There are multiple dam failures and dam safety incidents every year in the United States. There are currently no standard policy requirements for when or how to conduct failure or incident investigations. This paper first reviews one of the most significant failures in US history, the failure of Buffalo Creek Dam No. 3 in order to explore the impacts of one significant failure on dam safety policy in the US. This study proposes several decision-making tools utilizing optimization and machine learning frameworks to assess and improve the safety of the workplaces. The first chapter of this study presents a novel mathematical model to optimally locate a set of detectors to minimize the expected number of casualties in a given threat area. The problem is formulated as a nonlinear binary integer programming model and then solved as a linearized branch-and-bound algorithm. Several sensitivity analyses illustrate the model's robustness and draw key managerial insights. The Air Force employs ejection seats in its high-performance aircraft. While these systems are intended to ensure aircrew safety, the ejection process subjects the aircrew to potentially injurious forces. System validation includes evaluation of forces against a standard which is linked to the probability of injury. The Muti-Axial Neck Injury Criteria (MANIC) was developed to account for forces in all six degrees of freedom. Unfortunately, the MANIC is applied to each of the three linear input directions separately and applies different criterion values for each direction. The purpose of this project is to evaluate the safety and operational differences between three left-turn treatments: permitted, protected, and protected-permitted left-turn phasing. Such a framework can enhance the crash safety of lithium-ion batteries by investigating prevailing failure mechanisms such as thermal runaway. The Highway Safety Manual (HSM) lists four different methods for determining the change in crash frequency in order of reliability. Currently, the Utah Department of Transportation (UDOT) uses the fourth reliable method. The goal of this research was to develop a tool that the most reliable method mentioned in the HSM could be used to perform life-cycle benefit-cost analyses. A spreadsheet program was built that performs the HSM's Part C Predictive Method for 11 different roadway segment types mentioned in HSM using Excel macros and Visual Basic for Applications (VBA) programming. This thesis contains the results of the development of crash prediction models for curved segments of rural two-lane two-way highways in the state of Utah. The modeling effort included the calibration of the predictive model found in the Highway Safety Manual (HSM) as well as the development of Utah-specific models developed using negative binomial regression. The data for these models came from randomly sampled curved segments in Utah, with crash data coming from years 2008-2012. The total number of randomly sampled curved segments was 1,495. This thesis documents the results of the calibration of the Highway Safety Manual (HSM) safety performance function (SPF) for rural two-lane two-way roadway segments in Utah and the development of new SPFs using negative binomial and hierarchical Bayesian modeling techniques. SPFs estimate the safety of a roadway entity, such as a segment or intersection, in terms of number of crashes. The new SPFs were developed for comparison to the calibrated HSM SPF. In order to determine which roads can most benefit by the implementation of access management techniques, a prioritization process was developed to recommend various access management treatments such as limiting access points, installing raised medians, and ensuring adequate signal spacing along corridors. Advance warning signals installed upstream of a high-speed signalized intersection (HSSI) warn motorists of impending signal changes in an effort to reduce the frequency of red-light running (RLR) and crashes. A new advance warning signal design was tested on an approach to an HSSI in Utah to study the effects of the modified design on motorist behavior. The new design utilized an overhead dynamic blank-out sign and flashers.
