Assessing Patient Acceptance Of Integrating Oral Care With Prenatal Care In A Safety Net Clinic, Iyabo Obasanjo, William Mann

Iyabo Obasanjo

Background: Lack of adequate oral health has been associated with adverse pregnancy outcomes, and both the American Dental Association and the American College of Obstetrics and Gynecologists advocate oral health care for women while pregnant, especially women served by health centers serving the underinsured and uninsured.

Objective: A Rural, Safety Net Clinic, which provides healthcare to medically underserved persons in the Williamsburg area of Virginia, began offering dental care to pregnant women accessing prenatal care from January 2015. We want to document the clinic’s experience with patient acceptance of the free dental care offered.

Methods: We reviewed data on the …

Two million children experience sibling death annually and have problems that require clinical intervention although few receive such help. Effects on surviving siblings’ mental health has been well documented, however their physical health has not. This study described surviving siblings’ illnesses, treatments/health services at 2, 4, 6, and 13 months post-sibling death. The 132 children (76 girls, 56 boys, M 10.6 years, SD 3.43); 30% Hispanic, 51% Black, 26% White were recruited via hospital ICUs and published obituaries. Using a longitudinal design, parents reported types and numbers of surviving siblings’ illnesses, treatments/health services, and dates post-sibling death. Most of the …

Purpose

To (a) describe women’s prenatal and postpartum problems and advanced practice nurses (APN) interventions; and (b) determine if problems and APN interventions differed by women’s medical diagnosis (diabetes, hypertension, preterm labor).

Design and Methods

Content analysis of 85 interaction logs created by APNs during a randomized clinical trial in which half of physician-provided prenatal care was substituted with APN-provided prenatal care in the women’s homes. Patients’ problems and APN interventions were classified with the Omaha Classification System.

Findings

A total of 212,835 health problems and 212,835 APN interventions were identified. The dominant antenatal problems were physiologic (59.2%) and health-related …

Grandparent Health & Functioning After A Grandchild’S Death, Joanne M. Youngblut, Dorothy Brooten, Kathleen Blais, Jean Hanna, Theophile Niyonsenga Aug 2019

Grandparent Health & Functioning After A Grandchild’S Death, Joanne M. Youngblut, Dorothy Brooten, Kathleen Blais, Jean Hanna, Theophile Niyonsenga

JoAnne Youngblut

Approximately 160,000 grandparents experience the death of a grandchild each year; this represents a permanent, irrevocable loss for the grandparent resulting in physical and emotional responses. Grandparents who lose a grandchild experience increased alcohol and drug use, thoughts of suicide, and pain for their adult child who is also grieving. Supportive resources available to grieving grandparents, the effects of the grandchild’s death on the grandparent-parent relationship and the influence of race/ethnicity on grandparent grieving are discussed. Despite about 40,000 child and infant deaths each year, knowledge about grandparent health and functioning following the death of a grandchild is limited.

Consequences And Management Of Canine Brachycephaly In Veterinary Practice: Perspectives From Australian Veterinarians And Veterinary Specialists, Anne Fawcett, Vanessa Barrs, Magdoline Awad, Georgina Child, Laurencie Brunel, Erin Mooney, Fernando Martinez-Taboada, Beth Mcdonald, Paul Mcgreevy

Paul McGreevy, PhD

This article, written by veterinarians whose caseloads include brachycephalic dogs, argues that there is now widespread evidence documenting a link between extreme brachycephalic phenotypes and chronic disease, which compromises canine welfare. This paper is divided into nine sections exploring the breadth of the impact of brachycephaly on the incidence of disease, as indicated by pet insurance claims data from an Australian pet insurance provider, the stabilization of respiratory distress associated with brachycephalic obstructive airway syndrome (BOAS), challenges associated with sedation and the anaesthesia of patients with BOAS; effects of brachycephaly on the brain and associated neurological conditions, dermatological conditions associated …

Chemical Hypersensitivity: Inconsistencies In Global Research Perspectives And Healthcare Practice, Tahni Mcgaughy

Tahni McGaughy

All over the globe, an escalating number of people are developing distressing sensitivities to our environment’s everyday intoxicants. The experience of chemical hypersensitivity (CS) is embodied by proposed definitions of multiple chemical sensitivity (MCS) and idiopathic environmental intolerances (IEI), among other names. These acquired illnesses are characterized by a wide range of symptoms and reactions to low-level chemical exposures. Studies conducted in Japan, USA, Sweden, and Denmark found that 12% of the population has been diagnosed with chemical hypersensitivity, while total underreported prevalence is estimated between 15% and 27%. More importantly, research states that prevalence has increased by 300% within the …

Women’S Experiences With Prenatal Care: A Mixed-Methods Study Exploring The Influence Of The Social Determinants Of Health, Karen A. D'Angelo, Janelle K. Bryan, Brenda Kurz

Janelle K. Bryan

Background & Purpose: Racial and ethnic disparities pervade birth outcomes in the United States and the state of Connecticut. While Connecticut’s infant mortality rate is less than the national average, rates for the state’s Black/African American and Hispanic/Latino communities exceed it. This study explored how prenatal care in Connecticut may be enhanced to address these disparities.

Methods: In spring 2013, seven focus groups and two semi-structured interviews were conducted (n=47). Participants also self-administered brief surveys. Recruited by local service providers, participants were 18 or older, pregnant and/or in the first year post-partum at the time. Most self-identified as …

Biting The Hands That Feed “The Alligators”: A Case Study In Morbid Obesity Extremes, End-Of-Life Care, And Prohibitions On Harming And Accelerating The End Of Life, Michael J. Malinowski

Michael J. Malinowski

Obesity, recognized as a disease in the U.S. and at times as a terminal illness due to associated medical complications, is an American epidemic according to the Centers for Disease Control and Prevention (“CDC”), American Heart Association (“AHA”), and other authorities. More than one third of Americans (39.8% of adults and 18.5% of children) are medically obese. This article focuses on cases of “extreme morbid obesity” (“EMO”)—situations in which death is imminent without aggressive medical interventions, and bariatric surgery is the only treatment option with a realistic possibility of success. Bariatric surgeries themselves are very high risk for EMO patients. …
