Existing research regarding the analysis of the distribution of oral healthcare practitioners has been conducted across large metropolitan areas within the United States in order to determine the magnitude of healthcare resource deficits. Such research has demonstrated significant deficits in access to oral healthcare predominantly in lower-income urban areas. Consequently, patients in these socioeconomically disadvantaged areas have less access to routine oral care, which impacts their overall health and well-being. This dissertation includes two studies of rural age-related services in the state of Utah. The first study combines geospatial, demographic data (number of people over 55) and age-related services (hospitals, hospice providers, nursing homes, senior centers, and Areas Agencies on Aging) at the county level and census-tract level to determine localized differences in proportional access to age-related services. Higher and lower proportions are then predicted by contextual factors including rural/urban gradient, economic industry, and broadband access. Preventive health services and screenings are an important component in the continuum of care provided to individuals across all ages. Yet, research has shown that rural residents generally use fewer preventive health services and screenings. This study used the National Health Interview Survey to examine receipt of preventive health services (cholesterol check, fasting blood sugar test, mammogram, pap smear, and receipt of the HPV vaccine) by rural and urban women over the age of 18. Methods: This study examines the impact of race and rurality on health differences, including prevalence of prediabetes and type 2 diabetes (T2D), among overweight children accessing pediatric endocrine specialty care. Cross-sectional analysis of overweight/obese youth 10 to 18 years of age receiving pediatric endocrinology consultation for weight gain, hyperglycemia, and/or T2D from 2013 to 2016 at a Midwest tertiary pediatric center.

Results: The 722 patients were 42% white, 25% black, 22% Hispanic; 88% lived in urban areas and 12% in rural areas. Rurality was determined using zip code approximation of Rural-Urban Commuting Area (RUCA) codes. After adjusting for confounders, black …

METHODS: A literature search for studies relevant to homelessness and dental care was conducted. The PRISMA and ENTREQ guidelines were followed. Little research has investigated the barriers and facilitators to accessing healthcare for African, Caribbean, and Black people in Canada. This thesis identifies barriers and facilitators using a socio-ecological framework in two papers. Five focus groups were held with 22 ACB participants in Waterloo, Ontario. Thematic analysis guidelines informed the analysis of focus group data. The first paper identified barriers at the social (racism and employment, immigrant status) and institutional (wait times, out-of-pocket expenses) levels.
