Historically, mining has been viewed as an inherently high-risk industry. Nevertheless, the introduction of new technology and a heightened concern for safety has yielded marked reductions in accident and injury rates over the last several decades. In an effort to further reduce these rates, the human factors associated with incidents/accidents need to be addressed. A modified version of the Human Factors Classification and Analysis System (HFCAS-MI) was used to analyze lost time accidents and high-potential incidents from across Queensland, Australia and fatal accidents from the United States of America (USA) to identify human factor trends and system deficiencies within mining. …

Kentucky Transportation Center Research Report
The Kentucky Transportation Cabinet’s Division of Maintenance is responsible for identifying and prioritizing locations in need of guardrail. A procedure used by the Kentucky Transportation Cabinet was originally developed by the Kentucky Transportation Center in 1989 and updated in 2002. Based on frequent usage by the Kentucky Transportation Cabinet of the guardrail location rating system database, the need was identified to update the existing database and incorporate improved reporting capabilities. Numerous factors may contribute to high-severity crashes in highway work zones. Identifying these factors and then alleviating their impact is a challenging task that traffic engineers and researchers have to confront. In this study, the work zone risk factors that could increase the probability of causing fatalities when severe crashes occur were examined using a comprehensive approach. The researchers first identified the significant risk factors based on a screening process that incorporates both statistical analyses and empirical research findings. Falls are the leading cause of injury deaths among people 65 years and older. The National Safety Council reported that in 2005, 17,700 Americans met their death by falling, and of these deaths, the majority (over 80%) were people over 65 years of age [1]. It is certainly desirable to avoid the fall accidents altogether through developing a comprehensive fall prevention program [2]. However, in case of unavoidable falls, an effective injury-prevention technology is critical to minimize/reduce fall-related physical injuries. Recently, the concept of wearable airbag [3] emerged as one viable and promising injury-prevention approach.

The Work Zone Safety and Mobility Rule (Rule) was published in the Federal Register in September 2004 indicating that all state and local governments receiving Federal-aid funding were required to comply with provisions of the Rule no later than October 2007. Kentucky received an extension to October 2008. According to documented statistics, intersections are among the most hazardous locations on roadway systems. Many studies have extensively analyzed safety of signalized intersections, but did not put their major focus on the most frequent type of intersections, unsignalized intersections. Unsignalized intersections are those intersections with stop control, yield control and no traffic control. Unsignalized intersections can be differentiated from their signalized counterparts in that their operational functions take place without the presence of a traffic signal. Reduction of crash occurrence on the various roadway locations (mid-block segments; signalized intersections; un-signalized intersections) and the mitigation of injury severity in the event of a crash are the major concerns of transportation safety engineers. Multi lane arterial roadways (excluding freeways and expressways) account for forty-three percent of fatal crashes in the state of Florida. Significant contributing causes fall under the broad categories of aggressive driver behavior; adverse weather and environmental conditions; and roadway geometric and traffic factors. KENTUCKY’S TRAFFIC COLLISION FACTS report for 2009 is based on collision reports submitted to the Kentucky State Police Records Branch. As required by Kentucky Revised Statutes 189.635, “every law enforcement agency whose officers investigate a vehicle accident of which a report must be made...shall file a report of the accident...within ten days after investigation of the accident upon forms supplied by the bureau.” The stated purpose of this requirement is to utilize data on traffic collisions for such purposes as will improve the traffic safety program in the Commonwealth.
