperative navigation and guidance, which provides only gross imaging of anatomical structures. Inadequate imaging leading to suboptimal valve positioning contributes to many of the early complications experienced by TAVI patients, including valve embolism, coronary ostia obstruction, paravalvular leak, heart block, and secondary nephrotoxicity from contrast use.

Spray-dried lactose was mixed with 4 different lubricants – magnesium stearate, magnesium silicate, stearic acid, and calcium stearate – in various concentrations. Flowability testing revealed that magnesium stearate improved powder flow until a threshold was reached, at which addition of lubricant had no effect. The addition of calcium stearate also improved flow; however, additional lubricant over the optimum amount further decreased flow. Full placebo granules were manufactured by both high shear and fluidized bed techniques. A granule comparison showed different growth mechanisms, surface morphology, particle size distribution, and flow characteristics for each manufacturing technique. A markerless RSA method was used to determine the effect of orthotics on the normal, pes planus and pes cavus populations. Computed tomography (CT) was used to create bone models that were imported into the virtual environment. Joint coordinate systems were developed to measure kinematic changes in the hindfoot during weight-bearing gait and quiet standing. The objectives of this thesis were to (1) implement a fluoroscopy-based markerless RSA system on the foot, (2) determine the effect of various orthotics at midstance of fully weight-bearing dynamic gait, and (3) determine the effect of orthotics as measured using three different techniques. The hybridization of magnetic resonance imaging (MRI) with positron emission tomography (PET) or single photon emission computed tomography (SPECT) enables the collection of an assortment of biological data in spatial and temporal register. However, both PET and SPECT are subject to photon attenuation, a process that degrades image quality and precludes quantification. To correct for the effects of attenuation, the spatial distribution of linear attenuation coefficients (μ-coefficients) within and about the patient must be available. Unfortunately, extracting μ-coefficients from MRI is non-trivial. In this thesis, I explore the problem of MRI-based attenuation correction (AC) in emission tomography.

One of the challenges in the designing of clinically-relevant vascular substitutes is our lack of understanding on how vascular smooth muscle cells (VSMCs) and vascular endothelial cells (VECs) interact in the graft. The aim of this study was to examine the factors that play a role in VSMC and VEC interaction in 3D co-culture. Highly porous 3D poly(carbonate urethane) scaffolds were fabricated using a solvent casting and particulate leaching method. VSMCs and VECs were co-cultured for 48 hours. The objective of this research project was to develop a drug delivery system for recombinant human erythropoietin (rHu-EPO), a glycoprotein hormone used in the treatment of renal anaemia and chemotherapy induced anaemia, using the biopolymer chitosan as the base component. Two types of chitosan nanoparticles were produced through ionotropic gelation using flush mixing with either tripolyphosphate (TPP) or carrageenan polymer. Guided bone regeneration is a medical procedure which induces in vivo re-growth of bone using membranes and osteopromoting fillers. In this work, bacterial cellulose fibers were isolated and used as a basis for biomimetic hydroxyapatite growth, with the ultimate goal of producing GBR filler materials. Acetobacter xylinum generated BC using various carbon sources. Fibers were treated with phosphoric acid to phosphorylate functional groups and preconditioned with calcium to nucleate the HA. Conventional needle positioning techniques for small animal microinjections are fraught with issues of repeatability and targeting accuracy. To improve the outcomes of these interventions a small animal needle positioning system guided by micro-computed tomography (micro-CT) imaging was developed. A phantom was developed to calibrate the geometric accuracy of micro-CT scanners to a traceable standard of measurement. Use of the phantom ensures the geometric fidelity of micro-CT images for use in image-guided interventions or other demanding quantitative applications.
