Background: Advance care planning (ACP) is a discussion of a patient’s end-of-life healthcare wishes. Its purpose is to indicate these wishes clearly should the patient be unable to make their decisions known to providers and family members due to a sudden health decline or medical emergency. Healthcare providers, specifically nurse practitioners, physician assistants, and physicians in the primary care setting, are well-positioned to facilitate these conversations with their older adult patients. However, research states that limited provider knowledge of and confidence in implementing ACP are significant barriers to its application in practice.

Attention Deficit Hyperactivity Disorder (ADHD) is a highly prevalent, chronic condition with symptoms that include inattention, hyperactivity, and impulsivity. ADHD affects six million children in the United States (Tran et al., 2021). The purpose of this evidence-based practice project was to implement a holistic office protocol that addressed the chronicity and future health outcomes of children and adolescents, ages 2-17 years with ADHD through stimulant or non-stimulant medication prescriptions, behavioral therapy referrals, and education surrounding diet, and exercise. Access to primary care has a direct impact on morbidity and mortality, and is strongly influenced by indirect waiting time: the delay between the requested and allotted appointment day. Our models describe the heterogeneous appointment seeking patterns of a primary care patient panel using stochastic processes parameterized to reflect the diversity of primary care visit rates in the US. For capacity planning, we estimate the distribution of daily appointments, and show that the distribution variability can be reduced by heuristics that use patient flexibility regarding the day of the appointment. Hospital readmissions cost the U.S. healthcare system approximately $17.4 billion each year. Minimizing hospitalizations among Medicare beneficiaries is therefore a high priority. This study aimed to evaluate how identifying patients at high risk of hospital readmission—i.e., patients with complex health needs, such as those with chronic conditions—and referring them to a transitional care program impacts readmission rates in a primary care clinic.
