Problem: Falls are considered never events, yet continuously occur in the inpatient setting. Falls,

especially falls with injuries, impact the patients, the staff, and the hospital. Falls cause extended lengths of stay, affect the morale of the patients and the staff, and are non-reimbursable events.

Context: There have been an increased rate of falls within a medical/surgical/telemetry unit

microsystem at a Northern California hospital despite standardized screening and prevention tools. The unit can house up to 52 patients and is the designated Covid unit of the hospital at the time of this project.

Problem: According to the World Health Organization (2020), medication errors are one of the leading causes of injury and avoidable harm in health care globally.

Context: The county jail houses approximately 700 inmates/patients on a daily basis, and the potential for error when administering medications is high as medications are prepared manually using a paper medication administration record.

Interventions: The jail began using an electronic health record/barcode administration system on October 19, 2020.

Equine-assisted services incorporate the interaction of humans who face mental, physical, emotional, and/or social challenges and equines for therapeutic purposes. Recreational, physical, mental, social, and/or emotional goals are met through various equine-assisted services such as therapies, equine-assisted learning, and horsemanship.

The safe freeway merging and lane-changing operation for fully Autonomous Vehicles (AVs) in mixed traffic (i.e., the presence of AVs and non-AVs in the traffic stream) is a challenging task. The AV merging and lane-changing operation could increase crash risks and reduce operational efficiency significantly in mixed traffic.

This dissertation quantifies the freeway merging crash risk and develops a freeway merging decision strategy based on crash risk assessment for an AV attempting to merge in the target lane with non-AVs. Typical road users appear not to understand retroreflectivity despite nightly exposure to retroreflective materials such as road signs. While retroreflective surfaces have been engineered to reflect light back in the direction of its source, retroreflectors appear to be diffuse reflectors in most viewing conditions. This may lead road users to underestimate its value in nighttime roadway applications and contribute to vulnerable road users’ under-use of retroreflective markings to enhance their own conspicuity at night. A Quality Improvement (QI) project in a Midwestern Continuing Care Residential Community (CCRC) was designed to examine the evidence-base of four assistive technology platforms marketed as safety platforms as means to improve outcomes for older adults residing in memory care environments. Using the Evidence-Based Design Model Socio-Technical Systems Theory, survey data and observational data collected from the organization were considered with respect to the evidence-base supporting each technology. The Occupational Safety and Health Administration (OSHA) reports that over two million workers are victims of workplace violence (WPV) (Papa, 2013). Registered nurses are subjected to high risk for workplace violence from patients and visitors, with 25.5% reporting at least one victimization incident (Gillespie et al., 2013).
