The research-to-practice gap of approximately 15 years is due to many factors, one lacking clinician’s evidence-based practice (EBP) knowledge, skills, and competency. Evidence-Based Practice is a problem-solving process incorporating external evidence, clinical expertise, and patient preferences, guiding clinicians to integrate evidence into practice. Fostering a spirit of inquiry empowers nurses to engage in the EBP process to solve clinical practice problems and improve patient outcomes. A system-wide practice change for filtering IV fluids led to a wondering question about the best practice for filtering intravenous parenteral nutrition in the neonatal population.

Central Shared Governance focused on the strategic goal of promoting nurse health and well-being. A survey found interrupted or skipped meal breaks were common on inpatient units. We reviewed the literature and found that the majority of nurses experience workplace stress and that adequate breaks and break areas can decrease stress. Central Shared Governance identified this common occurrence as being misaligned with our nurse health and well-being goals.

According to the CDC’s (2019) review of health care data in 2019, over 150 million patients were seen in Emergency Departments across the United States. Of these patients, more than 20 million were admitted to the hospital. Longer Emergency Department wait times equate to worse patient outcomes, including higher in-hospital mortality rates and increased occurrence of in-hospital cardiac arrest.

In response to the pandemic, electrocardiogram (ECG) rhythm interpretation education transitioned to an online, self-paced course with optional instructor-led virtual labs. Challenges included technology issues, limited learner engagement, inefficient use of instructor time, and low pass rates. Lab capacity was 30, but average attendance was 5.5. Prescheduled tutoring required instructor availability with only 25% of offerings attended. Legacy learning management system permitted multiple test attempts leading to competency concerns and inaccurate passing rate data.

Acute kidney injury (AKI) following the use of contrast agents is a known complication of coronary angiography. The National Cardiovascular Data Registry (NCDR) defines AKI as an acute increase of > 0.3mg/dl or a 50% or greater increase in the serum level of creatinine from baseline and cardiac patients are at high risk for AKI. At a Midwest quaternary medical center, the risk adjusted AKI rate was 8.63% (Q3, 2021) which is above the national average of 3.46% (NCDR).

Background: Critical illness and the PICU environment may contribute to the development of Post Intensive Care Syndrome (PICS) in children, their parents, or their siblings. PICS has a negative impact on quality of life, including development of PTSD, reduced school performance, or delay in meeting developmental milestones. Diaries have been used to create context and memory after discharge, extending cognitive and emotional support beyond the PICU stay. Entries are staff-written in letter format, including details about the patient’s day, events, or feelings. Background and/or rationale

Labor pain is unavoidable and can be the biggest determining factor of the labor experience. In labor, a patient’s anxiety stimulates the sympathetic nervous system and releases stress related hormones, which increases the severity of pain. Nonpharmacologic interventions can decrease anxiety and the perception of pain. Benefits to nonpharmacologic strategies include sense of control when patients actively manage their pain, lack of serious side effects, minimal expense, and ease of use. Lavender aromatherapy can decrease labor anxiety while also decreasing pain perception.

Background: Multiple types of shift report exist, a “blended” bedside shift report incorporates verbal face to face between staff with a bedside component. Baseline audits in March 2023 showed during bedside report only 3% addressed codes status, 3% oxygen, 6% access, 3% safety, and 12% tubes and drains. Additional baseline data from March 2023 showed 71% compliance with No CPR band application and one fall. Within a virtual Nurse Residency Program, the opportunity existed to increase engagement among nurse resident participants. Program facilitators self-reported low level of engagement citing, “difficult to get engaged,” “offer activities for conversation,” and “allow time for engagement activities.” Literature suggests participants may feel uncomfortable engaging in virtual environments. Thus, a Rewards Program was developed to encourage engagement of participants and piloted in ten Nurse Residency cohorts.

In 2022, approximately 15,000 Registered nurses and 6,000 Nursing assistants were onboarded utilizing paper checklists across 26 hospitals. The 95 identified checklists required printing multiple pages and was cumbersome for leadership to monitor and access.

Nursing Education and Professional Development launched an electronic orientation checklist initiative. The checklists transitioned to electronic platform delivering an updated, comprehensive, and collaborative approach to system orientation resources. An unplanned extubation is a common adverse event for an intubated neonate that can lead to negative outcomes such as hypoxia, ventilator-associated pneumonia, extended hospital stays, and risk of injury. Prevention is a unique challenge in the NICU population due to small facial anatomy, increased ventilation days, and environmental factors.

Local Problem

A level 3 NICU experienced a total of five UEs in 2022 which equates to a rate of 3.59 per 100 ventilator days compared to a rate of 0 to 1 in previous years.

Background: The most common reported complication of diabetes mellitus is diabetic foot ulcers. Outpatients with diabetic foot wounds are at an increased risk for all cause hospitalization. Moreover, diabetics socioeconomic status has been shown to influence health outcomes associated with disease management.

Local Problem: At a Midwest quaternary medical center, within the outpatient wound clinic and Aurora Limb Preservation Program (ALPP), the 30-day hospital admission percentage for patients with diabetic foot ulcers was elevated in January 2022 (16%) and February 2022 (19%).

Background

Malignant hyperthermia (MH) is a potentially fatal reaction to a volatile anesthetic or succinylcholine producing a hypermetabolic state. Prompt recognition and treatment of MH are essential for survival.

Local Problem

Rapid Response Team (RRT) leaders identified a nursing knowledge gap about the process for treating MH for patients outside high-risk areas at a level 1 trauma center.

Method
