Total knee arthroplasty (TKA) is the only solution for treating arthritis of the knee joint. Although it is successful at reducing pain and returning function to affected joints, one in five patients still report dissatisfaction following their operation. Bicruciate stabilized (BCS) TKA was developed to improve outcomes by replicating normal knee structure and function. The biomechanical, functional, and clinical outcomes for the BCS design were investigated in this thesis through radiographic imaging techniques, wearable sensor systems, and questionnaires in a cohort of TKA patients.

Within recent years there has been an increase in the number of anterior cruciate ligament (ACL) injuries within female athletes (Learning how ACL, 2022). The ACL is a ligamentous structure within the knee that connects the femur and the tibia (Mayo, 2022). Its primary function is to provide stability, prevent the tibia from excessive anterior translation, and provide proprioceptive input for the lower limb (Mayo, 2022). Patellofemoral pain syndrome (PFPS) is a common form of anterior knee pain caused by general overuse, muscular weakness or strength imbalances, and poor movement coordination leading to abnormal alignment or mal-tracking of the patella. Recent guidelines do not recommend the use of a knee orthosis due to a lack of evidence supporting their effectiveness. Patellofemoral pain (PFP) is the most common knee injury among runners, military personnel, adolescents and recreationally active persons. PFP commonly presents as diffuse pain across the anterior knee that increases with activities such as running, squatting, and walking up and down stairs. Nearly 75% of patients with PFP either alter or completely stop physical activity as a result of the pain. There are numerous mechanisms that contribute to PFP symptoms; such as muscle weakness, abnormal joint mechanics and abnormal joint stress, which make it difficult to successfully treat. Adolescent Idiopathic Scoliosis, a three-dimensional deformity of the thoracolumbar spine, affects approximately 1-3% of patients ages 10-18. Surgical correction and treatment of the spinal column is a costly and high-risk task that is consistently complicated by factors such as patient-specific spinal deformities, curve flexibility, and surgeon experience. The following dissertation utilizes finite element analysis to develop a cost-effective, building-block approach by which surgical procedures and kinematic evaluations may be investigated. The purpose of this research is to identify abnormal gait parameters or patterns amongst young adults with multiple disabilities enrolled in a work transition program sponsored by a public-school system. Gait disorders are commonly seen in individuals with neurologic disorders, with significant research in children with autism. Gait disorders have been linked with fall and injury risk, with significant research in elderly populations. Purpose: Anterior cruciate ligament (ACL) injuries are one of the most common sports-related injuries as they account for around 100,000 injuries per year. This study sought to review research on prophylactic knee bracing (PKB) for the reduction of the risk factors related to injury of the ACL.
Methods: An extensive search of articles from 1990-2021 was performed with inclusion criteria of no previous ACL injury, no knee injury or surgery in the past 6 months, the use of a hinged knee brace, and use of kinetic, kinematic, and proprioceptive variables. This research aimed to determine if participants of varying anthropometrics (i.e., height) would perform automotive manufacturing tasks significantly differently based on joint angle and center of mass (COM) data. Twenty-four participants between the ages of 18 and 65 (¯x = 24.1 years, SD = 7.3) completed 14 manufacturing tasks, and their motions were recorded using two inertial motion capture (IMC) systems. Participants were chosen based on their measured height, including the 5th percentile female (Group 1-5F), 50th percentile female (Group 2-50F), or 95th percentile male (Group 3-95M) (Fryar, Gu, Ogden & Flegal, 2016). Previous studies have demonstrated that satisfaction and revision rates following total knee arthroplasty (TKA) are lower than those of comparable surgeries such as total hip replacements. A leading cause for these revisions is joint instability which may be due to improper ligament balancing or poorly aligned surgical implants. One of the methods used to investigate biomechanical forces and kinematics is computational modelling of the post-operative TKA knee.

Core strengthening and postural stability are desired outcomes of certain therapeutic exercises performed in horses. This study aimed to quantify changes in muscle activation at a walk and trot in horses traveling over eight consecutive ground poles evenly spaced (at 30 inches for walk and 48 inches for trot) in parallel fashion in a straight line, and with hindquarter and abdominal elastic resistance bands applied at 25% stretch. Surface electromyography (sEMG) data were collected for the longissimus dorsi and rectus abdominus muscles in six horses. A consensus, detailed understanding of carpal kinematics remains elusive. 4-dimensional CT (4DCT) is a validated modality capable of accurately studying in-vivokinematic motion. The objective of this work is to quantify normal, in-vivo kinematic motion of the carpus through a flexion-extension arc of motion using 4DCT. Ten healthy, un-injured volunteers underwent a 4DCT scanning protocol through a complete arc of flexion-extension motion. Kinematic changes in motion were quantified using helical axis motion data for each carpal bone. Helical axes were compared between bones and statistical analysis performed using repeated-measures ANOVA to identify difference in kinematic motion between bones (p<0.05). The carpus can be divided into four main kinematic blocks: the distal carpal block, the proximal carpal block and individual scaphoid and trapezial blocks. This work supports an additional segmentation of the trapezium from the distal carpal row, which suggests some modulation between the scaphoid and distal carpal row.

Total knee arthroplasty (TKA) continues to be a successful procedure for the past 50 years. We investigated the kinematics after under- and overstuffing the joint space with the polyethylene (PE) insert in both cruciate-retaining (CR) TKA and posterior-stabilized (PS) TKA. We compared CR and PS designs to evaluate their respective kinematic differences. This study employed a hybrid computational-experimental joint motion simulation on a 6 degrees of freedom joint motion simulator. Optimal management of the hip capsule during arthroscopic surgery has not been established. The impact of incisions made to the capsule during minimally invasive procedures on joint biomechanics, and whether repair provides any benefit, continue to be debated. The effectiveness of capsular repair to restore native kinematics may be insufficient. Therefore, a better understanding of joint behavior during various capsule conditions is needed. Ehlers-Danlos syndrome (EDS) is a group of heritable connective tissue disorders, consisting of thirteen different subtypes. Among the thirteen, Hypermobile Ehlers Danlos Syndrome (hEDS) is the most common. Individuals with this condition present with frequent joint instability that results in ongoing subluxations and dislocations. Secondary diagnoses of this condition include chronic pain, Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS), Major Depressive Disorder (MDD), and Generalized Anxiety Disorder (GAD). The current gold standard surgical approach for Total Knee Arthroplasty (TKA) is a medial parapatellar approach (MPA). We aimed to study a novel lateral subvastus lateralis approach (SLA) for TKA and compare patient outcomes and joint kinematics to the MPA.
Patients with neutral or varus alignment undergoing primary TKA were recruited to undergo the SLA. At one year postoperative, patient outcomes (WOMAC, SF-12, KSS) and joint kinematics (using radio stereometric analysis (RSA)) were analyzed. 7 LPA and 7 MPA patients were compared.

Background and Purpose: Stroke is a leading cause of disability that results in various neurological deficits, one of which is hemiplegia. This deficit alters the gait cycle, resulting in decreased propulsion force by plantar flexor muscles, decreased activation of dorsiflexor (DF) muscles, and increased coactivation of antagonistic ankle muscles. Stroke also leads to altered somatosensory input which results in decreased balance and gait speed, ultimately increasing fall risk. The purpose of the current study was to gain a better understanding of speech adaptation by examining kinematic and acoustic adaptation to bite block perturbation over time. Fifteen native American English speakers (7 female, 8 male) with no history of speech, language, or hearing deficits participated in the study. Custom bite blocks were created for speakers which created a 10mm interincisal gap when inserted. Dementia of the Alzheimer’s type is the most common form of dementia, affecting millions of people worldwide. Specific genetic mutations increase Alzheimer’s disease-related neuropathology and cognitive impairments. Deficits are observed in spatial orientation throughout the progression of Alzheimer’s disease, including wandering behavior or becoming lost in a familiar environment. Dementia of the Alzheimer’s type patients also exhibit disruptions in radial optic flow, which underlies self-movement cue processing. Currently, no treatments exist for Alzheimer’s disease. Broadly, these methods are used to measure kinematic or kinetic variables exhibited by the lifter, the barbell, or the lifter-barbell system. The current study examined the lingual kinematic and acoustic effects of bite blocks on vowels in a sentence context. Twenty adult native English speakers (10 male, 10 female) with no speech, language, or hearing deficits participated in the study. The corner vowels found in the sentence, The blue spot is on the black key again (i.e., /u/, /É‘/, /æ/, /i/), were measured kinematically and acoustically immediately before and after bite block insertion. Background: Shoulder pain is a common occurrence in the general population. Pain has been associated with shoulder impairments and pathology. Associations between shoulder limitations, impairments, and mechanisms of injury and altered scapular motion appear in the literature. The performance impact of different knee extension angles in the setup position for a backstroke start was evaluated using an instrumented starting surface. Ten backstroke swimmers completed maximum-effort starts in each of two setup positions: one with the knees flexed, and one with the knees extended. The start handles and touchpad were instrumented with tri-axial force plates. Activity of major hip and knee extensors was measured using surface electromyography. Body position was recorded throughout the start using two high-speed cameras. There was no difference in time to 10 m between the two conditions, indicating there is likely no advantage in …

Background: The excitability of the H-reflex pathway in the non-impaired nervous system can be augmented by altering the different parameters of a walking task, specifically slope. We sought to examine the adaptations in soleus H-reflex excitability and foot force control following an acute bout of upslope or downslope treadmill walking in people post-stroke compared to those who are non- impaired. Methods: We recruited 12 individuals with chronic post-stroke hemiparesis and 9 age-similar non- neurologically impaired individuals. The purpose of this study is to evaluate the effectiveness of a self-adherent ankle taping in reducing peak ankle inversion angle and peak ankle eversion moment upon landing during a max vertical jump and depth drop compared to jumping without a prophylactic ankle stabilizer.

The soleus muscle is a monoarticular plantarflexor composed slow-twitch fatigue-resistant muscle fibers. Through its attachment to the proximal tibia, contraction of the soleus muscle in a closed kinetic chain (when the foot is planted) produces a posterior pulling force on the posterior proximal tibia. The anterior cruciate ligament (ACL) is responsible for preventing anterior displacement of the tibia relative to the femur. Through the production of a posterior pulling force on the tibia, soleus muscle contraction in a closed kinetic chain could help reduce strain on the ACL. Low back pain (LBP) is a highly prevalent condition that is often characterized by persistent pain and limitations in the performance of daily functional activities. The repeated use of altered movement patterns during the performance of daily functional activities is proposed to contribute to the development and course of LBP. Specifically, in the case of LBP, the proposed alteration of movement is one in which the lumbar spine moves more readily than other joints that can contribute to the movement. The back squat exercise is a popular resistance exercise that involves synchronous movement of the entire body to perform the movement properly . The two main focuses when considering engaging in the back squat exercise are the risks of injury and the enhancement of performance. There is little information and scant understanding on the way body proportionality impacts the risk of injury and performance.
